A LARGE SECTION OF ROOF (SLIP HORSEBACK) FELL JUST AFTER LOADING THE FIRST SHUTTLE CAR OF THE PILLAR PUSHOUT IN NO. 4 ENTRY AT SURVEY STATION NO. 1288. THE ROOF FALL WAS ONE LARGE SECTION AND WAS APPROX. 18' WIDE, 0-8' THICK AND AT LEAST 50' LONG. THE ROOF WAS A SLICKENED SIDED SLIP (HORSEBACK) THAT WAS UNDETECTABLE AND FELL WITHOUT WARNING.
