WORK CREW HAD REPLACED ROCK CRUSHER HEAD. THE HEAD WAS RESTRAINED FROM ROTATING BY A CHAIN WHICH WAS TEMPORARILY WELDED TO THE CRUSHER FRAME. THE CHAIN WELD BROKE WHEN THE CRUSHER MOTOR WAS ST ARTED. EMPLOYEE WAS STILL ON THE DECK AND WAS STRUCK ACROSS THE FRONT OF HIS LEGS BY LOOSE CHAIN.
