Worker was welding in shop. They removed their welding hood after completing a weld and the slag popped and sent material into their eye. The material had to be removed by medical professionals and they were prescribed antibiotic and steroidal eye drops.

Were replacing a bad sprocket following troubleshooting of a gearbox. Removed the 4 bolts from the mounting plate, chain slipped and motor & gearbox fell. The falling equipment impacted the IW left pinky finger, requiring stitches, antibiotics and light duty.
Miner was checking oil on the D8 Bulldozer and they slipped on the tracks. Their foot went between the tracks and cab, and their ribs hit against the grab handle. They injured their ribs. 2 weeks later on 12/1/25, they sought medical care and were prescribed a muscle relaxant.
2 employees were working on a palletizer, but only one of them had it locked out. When the locked out employee was done, they asked the other employee if they were clear, and heard "yes". The other employee was still working, and when the machine was started, their right hand was caught, amputating the index finger, and degloving the middle finger.
Injured person was working on the hydraulic system of a D8 Bulldozer in the morning. Had to access the system below the floorboards of the equipment, and was working in an awkward position. Later in the day, they experienced pain in side/stomach and left work to seek medical care. At the hospital the employee was evaluated for back pain and prescribed hydrocodone.
Lab technician was sitting, entering data at a computer in the lab, when they received a spider bite to their lower right back. Bite developed a bad skin reaction consisting or redness, blistering, itching & pain.
Vacuum Truck was being driven around a corner when the truck tipped over, landing on its passenger side in & against a berm. Resulted in a broken tibia and fibula in left leg.
Miner was completing pre-start inspection and as miner climbed up to the cab miner's hand slipped off of the hand hold and miner fell approximately 2' to the ground.
After recent rain, the employee was walking in the plant area and slipped on some wet diatomaceous earth. The employee fell from the slip and landed on right shoulder and back.
Employee was performing maintenance and kneeled on the ground to perform work. When they stood, they felt a pop in their right knee. Employee had initial pain, but it went away. During the night after the shift, their knee began to hurt. The following day they reported the incident and were sent to the doctor to be checked out.
Employee was cleaning a screw as part of janitor cleaning task they tried to clean it thoroughly and used their hand to clean further and they cut the tip of their middle finger, left hand they then went to foreman and they provided first aid and as bleeding continued, employee was then taken to emergency room.

Worker was turning on a VFD. When the electrician engaged the switch EE saw a flash and heard a loud bang. The breaker worked and arc was inside cabinet. Previously during a maintenance work a panel was installed with a self taping screw that punctured a cord. When the system was energized it went to ground and caused and arc inside the cabinet.
On 4/12/2022 an employee was walking around the corner of a bench in the lab and they didn't see a rug that was on the ground. The employee tripped and fell, and landed on their arm. on 4/15/2022 the employee wanted to be examined by a PLHCP and the employee was placed on restricted duty.
employee got their finger caught between a wheel and a wheel chock causing a laceration on the middle finger of the right hand.
Condensation on driver side step
Employee was riding as a passenger in a Polaris Ranger UTV. The UTV tipped over onto it's right side hitting the ground; jarring the passenger which caused a right shoulder dislocation. On May 21st employee was seen by a physician and modified work duties were prescribed. Employee remains on modified work duties.
Contracted employee tripped on a piece of metal in the fabrication shop as they were stepping away from a push cart, where they were inspecting some test/instructional welds. When the individual fell they landed on their hip resulting in a fracture of the hip.
While employee was handling 33lb bags of product, employee felt soreness/pain in back from repetitive handling of bags.
The injured EE was adjusting the width of the forks on the fork truck. EE reported feeling discomfort in EE's lower back. After self-care didn't improve the situation EE was advised to restrict EE's movements and weight bearing capacity.
Employee was climbing off of the electrical bucket truck when they made the transition from the access ladder to the ground the employee reported that they twisted their ankle. The employee received care at our local clinic and we were able to accommodate the work restrictions
While employee was picking up the 50 lbs bags off the floor to stack them on the pallet the employee complained of pain at the back of shoulder. The employee was diagnosed with a muscle strain and have a restriction of a 20 lbs lifting capacity.
Operator was moving a load of D.E. when they slid on a haulroad and the truck impacted a berm. The operator was jostled in the cab and received first aid injuries. The operator was re-seen by the PLHCP and upon the operator request the PLHCP placed restrictions on them (date seen by PLHCP: 11/12/2019). Operator is on restricted work currently.
IW was beginning to disassemble floating gearbox for a screw conveyor. The IW removed guarding and was in the process of removing the drive belts between the motor and gearbox. As the employee loosened the belt tension bolts, the gearbox and motor fell. As it fell, the employee's right hand and arm were pulled downward, straining the employees right shoulder.
I.W. sat down on the Rippers of the Case Loader during EE's conversation with another emp. The loader rippers hydraulic pressure was suddenly released and the rippers dropped to the floor while the I.W. was sitting on them. This caused the I.W. to fall backwards into the scraper box. The I.W.'s lower back hit against the scraper box resulting in fracture to the L1 & L2 vertebrae.
Employee was lifting bags of off spec material into a dumpster and felt a pop in the neck. EE's neck started to get stiff and the pain started to increase. EE contacted the supervisor, went to the clinic, and was diagnosed with a cervical strain. EE was placed on restricted duty for 10 days with a potential to be released to full duty after a follow-up visit.
Employee complained of having pain in elbow due to repetitive motion while performing the task of shoveling material into a kiln feed hopper. This was counted as a first aid until June 5th when the employee reported that the pain didn't decrease. The PLHCP placed the worker on restricted duty, gave the employee a sling to wear to restrict movement.
EE was entering an overseas container to prepare to load with product. The loading dock plate did not extend properly. EE lifted the loading plate flap with hand to reposition the load plate and sustained a sprain to the right wrist.
Kinyon employee performing work on 5th level deck of the 7 system stepped backwards falling through the Humphrey Manlift deck opening, falling approx. 11ft to the deck below.
EE was removing damaged bags of finished product from the 2AP conveyor. EE stepped backward while holding a 50# in hands. EE tripped and fell to the floor and the bag landed on right shoulder. Note - EE shift started at 6P on 12/31/2017.
The lime slurry system plugged up. Employee shut the system down and performed lock out procedure. When employee opened the slurry line, hot slurry came in contact with both wrists above the glove cuff.
Haul truck driver fell backward while exiting cab of truck, striking head on pavement and sustaining a 1" cut that required staples. Employee admitted to rushing and not having mind on task, and was not certain that EE maintained 3 points of contact.
EE was using the SteelWeld Shear to cut metal pieces to make a machine guard. When EE activated the foot pedal on the machine, EE's hand was in position under the clamp to hold the metal. EE's right middle finger was caught between the hydraulic piston that holds the metal being cut on the SteelWeld Shear work table.
Body position obscured line of sight while lifting rail car vent lid, resulting in trip over cam locks on top of rail car. Trip resulted in partial fall into open hole. 2 ribs fractured.
EE was walking to the office and tripped on the steps leading to the main office entry. EE fell and landed on left knee. The case became reportable on Monday, January 30, 2017 when EE was assigned modified work duties.

Employee was using a hammer and chisel to drive a counter weight off a motor shaft. When striking the chisel with the hammer, the hammer slipped off the chisel and struck employee's left thumb.
EE adjusted an out of position 50# bag of finished product on the 2AP slip plate area. When stepping down from the slip plates, approximately 30" to the floor, EE twisted left knee.
A large rock rolled away from the stacker discharge pile. The screening plant operator passed the discharge pile and struck the rock with the front right tire. The seat bottomed out and the employee suffered a lumbar strain and inflammation.
EE was packing 50# bags of finished product at the Haver system. The bags were filled with light density material so EE pushed on the bags to help them convey to the palletizer. EE felt right shoulder soreness at the end of EE's shift. EE was seen by a doctor on 03/19/16 and placed on modified duties.
EE was removing 50# bags of finished product that had become jammed on the belt conveyor line of 4AP. The equipment was locked out. EE reached over the incline to clear bags when EE caught EE's right forearm on a piece of expanded metal. The contact resulted in a laceration that required sutures.
Employee was fueling a bulldozer and whilst dismounting slipped off the tracks and hit the ground. EE's head hit the bowl of an adjacent piece of equipment.
The Quarry operator was assigned to help the screening plant operator to clean up spillage in and around the screening plant. He was shoveling dirt into the bucket of the front end loader then he experienced pain in his lower back.
Employee is a scraper operator. For mining purpose, and respecting equipment design, scrapers are pushed out of the pit by dozers. Employee is complaining of neck pain due to this activity. Accident was declared on the 11/06/2015
Employee is quarry employee that operate most of the time a scrapper Employee informed his Doctor - While driving a scraper constant motion of neck and body due to rough roads neck area started to get really discomforting.
The maintenance employee was trying to remove the gear box with a pipe wrench. The pipe wrench was adjusted on the collar of the gear box. The employee applied all his body weight on the handle. The jaws of the pipe wrench slip. He lost balance and fell on the shaft, head first. The employee cut his head above the right eyebrow.
