Employee was climbing the ladder to the 901 Dust Collector, at the top, hand grip slipped causing the employee to fall forward causing a laceration to arm pit on the metal latch for the safety chain. The laceration required the employee to undergo a procedure to close the wound.

Mostrando los 28Contractor employee was cleaning the east walkway of the 731-B conveyor. While EE was moving material EE slipped on the uneven material EE was cleaning up. As EE fell EE reached out with EE's left hand and hit the moving conveyor. EE was taken to the local hospital and received stitched on two fingers. Ongoing investigation in progress to why the guard was removed.
Employee stepped backward, tripped on the angle iron frame of the kiln cooling fan base. While the employee was falling, they extended right arm trying to brace the fall. The employee landed on the concrete/metal pier pedestal. The employee felt pain in right shoulder and right arm. MRI results were confirmed on 3/16/2020
As employee was climbing down a ladder, EE missed the last step and sprained right ankle.
On 1/18/2017 employee was walking and slipped in the mud and jarred shoulder after couple of weeks of shoulder not getting any better. An MRI was performed, results were given on 2/24/2017 and diagnosis was two torn rotator cuff tendons
Employee suffered a non-occupational, non-reportable stroke. EE was taken to the hospital for treatment where EE received stitches for a slight wound sustained when EE collapsed from the stroke. These stitches did not result in a lost time injury.
Accident happened on 12/18/2013; employee was walking and slipped on some clinker then fell landing on her right ankle. Employee continued to work seeking no medical attention until 1/10/2014 an x-ray was taken and indicated a fibulas fracture.
On 12/12/2013 at or around 1:00 AM Employee was climbing up the step of the 4001 Haul truck, he fell forward striking his right knee on the bumper of the truck. Employee continued to work as the days passed his knee kept bothering him so he went to the Doctor on 1/9/2014 Doctor placed him on light duty, starting 1/13/2014.
EE was removing a pin on a bucket and he lost his footing and slipped.
The ee states he was using a small step ladder to add oil to the sweeper he operates. States his foot slipped off the ladder, causing him to step to the ground with all his weight on the left leg. He sprained his left knee, but didn't seek medical attention until 11/19/2007. He was placed on restricted duty with more followup exams scheduled to determine the extent of injury.
The ee states he was rushing up a stairwell to check on a bad hydrualic leak and when he made it to the final step he felt a pain behind his left knee. The ee requested a medical evaluation the following day, 10/19/07, and was placed on restricted duty, due to a sprained knee.
Cutting brace near shaft and stepped into hole. Miner fell 32 feet. Bruised hip and broken wrist.
The employee fractured his right wrist when he was knocked to the ground by falling wooden pallets. The employee and a coworker were stacking and relocating pallets on the north side of the packhouse, when the coworker lifted pallets causing them to topple. The employee was treated on the DOI and release on restricted duty status pending further treatment.
The employee was working on the coal pad when he stepped into a pothole and twisted his right knee. The pad was unlevel due to recent rains. He reported the incident to his foreman 2 days later and went to the doctor to receive treatment. The doctor prescribed medication and scheduled a MRI to determine the extent of injury. The employee was placed on restricted duty.
EMPLOYEE WAS WALKING DOWN A STEEL STAIRWAY WHEN HE SLIPPED & FELL BACKWARDS. THE STEPS WERE WET FROM RAIN THAT WAS FALLING & THE WORKERS BOOTS HAD MUD ON THE SOLES. HE GRABBED THE HANDRAIL TO STEADY HIMSELF & STRAINED HIS BACK. THE EE DID NOT ASK FOR MEDICAL ASSISTANCE UNTIL THE MORNING OF 7/02/04 & WAS PRESCRIBED MEDICATION & PHYSICAL THERAPY.
INJURED WORKER WAS WALKING ACROSS STEEL GRATING TO GRAB AIR LANCE LEANING ON HANDRAIL. HE PUT ALL WEIGHT ON LEFT LEG, AND LEFT KNEE CAP DISOCATED AND HE SLUMPED DOWN ON THE PLATFORM AND CALLE D FOR HELP.
INJURED EE WAS BEING SHOWN ACCESS TO THE TOP OF COAL BINS AS PART OF HIS TRAINING. AS THEY DESCENDED A SHORT STAIRWAY TO THE TOP OF BIN, EE WAS IN THE LEAD. HE OVERSTEPPED A STAIR AND PART OF HIS HEEL LANDED ON THE STAIR. AS HE LIFTED HIS OTHER FOOT HIS WEIGHT SHIFTED TO THE UNSUPPORTED FOOT AND IT SLIPPED OFF THE STAIR. HE GRABBED HANDRAIL AND JERKED HIS ARM BACKWARD.
THE INJURED WORKER GOT OUT OF HIS HAUL TRUCK TO CHECK THE FUEL GUAGE LOCATED ON THE SIDE OF THE FUEL TANK. AS HE WALKED ON THE MUDDY GROUND HE SLIPPED. HE STUCK OUT HIS HEAD TO STEADY HIMS ELF. HE HIT THE TIP IF HIS LEFT MIDDLE FINGER AGAINST THE FUEL TANK CAUSING THE FIRST KNUCKLE TO BECOME SORE.
EE WAS USING A 20 LB. JACKHAMMER WITH A SPADE BIT TO BREAK UP HARDENED CLINKER DUST SPILLED UNDER THE CEMENT CLINKER BINS & FEED CONVEYORS. HE LIFTED THE JACKHAMMER TO MOVE TO ANOTHER AREA & A S HE TOOK A STEP HIS FOOT SLIPPED ON THE RAIN WET SURFACE. THIS CAUSED HIS LEGS TO SPREAD AWKWARDLY AS HE TRIED TO STOP THE FALL. HE 1ST REPORTED TO SUPERVISOR 3/5/03. HERNIA REPAIR SET 3/28.
EE WAS COMPLETING HIS SHIFT AND WALKED OVER TO THE CLINKER LOADOUT BLDG TO CHECK THE BINE LEVERL INDICATORS. THE INDICTORS ARE IN A SMALL ROOM IN THE CENTER OF THE STRUCTURE AND A RAILROAD TRU CK THRU THE MIDDLE OF THE STRUCTURE BUT IS SELDOM USED. POWDERED LIMESTONE DUST IS STORED IS OVERHEAD BINS AND SOME HAD SPILLED OUT ONTO THE FLOOR . IT WAS WET FROM RAINT HAT BLEW THRU THE DO
INJURED EE HAD BEEN USING A CONCRETE VIBRATING RAT-TAIL IN THE PORTHOLE OF AN AIR SLIDE CONVEYOR. AS HE STEPPED DOWN HIS FOOT SLIPPED OFF THE RAILING AND HE FELL TO THE PLATFORM 3 FEET BELOW. HE GRABBED THE RAIL AS HE FELL CAUSING A STRAIN OF THE BICEP MUSCLE AND ROTATOR CUFF. HE ASKED TO BE SEEN BY A DR ON 11-25-02 AND WAS PUT ON RESTRICTED DUTY.
THE INJURED EE WAS SETTING THE BRAKE ON A RAILCAR THAT HAD BEEN SPOTTED. HE WAS STANDING ON THE SMALL PLATFORM AT THE END OF THE RAILCAR WHERE THE BRAKE WHEEL IS LOCATED. IT HAD BEEN RAINING AND THE RAILCAR WAS WET. AS HE PULLED ON THE WHEEL HIS FEET SLIPPED OFF THE PLATFORM. HIS LEFT THUMB WAS BENT BACKWARDS WHEN HE CAUGHT IT IN THEWHEEL SPOKES. HE WAS PUT ON RESTRICTED WITH BRAC
THE EMPLOYEE WAS WALKING DOWN A FLIGHT OF STAIRS WITH A GREASE GUN IN ONE HAND AND AN AIR FILTER AND SMALL OIL CAN IN ANOTHER. AS HE TOOK THE LAST STEP TO THE GROUND HE STEPPED DOWN AND HE SAY S HIS KNEE JUST GAVE OUT, BOWING TO THE OUTSIDE.WHEN HE CAME TO WORK THE FOLLOWING MONDAY, 6-11, THE KNEE WAS SWOLLEN AND PAINFUL TO WALK ON. HEADVISED SUPERVISOR OF INJURY MON.MORNING 6-11.
THE INJURED WORKER WAS WALKING ON AN ELEVATED CATWALK THAT RUNS ALONG AN AIRSLIDE. THE CATWALK IS ACCESSED BY A TWO-STEP LADDER. AS HE WALKED ALONG THE CATWALK HE DUCKED UNDER AN OVERHEAD PIPE BUT FORGOT TO NOTICE THE STEP DOWN. HE FELL ONTOTHE CATWALK LANDING ON HIS KNEE, CAUSING A LACERATION.
EMPLOYEE WAS WALKING FROM PLANT LUNCHROOM TO HIS WORKSITE AT THE SHIPPING SILOS. HE WALKED ACROSS A CONCRETE AREA AND STEPPED OVER AN 8" HIGH CURB. WHEN HIS FOOT LANDED ON ON DIRT HIS ANKLE RO LLED TO THE OUTSIDE AND HE FELL TO THE GROUND. THE NEXT DAY AN X-RAY SHOWED A FRACTURE.
THE WORKER WAS DUMPING THE EXTRA DUST BY OPENINGA SMALL PORTHOLE ON THE SIDE OF THE SCREEN AND LETTING THE DUST POUR OUT INTO A HOPPER ATTACHED TO A FORKLIFT. WHEN THE HOPPER WAS FULL HE NEEDE D TO CLOSE THE PORTHOLE DOOR. HE STOOD ON THE FRONT FENDER, PUT HIS RIGHT FOOT ON THE EDGE OF THE HOPPER AND CLOSED THE DOOR. HE STUCK HIS FOOT AND LOWER LEG IN HOT DUST.
EE S SUPERVISOR HANDED A 60 JACKHAMMER DOWN TO THE EE WHO WAS STANDING ON A PLATFORM A COUPLE OF STEPS BELOW. CLINKER BALLS HAD SPILLED ONTO THE PLATFORM FROM THE FEED END OF 2 FINISH MILL. HE SLIPPED ON THE BALLS AND DROPPED THE JACKHAMMERON HIS LEFT KNEE. THIS CAUSED A CONTUISION AND SWELLING OF THE KNEE. HE WAS PUT ON RESTRICTED DUTY.
INJURED EMPLOYEE WAS ON AN EXTENSION LADDER LEANING AGAINST THE OUTSIDE METAL WALL OF THE ELECTRIC SHOP ASSISTING WITH REPLACEMENT OF A PIECE OFSTRIPPING ON THE ROOF EDGE. HE RE-POSITIONED THE LADDER UNDER THE ROOF'S EDGE BUT DID NOT SHORTEN ITS LENGTH SO IT WAS AT AN ANGLE. LADDER SLID OUT FROM UNDER HIM. LANDED ON HIS FEET BREAKING A BONE.
