TM was unhooking a pickup truck from a trailer and when the jack popped off the ball the trailer moved forward and smashed their left index finger between ball on the truck and trailer frame.

EE was climbing out of the screen when EE slipped on a step and put left arm out to catch self. EE now has discomfort in left shoulder and went to doctor on 8/17/28 and had surgery on 8/25/23 for torn ligament.
Team Member was walking through the shop when a transfer box for a articulated truck (weight 454lbs) slipped off the cribbing and rolled onto and pinned their right leg. Employee freed themselves. Fracture just below the knee. Employee is being put in a brace and can bear weight but no bending at the knee.
EE was standing on a platform installing a threaded plug in the motor junction box using a pipe wrench. They had their left foot up one step on motor platform and right foot on the platform EE was standing on and while pulling the pipe wrench towards them they felt a pop and discomfort in right knee. Become a recordable on 12/1/2022 when cortisone shot was received.
Team member lost their balance getting onto the ladder of the grader. This loss of balance resulted in their left foot getting struck in the step hyperextending their left ankle while the rest of them fell backward. Team member was evaluated at the medical facility where they were diagnosed with a sprain and given a boot to immobilize the ankle.
EE was removing the timing gear off of a screen. EE slid the timing gear to the end of the shaft. EE's right hand was at the 6 o'clock position. It weighed more than expected and pinched EE's right middle finger between the guard and the timing gear. EE suffered a fracture in the right middle finger, and EE received approximately 10 stitches.
EE needed to put the headache ball up on truck number 43770. EE describes was trying to attach the headache ball to the truck and was struggling. EE made one last effort with all of EE's might and got it attached but when EE did this last effort is when EE felt the strain and pain in the left upper arm and shoulder.
Employee started having pain in side and was struggling to get breath. Due to an underlying health condition 911 was called. Due to EE's medical history EMS decided to transport EE to the hospital for assessment. EE was released from the hospital on 11/2, and tested positive for COVID-19 on the subsequent day.
Vape pen batteries exploded in pant pocket.
EE was using a winch (model 492) to back off the gravity take up weight when the winch handle slipped out of EE's hand while changing hands. The winch handle then spun around and the handle hit just above EE's right wrist. The handle was free spinning once EE's hand slipped off of it. Right wrist has a fracture of the ulna bone. Injury not in a cast but splint.

Mechanic apparently overheated and passed out. It appears that they passed out for a short period of time and was found sitting up against a tire. EMS evaluated and transported them to Hospital. After several bags of fluids via IV mechanic was released from the hospital around 10:30 p.m. was instructed to take two days off.
EE was running excavator when rear hydraulic line blew. EE got out of cab to check line and slipped on hydraulic oil and fell to ground. EE put left hand down to catch self and lacerated palm of left hand on rock.
A guard on the screen was being removed. This guard was a split guard that ended up hitting the spray bar above and couldn't be removed. The guard was wedged (stuck) in the up position. EE had EE's hand resting on timing belt sheave. The guard was either bumped or fell and smashed EE's right pinky finger. EE was wearing gloves.
Replacing railroad iron on 44LFC4 discharge chute, the steel box covering the outside end of the iron fell off onto the top of the miner's foot. Miner was wearing safety-toed boots, but the ~5"x8" box fell above the safety toe. Miner had a small puncture wound on the top of the right foot, requiring 2 stitches and a tetanus shot, with minimal swelling or bruising to the foot.
Employee was removing an excavator tooth shank from the excavator bucket. Employee air arced existing welds and EE struck the shank with a 16 lb sledge hammer. The hardened steel sledge caused a piece of hard facing to chip off of shank and embedded in right thigh. Had foreign body removed on 8/18/17 with stitches - 1 day lost time.
Employee slipped and fell on ice while opening the lower entrance gate
The dredge bucket ladder chain had walked off the ladder flat idlers. To realign the chain back on, employee used a 4x4 to lay between rock gravel and chain which will force chain back on. As he was holding the 4x4 his left ring finger was mashed against the chain. Laceration - received stitches (3).
Employee was walking u the metal stairs into the corex when slipped and Rt shin hit metal step. Treatment with 1 day lost time.
EE was shoveling off the cat walk when he struck the side and his forward shoveling motion was abruptly stopped causing injury to left wrist.
Employee was changing a motor on the 2000 gallon fuel tank. As he was climbing onto the tank, he put left foot on 1st step and as his weight went onto left foot he heard a pop in his left knee. *Employee worked full duty until surgery on 6-10-13*
Employee was changing the cutting edges on a D-6 dozer using an impact wrench. He was pushing the belt down into the cutting edge with his left hand when his right hand bumped the trigger on impact wrench, turning the bolt. The head of the bolt was sharp and it spun cutting through glove and cut L middle finger. Received 7 stitches.
Employee was getting off the 627 scrapper when she stepped on the bottom step of scrapper and rolled left ankle. 9-12-12 physical therapy has been requested, now recordable.
Employee was getting onto a gradall when he felt a pop in his left knee. EE worked full duty until surgery on 8/8/12.
Employee was getting off the dozer when foot touched the ground, his (L) ankle rolled.
Employee was pulling the chain to open the front gate when he was swing his right arm up to get the slack out of the chain his right wrist hit the corner of the office hour sign. This caused a laceration.
EE was helping change a bearing on S7 screen on the dredge. He was removing the counterweight from the shaft. When he slid the counterweight of the end of the shaft it dropped approx 3" & pinched left index finger between the counterweight he was removing & the counterweight on the bottom.
Measuring steel for liner when a sliver of metal went into his left eye. Was wearing safety glasses and face shield. Received med tx where foreign body was removed and he was given a prescription for eye drops.
Employee was fueling her tractor when she pinched her hand between the fuel nozzle and the neck on the gas tank. No medical treatment was needed. 9-4-08 - Received treatment, dx w/contusion - not recordable. 1-8-09 - Had surgery to remove small mass from left middle finger - recordable.
He was operating the clam dredge. He was adjusting a water hose, when bucket came back up and flung a rock at him. The rock struck him in the L upper arm, causing a laceration.
Coming down ladder and was on the bottom step when he hopped to the side with left foot partially landing on a pallet. This caused his left ankle to roll and he fell to the ground.
Employee had chained down a John Deere 400 In Dump and then raised the bed in transport position. This caused the chains to stretch and the chain binder to break and hit his nose.

EMPLOYEE PICKED UP A CAN OF PAINT & TURNED TO SET THE PAINT ON THE TABEL & HIS BACK POPPED.
Standing on trac of dozer urinating, his hip/leg gave out causing him to fall 6 feet to the ground. Employee was off work from 11/21 to 11/24/04 and again on 12/5/04 for a non-work related hip/leg injury.
Cleaning the dredge with a 2" water hose, he went back to untangle the hose and stepped on a 2" step and then stepped on the hose. This caused him to roll his left ankle. Diagnosed with a strain, given prescription, crutches, and taken off work.
CHANGING A SPRAY BAR NOZZEL, HE WAS LYING OVER A TUBE PULLING ON A PIPE WRENCH WHEN HE PULLED A MUSCLE IN HIS UPPER STOMACH, LEFT SIDE.

USING A GRINDER IN THE SHOP WHEN HE GOT A FOREIGN BODY IN HIS LEFT EYE. (PRESCRIPTION WAS GIVEN FOR ANTIBIOTIC DROPS.)
EE WAS WORKING ON AND TAKING MEASUREMENTS ON A DREDGE BUCKET IN THE SHOP. EE WAS ON THE DREDGE BUCKET WHEN HE FELL 3-4 FEET ON THE CONCRETE FLOOR HITTING HIS RIGHT HIP.
REMOVING BROKEN STUD UNDER A DOZER WHEN HE GOT A FOREIGN OBJECT IN HIS RIGHT EYE. HE WAS WEARING SAFETY GLASSES.
EE WAS ATEMPTING TO REMOVE A DETACHABLE BUCKET FROM SKID STEER LOADER. THE BUCKET WAS RAISED SEVERAL INCHES OFF GROUND. EE HAD RELEASE LATCHES OPEN ON BUCKET, BUT BUCKET WOULD NOT RELEASE DUE TO MATERIAL BUILT UP ON LATCHES. BUCKET THEN RELEASED AND FELL ON EE'S LEFT FOOT.
CLIMBING OFF THE ANCHOR DOZER WHEN HE SLIPPED ON THE TRACK & HIT HIS LEFT ARM ON THE BLADE ARM.
EMPLOYEE WAS USING A ROPE TO LOWER A PART FROM ONE LEVEL TO ANOTHER. THE ROPE HAD 3 WRAPS AROUND A PIPE/RAIL. AS HE WAS LOWERING, THE ROPE CAUGHT ON FRAME ALLOWING SLACK IN TOP 1/3 OF ROPE. WH EN ROPE WAS FREED, EMPLOYEES FINGER WAS CAUGHT BETWEEN ROPE & RAIL.
CROSSING A FIELD CONVEYOR TO REPLACE A ROLLER WHEN HE SLIPPED ON ICE SNOW ON THE GROUND AND FELL TWISTING HIS LEFT ANKLE. HE RECEIVED MEDICAL TREATMENT, DIAGNOSED WITH LEFT ANKLE STRAIN AND GI VEN AN AIR SPLINT.
GRINDING ON THE HEAD BOX ON FLOATER CONVEYOR #6 WHEN THE GRINDER JUMPED STRIKING HIM IN LOWER LIP.
