Roof Bolter fatality
A LARGE SECTION OF ROOF (SLIP HORSEBACK) FELL JUST AFTER LOADING THE FIRST SHUTTLE CAR OF THE PILLAR PUSHOUT IN NO. 4 ENTRY AT SURVEY STATION NO. 1288. THE ROOF FALL WAS ONE LARGE SECTION AND WAS APPROX. 18' WIDE, 0-8' THICK AND AT LEAST 50' LONG. THE ROOF WAS A SLICKENED SIDED SLIP (HORSEBACK) THAT WAS UNDETECTABLE AND FELL WITHOUT WARNING. The EE was loading lifters when a piece of rock (approx. 4'x 5' x 2') fell from the ceiling and struck the EE. The EE was not pinned under the rock. This resulted in a fatal accident.
