Mostrando los 13EE was walking across the drag line bench. EE slipped down. EE states that when EE stood up EE felt pain in the groin. EE was seen at DR the next day. No treatment. EE reports no improvement. Dr listed restrictions that could not be met on 04/12/2018. EE missed first shift on 04/16/2018. Follow up with general surgeon on 04/24/2018.
Employee became overheated at the job site. His supervisor offered him hydration and put him in the truck to cool down. It became apparent that he may need a doctor because of pre-existing medical conditions and the fact that he was not taking his blood pressure medications. The hospital treated for the heat stroke, but kept him for other diagnostics.
Employee placed hand on hot metal, burning first finger on right hand.
Employee's were relocating 55-gallon drums from the "grease rack' area to a storage trailer. They were using a dolly to move the drums. A drum must be moved from the dolly into the trailer, which sits 6" off the ground. When the employees lifted a drum, EE felt something pull in his upper left side area.
I was digging a perimeter drainage ditch in the new pit area. There is some standing water (mabye 2ft deep). This limited the visibility of the actual ditch sides. I apparently got too close to the edge of the ditch and the machine tipped into the ditch, cab side first. The cab strted to fill with water and I (in a panic) kicked out the front window of the cab and crawled through.

Employee was lifting (with a truck-mounted hoist) a piece of cutting edge to be installed on the bucket of the Komatsu WA-500 endloader. When the cutting edge was nearly in place, the pin fell out of the lifting chain, causing the cutting edge to scrape along his thigh and shin.
Was getting onto machine, foot slipped and leg was caught in track and twisted knee.
Employee stated that he was removing rocks from the hopper and felt a snap in his side, he continued to work telling a couple of co-workers, but failed to tell his supervisor untill the following week, EE had been to Dr and thay are stating that he has abd. hernia.
Employee fell after stepping off roadbase catwalk; looked to see how area around tail section had been washed away to allow unit to turn, as he hopped down with his left foot, ground gave away. He turned to his right trying not to fall on conveyor, he fell to the ground on left side. Said that his left lower back was in pain.
