The injured was in the process of removing a rail car from the 8 Right track onto the 3 East Main Line. EE was attempting to align the draw head on the car, when the rail gate slipped loose & fell catching the EE's rt hand between the gate & the draw head, causing a laceration to the palm of EE's right hand and bruise on top of EE's hand. The laceration required 10 stitches.

Injured worker's right thigh made contact with pour chute, this resulted in stitches to the right thigh.

EE was cutting a section of fabric to protect mine phone. As EE proceeded, the knife penetrated the fabric, cutting through EE's glove, resulting in a cut that required sutures.

EE received impact to right eye/face when the sledgehammer EE was using to open an access door glanced off, striking EE. On 1/12/2016, individual underwent surgical procedure on right eye.

Was found on 11/08/15 at 12:43 pm by a security officer. She was unconscious and unresponsive.

A non work related accident happened on mine property. The injured had not started any duties or reported to work. The injured was traveling the Home Mtn mine access road in his personal vehicle when he stated that he fell asleep and his vehicle hit a utility pole and went over an embankment causing a laceration to his forehead and pain in his left shoulder.

The injured was operating one of the two buses that helped push a broken down tram into a side track on 18 rt section. The injured stepped out of the operators compartment of the No. 3 bus when he stepped on the side of a water line that was beside the bus. The injured twisted his right ankle, causing a fracture to his ankle.

The injured was operating a tram on 8 vent bottom when the rock dust hose started to slip off of the tram. The injured raised up out of the operators deck to put the hose back on top of the tram as he did this he accidentally hit the dead man and the tram moved forward, causing him to strike his head against the mine roof. The injured suffered a strain/sprain of the neck.

Job-related hearing loss. Workers compensation compromise settlement.

Two tram operators were operating trams headed toward 9 Vent bottom when they seen the victim laying next to the track rail. He was unresponsive and had no pulse. CPR started immediately. Cause of death was probably heart attack. The victim had no signs of injury on his body.

While working to loosen the cone nuts on the work deck, the wrench slipped pinching the palm of his right hand. This resulted in stitches.

The injured was in the track entry between 25 rt. and 26 rt. when the incident occurred. He was lifting a partial barrel of chain onto a track mounted jeep when he felt a sharp pain in the bottom of his right foot.

Job related hearing loss. Workers compensation compromise settlement.

The injured was roof bolting on 25 right no. 3 heading left brk. when the injury occurred. He was starting the first hole in the 3rd row of bolts when a piece of rock fell out from the mine roof and struck him on the left foot behind the metatarsal plate on his boot. The injured fractured 3 bones in the top of his foot. The rock measured 205"x19" by 4" thick.

The injured was washing his coal hauler off on Southeast Mains. He walked around the coal hauler and stepped in a hole and slipped and fell, causing a strain/sprain to his right shoulder.

Injured was working on 25 rt section helping pull rails off the truck & placing them out in the track entry. He was in the process of putting bolts in next rail to be pulled off, & while he was rolling the rail over his bar slipped & he mashed rt little finger between the jack bar & the end gate of the rail trucks. Injured sustained a fracture to the end of his rt little finger.

The injured was carrying crib blocks to level storage unit for the longwall. He bent over and picked up a crib blocked and when he turned he felt something pop in his right knee. He sustained a sprain or a strain.

Employee was working at a desk in the office at the Horn Mt. Portal of the Buchanan 1 Mine on 06-29-14. At about 8:30 AM she started feeling sick and left at 10:30 AM. She went to the doctor and it was determined that she had a urinary track infection. She returned to work on her next scheduled day to work which was 07-01-14.

The weather was wet and rainy. The security officer was stepping out of the office door and stepped on a loose concrete block step. As a result, the officer fell forward onto his right knee. He said his knee was okay other than slight soreness and was reporting the fall for reporting purposes.

EE was lifting drill steel, when he felt a pain in his left shoulder. On 12/15/2014, EE underwent surgical procedure on left shoulder.

IE was working at the old bunker on 1 east. He was helping take roof jacks thru an air lock door. Once the they got the jacks thru the doors they were throwing them between two roof jacks that were already set to keep the walkway clear. The cuff of the IE glove hung on the jack and carried his hand into the jack that was set, causing a cut to the palm of his L hand, 6 sutures.

On 3/29/14 said his left arm was numb and he was having chest pains. Supervisor reported to site. EE refused an ambulance to be called and said it was not job related. Supervisor observed EE before he departed site by driving. His mother was going to meet him and take him to the ER. He was asked to provide a medical excuse. EE is not schedule to RTW until 4/1/14/. No lost time.

Injured worker stepped over the deck, and slipped when he made contact on the surface of the split set. On 02/20/2014, a fracture to his ankle was confirmed.

EE was helping change out 4" water line. Pipe got stuck in mud and ee picked up on end of pipe to free it. 1 of other EE's was on other end of pipe using a bar to help move his end over. Water line shifted and caught ee left ring finger between the pipe and a timber, causing a fracture. Now had left ring finger amputated at knuckle
