Employees on site were completing a swap of a rental screen plant for a company owned screen plant. EE climbed off of the catwalk and onto a discharge conveyor belt. EE lost ee balance and fell to the ground and landed on side.

Light rain was falling, standing water, wet equipment, electrical wiring was color coded incorrectly and wired wrong in the junction box, the ground lead was wired into the buss bar and caused the equipment frame to become energized. Employee set up a metal ladder to inspect equipment and received electrical shock.

EE reported pain in his left shoulder muscle and felt numbness in his left arm and hand.

Employee was working under conveyor and grinding. He remembers his right eye being irritated at the time but did not report the incident until 7-23-07 since it had not cleared up on its own.

Truck driver slipped while stepping to truck box to look at load. He fell to the ground and has pain in lower back and knee.

Employee fell from front end loader ladder while dismounting loader. He fell backwards and injured lower back.

SETTING JACK ON VIBRATORY JARS, TWISTED OR BRUISED KNEE. MAY HAVE KNELT ON ROCK - SPRAINED KNEE.

WORKING IN PIT AREA WITH THE DOZER AND WIND BLEW FRAGMENT IN EYE.

UNHOOKING SCREENING PLANT FROM THE TRUCK/TRACTOR PULLING ON THE 5TH WHEEL PIN WHEN THE PIN LEVER UNHOOKED WITH A SUDDEN SNAP. HE LOST HIS GRIP & BALANCE FALLING TO THE GROUND ON HIS RIGHT SHOU LDER. HE DID NOT SEE THE DOCTOR UNTIL 12/27/02 WHEN THE DOCTOR CONFIRMED A ROTATOR CUFF TEAR. OPERATION CLOSED FOR SEASON.

CARRYING A HYDRAULIC PUMP FOR CONE CRUSHER. STEPPED ON ROCK AND PULLED TENDON IN LEFT ANKLE AND LEFT SHOULDER.
