The fatally injured miner and co-workers were in the process of building cribs during recovery of a longwall shield when unconsolidated rocks fell through the roof mesh and struck the miner.
A miner was fatally injured when they were pinned between an air-lock equipment door and a concrete rib barrier located near the shaft bottom.
Was removing a shield leg jack when the jack swung around and hit EE's right ankle, catching both legs between the cell plate the shield foot.
Walking across the 650 duckbill to grab cribs for relay bar. EE stepped off the duckbill into a hole and twisted EE's left ankle.
A roof fall occurred at District #4 Headgate #1, #2 Entry, Crosscut #62 in the Intersection outby the Section Loading Point.
A small distortion was found on the rope during the slope hoist rope examination at the 355 foot mark behind the brakeman car.
Getting rope off of cable, EE hit an airline, the force of the air knocked EE to the ground injuring EE's right knee. EE was taken off work on 5/18/2026 for surgery.
Helping install the rub rail on the side of a new miner when right index finger got pinched between the frame and rail, causing to receive stitches.
While attempting to hook the chain to the supply trailer at the slope bottom to unhook the trailer, the car rolled back and hit EE in the right ankle causing a small cut that required stitches.
While cutting bands off a bundle of screen wire, as they were stepping off the back of the bolter, their right wrist made contact with the edge of the screen wire causing a laceration on inner right wrist. Causing them to receive stitches.
While changing the intermass jack on the #411 Roofbolter, they removed the top jack pin and had a long punch holding the top eye of the jack. They removed the bottom jack pin, as they were trying to remove the jack it slipped causing left pinky finger to be pinched between the stem and the eye of the jack.
During examination of the Slope Hoist rope, a distortion was found on the rope at the 220 foot mark behind the Brakeman Car. Upon further investigation a faulty rope roller was found at the Slope Turnout at the 220 foot area off the mine bottom.
While roof bolting operator's pusher steel cross threaded in the drill chuck. Operator attempted to dislodge the drill steel, the drill steel was bent, the steel kicked back striking operator in the hardhat causing a laceration. Operator received stitches.
While cutting a can of glue, knife slipped and cut left index finger, causing them to receive 3 stitches.
As EE stood up getting off the mantrip, EE felt a sharp pain in EE's right knee. EE went to take a step and EE's knee failed causing EE to fall to the ground.
A roof fall occurred inby the loading at #58 crosscut, 2-Right on the District #4 Tailgate.
While picking up a belt spreader off of a belt structure trailer, EE felt a sharp pain in EE's neck and right shoulder.
While using an Emu to pull lifeline. EE grabbed the lifeline to break a zip tie, when EE grabbed the lifeline it wrapped around EE's ring finger.
The Magnetic brakes on the back set of rail trucks on the brakeman car were damaged. A bolt was found broken on the brake bracket.
While cutting the bottom of a chute out, slag landed on the torch hoses melting them. While climbing out of the chute to shut off the oxygen and acetylene tanks, EE strained EE's right foot.
While cribbing up the headgate shield, a crib got dropped on EE's hand smashing EE's right index finger, causing EE to receive stitches.
The rock dust got delayed and then dumped all at once causing the drop hose to slap EE in EE's left wrist and forearm, fracturing EE's wrist.
A roof fall occurred at crosscut #27 between #2 and #3 entries on Headgate #1 damaging the return stopping.
The crosshead for the Single Drum Lining Hoist became caught on top of a concrete pipe bracket that extended from the shaft liner 8.5" at 388' as the hook continued to be lowered down to a depth of 500'. The crew moved the work deck back to center the crosshead fell off the concrete pipe bracket and fell down to the headache ball causing 6' of damage to lower end of the hoist rope.
While employee was cutting a piece of belting on the roof bolter that was attached to the ATRS arm, with their right hand using a belt knife, the knife slipped and cut their left forearm causing them to receive stitches.
EE was getting a crib off of the head drive when a rock fell out from between the shields, striking EE's left hand.
Bending over picking rocks up, EE felt a pop above EE's groin. EE was taken off work on 05/28/2025
While shoveling on the slope belt, EE saw a rock bounce off the top belt and land on the belt rail. EE tried to pick it up, but it was heavier than EE thought. The rock fell towards the bottom belt and smashed EE's left ring finger and pinky finger on the top roller frame.
Rockfall/adverse roof conditions due to abutment pressures at crosscut #31, #3 entry, blocking access off of the longwall tailgate.
While climbing up on a diesel tractor EE felt a pop in right shoulder. EE was taken off work on 04/09/2025
EE was drilling a hole in the top when the steel clogged. EE hit the steel with a 1' wrench. The wrench popped back up hitting EE in the mouth, splitting EE's lip as well as knocked out 2 of EE's teeth.
EE was standing on the shield. When the shield went into auto pull it came in to contact with EE's right foot, pushing EE's foot into the pan line. Was taken off work on 02/20/2025.
When removing the scraper, the frame that had been unbolted fell down, hitting EE's right upper leg and right knee cap. EE was taken off work on 02/19/2025.
Walking back from #2 entry at crosscut #42 to the longwall face, they slipped in a wet product from the pumpable cribs, twisting left ankle.
While reclaiming cables and hoses from the roof for the panline, they had their left hand under the cables to support them. The force of the falling cables and hoses slammed their left hand down against the panline.
EE was placing hog panel on ATRS when it got bound up, EE pulled on it and felt a burning sensation from right shoulder down to EE's bicep.
While traveling to #1 entry in a shuttle car, EE hit a bump in the haulage road, causing EE to strike EE's head on the shuttle car canopy. At the time EE felt pain in mid-chest and mid back area.
While building a double block wall, EE lifted a block up and was trying to get it in place when it rolled and landed on EE's right pinky finger. EE was wearing black cheetah gloves at the time of incident. EE was taken off work on 01/20/2025 for surgery.
While lifting block for building wall EE experienced pain in EE's groin and hip. EE was taken off work on December 13, 2024 for surgery.
Unloading pump ride, they grabbed pallet from back of ride, as they were pulling the pallet a Victaulic coupler that was underneath pallet gashed their left-hand palm area. Causing them to receive stitches.
Operator was backing miner out of cut, they went to reach their tools on the machine and a rock fell off the rib and smashed their hand. Causing them to receive stitches to the top of their right hand.
While moving a trailer hitch out of the slope yard track area, EE felt pain in EE's left side lower back. Was taken off work on 11/20/2024
Helping another crew member load props in a scoop bucket, EE threw EE's end of the prop in the bucket and it kicked back, smashing EE's ring finger between the bucket and the prop, causing EE to receive stitches.
While working on a conveyor chain under a shuttle car, when getting up off the ground, hard hat fell off and they tried to catch it, causing them to lose balance and fall head first into the shuttle car, causing a laceration which required staples.
While pulling a 72" bottom frame from the slope track through the compartment mandoor EE strained EE's lower back. Was taken off work on 10/17/2024
A rock came off the drum of the shearer and hit them in the face, causing them to receive stitches to their chin.
Assisting belt crew during a belt extension on the D3HG8 section. While installing a bottom roller the bottom roller bracket was not sitting flush enough with belt rail to install the "J" bolt. EE kicked the bottom roller bracket with right leg trying to get bracket in place and felt a pop in left knee.
Employee and another crew member were hanging miner cable in the #5 entry. When they lifted the cable to hang it, employee pulled something in their left shoulder. Continued to work until 7/18/2024 When employee was taken off work.
While working on the North side of the slope belt at Arch 737, EE stepped on mandoor, the door shifted causing EE to fall through. EE landed below on the slope track, lacerating left arm causing EE to receive stitches.
Checking water sprays on feeder as they turned to walk away rolling their foot. As they tried to catch themselves, they twisted their left wrist. Fracturing the wrist.
While looking over right shoulder as EE was backing out of the x-cut, EE turned into a bundle of cribs on the left side. The cribs pushed into EE's lower left leg fracturing EE's left ankle.
Trying to get the U-joint out of shuttle car by hitting it with a hammer. The hammer bounced off and struck EE's left thumb, causing EE to receive stitches. EE was wearing gloves at the time of incident.
Cutting bands off of dust hose, EE lost footing on the shield foot. When EE slipped EE cut self on left forearm while trying to catch self, causing to receive stitches.
Diesel Scoop was pushing shuttle car from the dump end, EE was in the shuttle car with left hand on the door frame. The bucket of the scoop slid across the cab of the shuttle car, pushing the door against EE's hand, mashing left ring finger causing EE to receive stitches. Employee was wearing gloves at the time of the incident.
Filling up supply insert, while transferring 7' roof bolts from one supply insert to another, EE stepped on some loose roof bolts laying in insert causing EE to slip and fall. EE tried to catch self with left hand, EE's hand went between the pile of loose bolts and the side insert, jamming EE's left hand and wrist.
While removing shield to shield cables, EE was cutting the cable ties holding the cables to a trunk line. EE's left hand was holding the trunk line and cutting the cable tie, as EE pulled through the cable tie the blade struck in in the forearm causing EE to receive stitches.
EE was drilling a hole in the mine floor with a sinking hammer drill to install an anchor pin for the take-up track structure. When they finished drilling the hole, the drill steel became stuck. As they were pulling on the hammer drill to free the stuck drill steel, it pulled free causing their right thumb to be smashed.
In the process of loading the last clean up car in the #3 entry on the left side of the cut. They positioned the miner cable on the rib and started loading the shuttle car. As employee was loading the car, a rock 40" long, 8" thick and 8" wide fell from the roof at the left rib line and cantilevered back striking them in the head and back area.
While wearing cut resistant gloves and holding a bag of rock dust, an employee cut the bag of dust and accidently cut EE's right thumb, causing EE to receive stitches.
While trying to break a bolt free on a cover of the roof bolter, the wrench slipped and EE's hand jammed into the frame of the bolter, fracturing EE's left thumb.
A base lift cylinder rolled off the landing, Employee and a co-worker lifted jack back up on landing and employee felt a pull in their stomach. Was taken off work on 1/12/2024 for surgery to repair hernia.
While making a splice, left hand was underneath cable while cutting top. Once blade went over end of cable, the knife blade cut top of left hand. EE received stitches.
While stepping down off the stopping EE was building, EE stepped on a pin board causing EE to lose EE's balance and strained EE's left knee.
Getting off the Getman tractor with 3 points of contact, EE stepped down and knee gave out. EE continued working until 11/14/2023 when EE was taken off work.
While attempting to step up on trailer, when EE placed EE's left foot on the side of trailer, EE's foot slipped off hyperextending left knee. EE continued working EE's normal duties until 12/13/2023 when EE was taken off work.
Reclaiming ventilation tubing, tubing fell hitting EE on the head. Was taken off work on 9/9/2023.
Loading oil on stinger ride and twisted knee.
Was assisting in function testing shields. EE was on shield #44 and hit base lift button on adjacent control. #43 base lift pressure hose was blown off base lift, hose came out of shield and struck EE near EE's right eye.
One of the two hydraulic jacks broke at the weld on the slope track collar ventilation doors preventing the slope brake car from being able to completely exit the mine.
After taking a Simplex Jack down while assisting in bolting of the Longwall face, EE turned away and EE's right foot slipped into the pan line chain sideways. When EE pulled their foot out, EE heard a pop.
An unplanned roof fall in #7 entry, at crosscut #59 to crosscut #60 in the Main North return was found by Mine Examiners. The fall measured 19 ft. wide, 12 ft. in height, and 120 ft. in length. Area was mined 06/19/2015
While loading a Strata prop on to a shuttle car EE's glove caught on the wire of the prop. EE couldn't get hand away from the prop as the prop dropped on the shuttle car causing it to smash EE's right ring and middle fingers.
A small rock fell between shield #68 and #69 hitting EE in the left thumb, causing a laceration in which required stitches.
The employee was walking and slipped on slickline, twisting lower back. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.
Left side bolter lost suction bolter on right side. Employee went to help, on the way back around, on the back side of the bolter, they slipped on a muddy rock and hyperextended their right knee. Was able to finish shift. They continued working and was taken off work on 7/20/2023 by doctor.
While shoveling on LW shields, EE put a hand on cable tray to steady self as shields were being moved. Rocks fell out between the shields and landed on top of EE's right hand.
EE was on longwall to help shovel shield feet. EE began to walk back toward the headgate, turned head to speak with someone and struck head on a shield canopy that was positioned a little lower cutting right brow. EE received stitches.
Walking down belt line beside the monorail and stepped in a hole, felt something pop in right foot. Was taken off work on 04/19/2023
The injured miner was riding in a mantrip. As they were backing up, a bolt on the passenger seat broke, causing the miner to fall from the mantrip where EE was struck in the lower leg by the front wheel.
While assisting a group of guys cleaning shield feet on the face, as EE was walking around, one of the guys threw a rock striking EE in the face. EE received stitches in the left eyebrow.
Putting in belt structure during belt move, EE stepped in hole and felt left knee pop.
Walking at Power Boxes, EE stepped on the miner cable with left foot. Lost footing and twisted left knee, lost balance and fell to the ground.
Loading top of Head Drive with crib block. Carrying two cribs on EE's left shoulder, EE lost EE's footing causing EE to fall forward. The cribs slid forward from EE's shoulder hitting EE's right middle finger against the side of the drive.
An unplanned roof fall between #7 and #8 entry of the #7 entry of the Main North Travel Road was found by the pre-shift mine examiners. The fall measured 8 ft. in height, 18 ft. in width and 60 ft. in length.
Elevated CO levels were detected at the walkable bleeders near the district #2 bleeder fan shaft. Upon investigation it was determined the source of the elevated CO was located at the District 2 Longwall Panel 8 tear down face.
Employee was pulling down a hanging piece of tin when the tin ripped and cut employee's thumb. Had surgery and received stitches.
Carrying a roll of hose towards the tailgate, EE stepped wrong, straining EE's right knee.
Was loading cribs on head drive up high. Stacking cribs 4 high when a top crib fell off the stack, hitting EE's right ring finger.
Bent over and picked up high voltage cable, felt something pop in lower back.
The Hoistman was sending down a longwall shield loaded on a shield car that was attached to the Frontier-Kemper Brakeman Car. As the shield car and the brakeman car traveled through the top switch above the slope collar, the brakeman cars front wheels off tracked and hit a track guide rail, causing damage to the brakeman cars front wheels.
Was walking to 5 right overcast, lost footing and knee popped. EE was taken off work on 09/28/2022.
Carrying a piece of tubing in mud, EE slipped and dropped the other end of tube, EE felt a muscle pull in EE's lower back.
EE was running tractor, went to turn in to crosscut 50 to get out of the way of the water wagon. EE realized there was a trailer already in the crosscut, started to back-up out of the crosscut with EE's fingers resting over the guard plate at the articulation point, which resulted in crushing and amputation of two fingers.
Unplanned roof fall at crosscut #17, entry #4, on the West Submains, at spad Station #31655. Fall dimensions 19' wide, 20' long, 6' high. Fall did not cause injury, impair ventilation, or entrap miners. Fall area developed on March 1st, 2018. Reported to MSHA at 6:35PM on 08/21/2022.
EE had just taken the bucket off the Scoop. While backing outby the scoop canopy hit the air outlet on the 4" compressed airline which caused the airline to be separated from the victaulic coupler. This caused the rock dust in the entry to create zero visibility condition. EE continued outby and the 4" line entered the cab of the scoop and struck the operator in the head.
The employee was trying to cut a link off of a chain so the chain would fit a lock box. Employee was holding the chain with channel locks and cutting link with a battery powered grinder. The grinder slipped off of chain and cut their left hand causing them to receive stitches.
While offloading structure on the belt line, EE felt a burning feeling on the inside part of right shoulder. EE was taken off work on 08/31/2022 for surgery.
When shutting equipment door, EE's finger was smashed by the walk thru door. Fracturing EE's finger.
While cutting pump hose, EE was kneeling down on one knee cutting it. The knife came out of the hose striking left thigh area causing EE to receive stitches.
While lifting belt up to install belt structure during a belt move, EE felt a sharp pain in EE's back.
EE was changing a broken hydraulic fitting. When the hose broke off the fitting, EE felt a pain in the right elbow. EE continued working and was taken off work for surgery on 5/20/2022 for a tear in the bicep.
While walking to the face to get tools to put away, EE slipped next to the stage loader and felt left knee pop. EE continued to work until all test results were received, EE was then taken off work on 5/23/2022.
Loading belt structure on to a 5th wheel trailer being pulled by a getman. EE saw an additional top roller as the trailer was exiting the crosscut and while attempting to toss the roller on the trailer EE placed EE's foot in-line with the 5th wheel trailer tire and the tire ran over the end of EE's Met steel toe boots. Resulted in 4 broken toes.
The mine monitoring system on the surface identified elevated CO at sensor #108, located at the LW TG. Upon investigation, flames were discovered where expansion foam had been applied in a roof cavity near #50 shield at the LW face. A miner immediately applied a fire extinguisher and quickly suppressed the flames. The area was then cooled with water and firefighting foam.
EE had removed the Range Arm pin and was tossing it up on the shield. As EE tossed it, EE did not get it as high as needed and caught right pinky finger between the pin and shield causing a laceration and fracture.
EE was standing by the miner at the 7-Right raise bore shaft area, a rock ricochet off the gob/rock area at the shaft hole and propelled back 75' to EE's location striking EE in the left wrist causing EE to receive stitches.
EE complained of pain in EE's right leg, back and neck as a result of riding as a passenger in a mantrip along the roadway. The roadway was being graded to increase the height in preparation for the upcoming longwall move. The miner finished EE's shift. After consulting with a doctor EE was taken off work on 3/2/2022.
The employee was checking the silicate level in one of the tankers when the wind blew the opened tanker lid closed on the the employee's left thumb. X-rays revealed a small fracture. Employee was wearing metacarpal gloves at the time of the incident.
EE was walking away from the face area. EE stopped and bent over to get in dinner bucket and was struck by the battery scoop.
EE pulled on hog panel to free it up on the bolter, EE felt a pop in right shoulder.
Shoveling on the belt on the back side of stage loader. When finished shoveling EE crossed the belt at the crossover outby the stage loader and jumped down about 3 feet. When EE's feet hit the ground, EE felt a pain in the right hip and lower back area.
Unplanned roof fall between crosscut #14 & #15 #1 entry of the Main North #2 return inby spad station #2387.5. Supported with 7' TR Bolts and 12' cable bolts. Fall dimensions - 17' wide, 20' long, 7' high. Fall did not cause injury, impair ventilation or entrap miners. Fall area developed on 1/8/2019. Reported to MSHA at 6:47 AM on 09/08/2021.
Unplanned roof fall at crosscut 49 1/2 #4 entry, station #6414.5 on the Main North Belt line. Fall occurred in a pillar line supported with 8' T.R. bolts and 11' cable bolts. Fall dimensions 18 ft. wide, 22 ft. long, 9 ft. high. Fall did not cause injury, impair ventilation or entrap miners. Fall area developed on 03/22/2015. Reported to MSHA @ 8:32 PM on 08/11/2021.
Loading belt structure on trailer and felt pain in left shoulder. Was taken off work on 9/24/2021 for surgery.
While lifting a 2" pump onto a mantrip with the help of a coworker, having to lift the pump high over a tool box, EE felt a strain in right shoulder. Was taken off work on 8/13/2021 due to restriction of no use of right arm.
While unhooking staging beams threw chains up on mule, felt a pinch and pain in groin area. Continued working up until surgery on June, 11, 2021.
While installing roof bolts in #3 entry employee had just finished tightening up a bolt. Employee was resting the starter steel on top of left foot, while dropping the boom down, to set up for next hole. The steel caught a bolt on side of pot, while it was coming down driving the steel through foot.
Was cutting an old belt to make belt skirts. Employee was pulling the blade towards themselves and the knife pulled through the cut and across right knee on the outside, causing a couple inch laceration. Employee received 2 stitches and 10 staples.
While pulling a piece of angle iron on top of the North Sub Main I.L.S., employee smashed left pinky finger between the angle iron and the head frame at dump. Received stitches to the left pinky finger.
Laying track for the mule train on 9 Left setup. EE stuck a 4' pry bar in the hole on the end of a 33" rail trying to stand the rail up to bolt to another rail, the bar slipped and the rail rolled back to its original position causing the bar to strike EE's chin. This caused a laceration in which EE received stitches to EE's chin.
Hanging miner cable and tie wire came loose twisting EE's arm back. Causing EE to have surgery on March 3, 2020 to repair tendon.
Changing bits on the miner, struck right pinky finger with hammer, causing them to receive 3 stitches.
Was getting out of scoop when EE's left foot slid under the control panel in the cab and stepped out of the cab with EE's right foot. EE's right foot rolled and EE lost EE's balance falling to the ground with EE's left foot still in the cab under the control panel.
While walking by the miner their feet slid out from under them, when they reached out to catch themselves with their left hand, wrist hit a miner bit on the head of the miner.
Moving the wash down hose across the top of the monorail. The wash down hose got stuck, when EE tugged on the hose it came on and whipped, striking EE in the mouth causing a broken tooth and stitches.
Helping remove an impact roller from the magnet area. As EE was walking away with the roller, EE tripped on a waterline and fell with the forearm hitting the bracket that the roller mounts in. Received stitches to the arm.
Loading a piece of belt hand rail onto sled, it slid down striking EE's finger catching it between another rail. EE received stitches to the left ring finger.
Shoveling on the belt line, employee lifted a rock and one side was heavier than the other which resulted in the rock to slip and pin employee's index finger between the rock and discharge line. Fracturing the right index finger.
Removing yoke from a wet brake with a hammer and punch. A piece of the hammer broke off and hit EE's index finger on the left hand. On 7/19/2019 EE had to have a small piece of metal removed from the finger causing EE to receive stitches.
Employee reported on 6/10/2019 that on 5/29/2019 while loading cable on pump car, they felt a sharp pain in their neck. Became LWD on 8/8/2019 due to surgery.
Pulling a pontoon jack off of the tail when the comealong came loose from the chain hanger striking employee on the side of head cutting right ear. Received stitches to ear
Cutting metal banding on a stack of hog panel, when the band broke, it cut middle right finger on top of the knuckle. Received stitches to finger.
The shaft hoist trailing cable was caught by the limit switch bracket and damaged the cable.
While splicing cable, EE's hand slipped when cutting outer jacket, cutting left hand on the outer index knuckle. Resulting the need of stitches. Infection set into wound, resulting in surgery on 5/14/19, has not returned to work due to recovery.
EE exited the road grader to clean off the camera. When walking around machine, EE slipped catching themselves with left arm on the grader. EE felt pain in the shoulder area. EE was taken off work May 1st for surgery.
Dragging pressure hoses, EE had a hose on right shoulder, EE twisted to the left so EE could grab hose with left hand when EE felt pain in lower back. Was taken off work on 07/24/2019 to have back surgery.
On 1/30/2019 while breaking apart 10" waterline. The pipe swung over and struck EE's hammer while EE was holding it, jamming right elbow. On 2/11/2019 EE was trying to remove bit from miner head, missed bit with hammer causing further pain to right elbow. On January 8, 2020 EE became a Lost Work Day injury due to having surgery on right elbow.
Picked up 2 cribs over EE's shoulder and as EE lifted them over EE's shoulder EE felt a pull in EE's right groin area and dropped the cribs when EE felt the pain. EE was diagnosed with a hernia and taken off work Friday, January 18th for surgery becoming a Lost Work Day incident
Was driving down the West Submains in a small diesel golf cart when EE ran over a rock. The steering wheel started to spin catching EE's hand between ring finger and pinky finger causing a laceration between the two fingers. Resulting in stitches in hand.
Employee was changing belt rollers. They dropped roller on right hand finger tip. Required 17 stitches.
EE was repositioning a belt cutter to splice the belt. The cutter was hung up on belt. When ee grabbed the belt cutter, fingers were under the cutter. The blade on the cutter was not fully retracted and lacerated left ring finger. EE received stitches in finger.
While striking an allen wrench with a sledge hammer in effort to loosen the bolt on the miner gear case, the allen wrench flew off striking employee in the left hand. Employee received stitches in their finger.
While operating the shear, EE was standing on the shield foot when a rock discharged off the shear cutting head striking EE in the left arm. EE received staples in the arm.
While washing the Ram Car at the shop, employee stepped over the center section of the car and slipped in the hole on the bed of the Ram Car, and fell striking head on the bed of the car. EE was taken to hospital and received staples in the head.
When turning on 1 1/2 miner hose to wash down the slope, the hose slipped out of employee's hand and due to pressure the hose started whipping from side to side striking employee on the left leg.
Employee was picking up scrap metal and a jack fell and struck EE on the right side of EE's back. EE stated that a jack had come loose for an unknown reason. At that time EE was taken to the emergency where EE received 17 stitches and was released.
Making the first cut of 2-L when EE was struck by a rock in the back of the head and neck. EE was seen at the Hospital and was released to work. EE continued working then was seen by EE's personal doctor due to having vision problems and was then taken off work beginning 9/17/2018 with the diagnosis of a concussion.
Was handling block, twisted knee and felt a pop. This incident became a medical treatment incident on 9/14/2018 due to having surgery to repair knee.
While swinging a sledge hammer, ee slipped and twisted right knee causing a strain. Became Medical Treatment due to having surgery on 7/26/2018.
Was pulling water fill hose and slipped in mud, falling down, felt a pull in lower back. Employee continued to work until April 10th in which employee was then taken off work for surgery.
Was helping change out scoop battery, picked up jumper cable and tripped, causing them to fall in to the scoop battery. Employee tried to catch themselves, their hand slid on top of battery striking lock.
EE was trying to remove a pinner steel hung in the roof. While pulling and pushing the steel to fit the pinner chuck, and hit the wrong control lever causing right ring finger to be pinched between the pinner steel and the pinner steel clamp.
While taking canopy post off, the cylinder collapsed and caught EE's fingers between the post collars. Received stitches to left index finger.
Employee was walking down the slope, stopped to turn around and double check area. Employee tripped on a chunk of coal and fell forward hitting right hand on the top roller.
Unplanned roof fall @ xc #79 1/2, entry #1, 5-L beltline. Fall occurred in the pillar line between xc #79 and xc #80 area was supported with 7' TR bolts and 100 ton heizman jacks with cross bars. Fall dimensions - 30 ft. long, 19 ft. wide, 8 ft. high. Fall did not cause injury, inpair ventilation, or intrap miners. Fall area developed 02/11/2017 to 02/13/2017
Loss of control panel communication for the hoist operators control panel on the shaft hoist. It was found that 2 small breakers were knocked on a breaker box at the shaft collar.
EE was operating articulating truck. Truck was loaded, traveling to S-pile. Truck dropped into rut. Hit edge of rut jolting truck, ee bumped arm rest of seat injuring Rt lower back.
Unplanned roof fall @ xc #6, entry #2, station #750, 7-L travelroad. Fall occurred in an intersection. 4-way, supported with 7' TR bolts, 12" cable bolts, 100 ton heintzmann jacks + bars. Fall dimensions - 25 ft. long, 18 ft. wide, 12 ft. high. Fall did not cause injury, impair ventilation or entrap miners.
EE was supervising personnel drilling holes at an angle above the LW pillar for Jennchem glue insertion to help stabilize the strata in advance of the LW face. EE was struck and covered by rock that fell between the permanent roof supports and broke through the wire mesh. This employee received a broken ankle, pelvis and ribs and remains off work.
Was down on knees digging a hole to set prop, a piece of material struck them in the back of the head. This incident became a Lost Time injury on 10/26/2017 due to becoming unable to perform essential job functions assigned to them.
Was walking down crosscut, dragging tubing, they slipped and fell towards the rib cutting the left arm on bolt and plate. They received stitches to the upper left arm.
Was walking out secondary escapeway, going over the overcast, when employee stepped down employee hyper extended right knee. Employee was taken off work and became a Lost Work Day injury on 10/30/2017 due to unable to perform essential job functions and is scheduled for surgery.
Employee was replacing bent rails and spreaders, was putting tops in place to bolt down, the top roller flipped catching employee's right hand between the rail and top roller. Employee received 2 stitches in right finger
Was loading the high voltage wheel rack on a power move on 6 Left. The forks on the lowtrack were not positioned to fit the wheel rack. While attempting to move forks further apart, employee's fingers were mashed between the fork and the mast. Resulting in two fractured finger tips & stitches in one of employee's fingers.
A rock came off of the car and struck employee on the foot, fracturing the right 4th toe.
Employee was filling up duster at fan. Employee had right hand sitting on top of the dust hopper on the fan when a small piece of loose rock came off of corner between roof and rib striking employee's hand, causing employee to receive stitches
Was driving the scoop and hit a hole or rock causing the scoop to slam. This caused EE's teeth to hit together breaking EE's back right tooth.
EE was walking through crosscut to make the on shift of the faces. EE stepped on a crib tie, the crib tie rolled and EE's knee twisted and buckled. EE had surgery on knee on September 21, 2017 and was taken off work at that time for recovery.
Hitting a bit with hammer and a piece of the hammer came off striking EE in the hand. Medical Treatment due to receiving stitches.
Employee was standing in front of the miner, lifting cable and water line off hooks on the side of the pan. When they attempted to lift the cable and waterline they both were tighter than thought, as they pulled their arm they had a ripping feeling and let go. On 6/16/2017 they received stitches due to surgery.
While packing steel bar, employee tripped causing the piece of material to fall on employee's hand, cutting the index finger on employee's right hand. This caused employee to receive stitched.
Unplanned roof fall at XC #42, entry #2, station #6218, 4-L Headgate. Fall occurred in an intersection, 4-way, supported with 8' TR bolts and 12 ft. cable bolts (star pattern). Fall dimensions - 25 ft. long, ,19 ft. wide and 12 ft. high. Did not cause injury, inpair ventilation or intrap miners. Fall area developed 5/4/2017 to 5/7/2017. Reported to MSHA at 7:20 am 5/10/2017.
EE was changing out bret-by on the shear carrying two pieces at a time from the head drive to the mid-face. While getting through the head gate shields with the bret-by ee fell forward, leg stayed between the shield foot, causing a pop along with pain in right knee. *Became medical treatment due to surgery on 5/26/17*
While building a crib, a co-worker slid another crib into place. The injured employees hand was caught between the two cribs resulting in a fracture to the tip of left middle finger. The employee missed two shifts then returned to full-duty.
Employee was struck in the head by a pinner steel, knocking ee back toward the pinner walkway, ee was diagnosed with a concussion, as well as received 5 staples in head.
Employee was dropping the miner cable, the cable fell down and hit a hand against a pie pan and cut the middle finger. Employee received stitches in the left middle finger.
Unplanned Roof Fall at xc#52, #4 entry station #6835 on the Main North belt. Fall occurred in an intersection, 4-way supported with 8' T.R. bolts. Fall dimensions - 17' wide by 30' long and 10' high. Fall did not cause injury, impair ventilation or entrap miners. Fall area developed 4-25-2015. Reported to MSHA at 4:55 AM on 3-8-2017.
While picking up the roof bolter cable to hang, the operator of the roof bolter trammed the pinner forward, the pinner cable reel stuck causing a strain in employee's left shoulder area. Employee was taken off work on 04/28/2017 for surgery.
Employee was operating a Getman tractor pushing a sled around the corner, employee grabbed the outside of the cab to pull self up in the cab seat. As the center section (pulling end) of the Getman contacted the cab (motor end), it smashed employee's hand between the two sections.
While flushing the bull hose at the end of the water line at the feeder, ee felt a pain in left arm. A sharp end of a belt splice pin extending out of a splice cut left forearm requiring stitches.
Employee was cleaning up the walkway and picked up a spill board that had coal on it. While bending over to pick up the piece of steel, employee turned and felt a pain in the chest. Was taken off work by doctor beginning 2/6/2017.
After finishing bolting the 1st turn of the left handed crosscut, employee was loading the bolter tray with roof bolts. As employee bent over to grab the bolts, a rock fell out between the pins of the bolted crosscut. Striking employee in the right shoulder area knocking employee forward.
While cleaning out trash with a mini-trac, ee was backing out of the crosscut a cable bolt flew up from the engine tire and struck ee in the right shoulder and head. This incident resulted in a fractured collarbone and 4 stitches in forehead.
A roof bolter operator had just installed the 1st pin in a rib strap on the outby end. EE unhooked the strap on the inby end, as EE let go the strap sprung out and down hitting EE on the side of the foot. On Dec. 21st EE was diagnosed with second and third metatarsal fractures and was taken off work.
Trip / Struck against. On Nov 29th, 2016 the injured miner was walking to the mantrip that was parked in the crosscut between entry #1 and entry #2. Miner stepped on uneven footing and stumbled about 10 feet, falling into the seat rail of the mantrip. Striking left ribs and cutting left arm. Resulting in fractured ribs.
While hanging 30" tubing employee felt a strain in their back. Employee was taken off work beginning 2/4/2017 by their doctor to complete an occupational therapy program.
While pulling tubing down to take to the face, finger got caught at the joint between two pieces as they were coming down, partially amputating the tip of left ring finger.
While driving pins out of the 3 Left air doors a piece of metal broke off of hammer and imbedded in left forearm. The metal was removed on 10/21/2016 and stitches were required at this time to close the incision.
Employee was putting a conveyor chain back together on a continuous miner. While beating the conveyor link back together a piece of metal struck employee, cutting left leg in which stitches were required.
Unplanned roof fall at xc#63, #2 entry on the 4-L travel road. Fall occurred in a intersection (4-way, supported with 8' tr bolts, cable bolts - 16' and truss bolts. Fall dimensions 18' wide, 35' long and 12' high. Fall did not cause injury, impair ventilation or entrap miners. Fall area developed on July 21st-22nd of 2015. Reported to MSHA at 12:52 AM 07/26/2016.
While watching a skid being pulled/sheaved out of the crosscut, employee's footing became unstable and EE accidentally grabbed the rope. The left hand became caught in between the rope and the sheave.
Unplanned Roof Fall at xc #106, #3 entry station #16773 on the 3-L tailgate. Fall occurred in an intersection (4-way), supported with 8' TR bolts and truss bolts. Fall dimensions - 18' wide x 30' long x 8' high. Fall did not cause injury, impair ventilation or entrap miners. Fall area developed 04/07/2016. Reported to MSHA at 8:55 pm on 05/08/2016.
While operating a mini track, EE bumped a crib, dislodging the crib tie. The crib block struck the employee in the head, in which EE received stitches.
A piece of metal broke off in EE's hand while handling a track tie. EE had to have it cut out and received stitches.
Unplanned roof fall @ in #2 entry, ft. 1536 on the 4-L unit MMU-004. Fall occurred inby the intersection at + 1536 supported with 8' TR bolts. Fall dimensions 17" wide by 20" length x 8.5" to 9 ft. high. Fall did not cause injury, impair ventilation or entrap miners. Fall area developed the week of March 21, 2016.
On 2/26/2016 EE was hooking a welder trailer to a ride, when it tipped over from the center pulling EE to the ground. On March, 14th EE was taken off work due to no longer able to perform regular job duties due to this incident.
EE was walking the long wall face. When EE crossed shield to get to the tailgate entry, EE fell, striking arm on a sharp rock, causing a laceration.

EE was cutting sheet metal when a sharp piece of rock fell between the screen wire striking EE on the nose.
