EE was lifting a roller frame and while setting it down, the roller (can) rotated or turned and pinched EE's right pinky finger.

EE was cutting off the inner race to change a bearing on the tail pulley of the screen feed belt. The splatter from the torch was landing directly onto a Chain Jack chain link. The Chain Jack was being used to horizontally suspend the tail pulley assembly. The chain link gave away which immediately caused the tail pulley assembly to release horizontally and pin EE's hand.

Employee was adjusting a work light on a service truck. As employee dismounted the rear of the truck, employee stepped on a rock. When employee let go of the rear of the truck, employee's left ankle rolled.

Employee was cleaning around base of 7x20 structure using a Bobcat MT 55. Employee hit a rut which caused ee to clench mouth shut quickly. This caused ee to fracture a tooth.

Employee was assisting in changing out a winch motor, employee lost balance after stepping up to the maintenance deck on a Drill Tech T35 (Blast hole drilling rig) and lost balance causing him to lose traction on the deck and then fall from the deck to the ground.

Employee #1 was holding a wrench on a bolt while another employee was using an air impact wrench. Employee #1 had placed his pinky finger of his left hand in a pinch point. As the bolt tightened, it crushed his finger.

Changing out wear liners in crusher, took out a bolt and liner fell out, smashing the ring finger between the liner and a breaker bar. Damage was to the tip of the finger, which broke the tip and took 5 stitches to sew up.

INDIVIDUAL WAS TAKING OFF A BEARING SHAFT ASSEMBLY (WEIGHING 75 LBS.) FROM A TAKE-UP PULLEY IN THE SHOP. HIS RIGHT HAND SLIPPED OFF AND THE BEARING SHAFT ASSEMBLY FELL ON HIS LEFT HAND (RING FINGER). INJURY REQUIRED FIVE STITCHES - NO BROKEN BONES. EE RETURNED TO WORK THE SAME DAY.

EE PLACED J WRENCH ON THE DRILL STEEL SO THAT IT WOULD REST AGAINST THE MAST WHILE THE PIPE WRENCH TURNED THE HAMMERWHEN HAMMER BROKE LOOSE, THE J WRENCH KICKED BACK STRIKING THE EE IN THE RIBS.
