The loader operator was lowering a piece of metal chained to the bucket onto the top of a rock crushing plant. The loader operator did not see that the accident victim was between the loader bucket and the rock crusher frame.

There were no witnesses to the accident. The operator was found outside of his loader buried under gravel that had sloughed off of the stock pile he was loading from. Fellow employees were attempting to remove the gravel from on top of him when officials showed up. Officials declared him dead at the scene.

A SHELF SUPPORT AT 12 FOTO LEVEL CAME LOOSE COLLAPSING THE SHELF THE BARREL SAT ON. WITNESS LIFTED THE BARREL FROM HIM AND RAN TOT HENEARBY PARTS DE
A FLEET OF SCRAPERS WAS HAULING SUBSOIL TO A STOCKPILE.TWO CAT 651 SCRAPERS COLLIDED HEAD-ON ON THE HAUL ROAD.THE HAUL ROAD WAS WIDE ENOUGH TO HANDLE THE TRAFFIC.THE HAUL ROAD WAS IN GOOD COND ITION AND VISIBILITY WAS GOOD.IT APPEARS THAT THE MAN THAT WAS FATALLY INJURED WAS ON THE WRONG LANE.

A COAL HAULER AND SCRAPER COLLIDED AT AN INTERSECTION WHERE SCRAPER WAS CROSSING HAUL ROAD.
EMPL UP ON PLATFORM ON BOOM OF SHOVEL REMOVING BRASS BUSHINGS,FELL 30' TO GROUND BELOW THROUGH OPEN END OF PLATFORM.FAILED TO USE SAFETY BELT & LINE PROVIDED AT WORK SITE.
IT IS NOT POSSI6LE TO ASCERTAIN THE CAUSE OF THIS ACCIDENT THE ACCIDENT APPEARED AS THOUGH IT MAY HAVE RESULTED FROM A TIRE FALLING FROM SLINGS WHICH WAS USED IN TIRE REPAIR
