CRUSHING OPERATIONS SHUT DOWN DUE TO RAIN, MAINTENANCE WAS BEING PERFORMED ON THE CRUSHER. EE WAS STEPPING FROM LADDER ONTO THE CATWALK. THE CATWALK BOUNCED APPROXIMATELY 1", AND WAS SLICK DUE TO THE RAIN CAUSING THE EE TO ROLL ANKLE AND FALL.

ACCIDENT OCCURRED WHEN INJURED WAS CAHNGING THE LACING ON THE BELTS FOR THE CRUSHER.WHILE PRYING ON OLD LACING,THE LACING GAVE WAY FLIPPING HIM IN THE EYE.THIS COULD HAVE BEEN PREVENTED IF HE HAD BEEN WEARING HIS SAFETY GLASSES PROVIDED.NO DAMAGE TO THE EQUIPMENT RESULTED FROM THE ACCIDENT.

IT TURNED AND HE SLIPPED OFF THE LADDER BREAKING HIS LE FT ANKLE. THERE WAS NO DAMAGE TO ANY EQUIPMENT AND NO COMPLIANCE BROKEN. PROTECTIVE HAND RAILINGS WERE PRESENT AND WERE BEING USED NO JOB SKILLS OR TRAINING COULD HAVE PREVENTED THIS ACCIDENT

INJURED WAS SERVICING A BROKEN BELT ON THE CRUSHER WHEN HE CAUGHT A SHARP PIECE OF METAL CUTTING HIS RIGHT THIGH.THERE WAS NOT DAMAGE TO ANY EQUIPMENT AND NO RULES OF COMPLIANCE WERE BROKEN AT THIS TIME.JOB SKILLS AND TRAINING WERE PROPERLYUSED THIS COULD HAVE BEEN AVOIDED WITH COMMON SENSE WATCHING EVERYTHING SURROUNDING HIM.
