Contract truck driver was loaded at the Agg Plant and while descending the hill to the scales, lost EE's brakes, jumped out of the dump truck receiving facial injuries and the truck continued down the road driverless through two berms before finally coming to rest a third berm.

Employee was being transported to the west pit by a supervisor when they stepped out of the truck and slipped on ice and landed on right hip and right elbow. After onsite evaluation it was determined to take them to the ER for imaging to ensure no further injuries. Employee was diagnosed with a strained shoulder, put on restricted duty and instructed to follow-up with an ortho.

Maintenance contractor was changing out conveyor belt when the anchor point they were using on the surge tunnel broke free causing the snatch block to strike the contractor in the left hip fracturing EE's pelvis.

Employee was injured while replacing belt line when EE was struck by a snatch block that tore away from the structure it was attached to.

Crew was changing belt on BC-8 when the roll of belt being suspended by the boom truck shifted while pulling the belt and crushed little finger on right hand between the roll and the handrail of the man lift they was working from.

The employee was performing the routine task of welding new wear plates in the feeder bin of the jaw crusher. The employee stepped out of the bin and felt a pop in right knee. After being seen at clinic the employee was referred to get a precautionary MRI, which revealed a torn meniscus requiring surgery to repair.

Employee was walking down catwalk and felt a pop in EE's left knee. Employee was seen by doctor who found no injuries but requested further imaging and placed employee on restricted duty until imaging was completed.

Employee was conducting pre shift inspection on sand plant, slipped on ice, fell and broke their left wrist.

Employee's right foot slipped on loose ground causing EE to fall down the slope while checking pump. The employee landed on EE's right shoulder and right hip and struck the right side of EE's face. Out of an abundance of caution, emergency responders were called and the employee was taken to the hospital via ambulance.

IP was working on a project to clear trees that were fallen with an excavator and the IP was cutting off limbs to allow the excavator to safely move them to the burn pile. IP was cutting a limb approximately five feet off the ground when the tree rolled back towards IP and the limb IP had just cut hit IP in the chest, knocking IP to the ground.

EE was operating a CAT 773 haul truck when EE hit the left side of head on the inside of the operator's cab after hitting a pothole/bump in the road. After seeking medical attention, EE was given medications in the ER but did not receive a written prescription and was released to full duty.

Blaster was working on a bench and fell, as the blaster attempted to catch self from the fall cut a finger in the process. There was a laceration to the left pinky finger requiring 6 stitches.

Employee was cutting a piece of metal from the old plant, the metal had some spring tension on it. When cut through, the piece of metal sprung and hit the employee's leg just below the knee on the right leg. There was some swelling at the point of impact. Employee worked the rest of the day and requested to have the leg checked out the following day.

Hauler operator was driving back to the strip after dumping EE's load at the waste area. There was a washout on the road and as EE tried to avoid some of the washout EE's right front tire hit a rock and the cab of the articulated truck EE was driving shifted. EE said EE felt a pain in EE's left lower back down into EE's leg. The employee was taken to be medically examined.

Working in the Mercer Lab, employee was preparing a test sample. EE reached back with right arm to get gloves and said shoulder popped. EE said it hurt but EE worked the remainder of shift. Monday 6/14/21 EE reported the incident. The following Monday 6/21/21 EE reported it was still bothering EE. EE was taken to a medical clinic in Princeton WV to be evaluated.

Employee was restacking a pallet of pulverized limestone 50 pound bags. EE said put one knee on the ground and picked up a bag and felt a burning sensation in the lower back.

Rolling a fuel hose off a hose reel and had the nozzle in hand and it twisted their right hand/wrist.

Employee was replacing a valve on a water tank, was cutting insulation off the valve to gain access. After cutting of insulation EE was about to cut a small wire tie off, the tie snapped quickly and they cut their hand between the ring finger and little finger on left hand. The cut required 2 stitches.

Pulling belt on tugger machine, hook broke causing tugger to move forward and sling chain, hitting employee on leg and ricochet into mouth.

Employee was cleaning up around the shop, lifted a section of drill steel and strained back. EE was treated at clinic and released to work on light duty. Released to full duty on 08/21/2020.

Greasing equipment and while descending the excavator, EE put foot on the ground and EE's back popped. EE immediately felt pain in the lower back. EE was taken to medical office where EE was treated and released with restrictive duty. EE was examined again on Friday 5/29/2020 and was released to full duty.

Tramming skid steer to assist in cleanup of debris around a work site. While tramming, employee ran over a rock that caused the machine to raise up on its back tires and then slam back down on the front. This caused the machine to bounce and injured the operator's neck.

Employee was working to remove a pulley from the shaft of an air compressor in the bone yard to retrieve the air line. Employee used a wedge and hammer to push the pulley away from the shaft. As the employee removed from the wedge, the pressure pushed the wedge back up and struck the employee in the mouth breaking two front teeth.

Employee was returning hurriedly to the control room to shut down a feeder when they slipped and fell resulting in a sprain of the right knee.

Employee was bent down under a belt as EE turned sideways to continue spraying EE felt a pain in the back.

Employee was under the loader in shop on a creeper draining the oil. Employee had safety glasses on. While engaged in the activity a small piece of dirt fell from the under side of loader and got in employee's left eye resulting in irritation. Eye was flushed on site and flushed again by a medical pro, nothing was removed from the eye. Doc prescribed antibiotic eye drops.

Was wrapping dozer truck with chain and mechanic's truck crane. The track got stuck and the employee released tension on the track that is when the track inadvertently slipped pinching employees pinky finger.

Operator was lifting a 5 gallon container of rock oil about shoulder height to fill a tank which has led to a lower back problem.

Employee complained of left shoulder pain after shift. Employee had assisted in shoveling during a clean up after train loading was completed.

After completing repair of the electronic eye that monitors belt rotation of the #10 belt, employee was returning to the man lift. While fully protected from falling by a fall protection system employee was preparing to re-enter the man lift. As man lift boomed closer employees fingers (left ring and pinky) were caught between the man lift and the structure.

Employee was tightening a nut on the Telsmith cone crusher using a sledge hammer to strike a bar that goes through the nut. As the employee was striking the bar the sledge hammer slipped causing a popping sound and pain in the employees right wrist

Employee was using an air hammer to remove a bushing. The bushing splintered striking the employee in the face. A small sliver of metal was embedded into the employees forehead.

Employee was using a come along to pull the front of a screen back to obtain access to change a middle deck screen. While operating the come along employees hand slipped from the handle striking an adjacent I beam.

While walking to the off road fuel tank, employee tripped over a piece of rebar that had extended out of a broken concrete curb. When EE fell a piece of concrete punctured EE's left leg.

Cleaning a chute at the lime plant using a dead-blow mallet, a square-point, short-handled shovel, and an air wand employee attempted to use hand to remove a larger piece of material. This task requires the motor to continue to run to allow for more free-falling material to be moved into the chute until the hopper is empty/clear.

Employees were changing a screen at VS-2. After LOTO, the screen was unbolted and secured and as employee was positioning the screen for removal, the screen shifted and pinned employees finger between the screen and the lip / edge of the chute.

Employee exited the excavator and slipped on a rock and fell from ground level impacting the left side of their body (hip, tail bone, shoulder, wrist)

During crusher maintenance. The employee first removed the center plate which allows the shoes to be knocked out. As the shoe was placed along the top edge of the crusher it began to slide forward. In an instinctive move the employee reached out to catch the shoe resulting in the shoe mashing the tip of the employee's right pinky finger against the inside of the crusher.

Excavator operator was loading haul truck operated. The truck was nearly loaded when a large sized rock rolled from excavator bucket and fell landing on the rear of the haul truck bed causing haul truck to rock backward jarring operator in EE's seat.

Employee had been transporting water to fill the pond used by the wash plant. On EE's second trip as EE attempted to spray the water from the front sprayers into the pond, the truck moved forward over the berm into the pond. The employee exited the truck through the side door and made EE's way to the water's edge. EE sustained a cut on EE's right ear.

Employee was operating the Cat 980G loader, working the 3' stock pile and dressing up the berms. Employee claims the bucket was loaded and in the raised position with the loader articulated slightly to the left when the bucket suddenly free fell to the ground. This action caused the cab to bounce forward and the rear of the loader to rise from the ground then settle back.

Employee was welding and lifted his shield to chip. A piece of slag went into his eye-lid. The slag was removed and he worked the remainder of the shift. The next day he came to work at 4pm and worked until approximately 9pm. At 9pm while rubbing his eye and it became irritated so he went to Greenbrier Medical Center and was told he had a corneal abrasion.

Replacing compressed gas cylinder from service truck. He began moving it by rotating the cap when the cap came off the bottle and hit him in the tooth. The tooth has chipped and had to be capped.

Miner was cleaning the windshield of a 990 FEL when he slipped and fell to the ground roughly 10-12 feet breaking his femur into his pelvis bone. Was rushed to the hospital and put straight into surgery the next day.
