On 4/26/2015 while examining the SCBA's on the Main North, he tripped over a cable bolt and fell on his left arm. He was taken off work in 8/5/2015 for surgery on shoulder.

Employee was standing in the entry when he heard what he thought was a rib falling. He turned to move away and tripped over a lump of coal along the rib. As he fell his foot caught on this lump, preventing his foot from rotating with his lower leg resulting in a twisted knee.
The employee was operating a roof bolter, installing angle brackets. He had his foot on the drill mast so he could reach to install the bracket. His foot slipped causing him to fall. He caught himself with his left hand, causing him to strain his shoulder. He sought medical attention on 1/15/2014. He has been held out of service pending an MRI.
As the employee stepped to the ground his ankle rolled, causing him to fall to the ground. The employee stated that he misjudged the distance from the cab to the floor and resulted in a sprained ankle. A follow up visit with an Orthopedic consult identified that a fracture had re-occurred. The ankle had been fractured in an off the job event about a month ago.
Employee slipped while racking drill pipe in the derrick board, he stepped between the fingers in the rack causing him to fall backwards striking his back on the board, resulting in soreness in his back
The employee slipped on muddy bottoms while carrying supplies. He attempted to catch himself with his right arm. The arm struck a blunt steel object during the fall. The employee suffered a fractured forearm.
The employee slipped and fell in extremely muddy and slick conditions on grade. The result was a twist of the employee's right knee. No medical attention was necessary at the time of the incident. The employee has experienced continued pain from the incident and has gone for x-rays and has been issued meds and set up for physical therapy. No lost time.
