Clarksville Quarry 1375 Metal/Non-Metal

EE was cutting off the inner race to change a bearing on the tail pulley of the screen feed belt. The splatter from the torch was landing directly onto a Chain Jack chain link. The Chain Jack was being used to horizontally suspend the tail pulley assembly. The chain link gave away which immediately caused the tail pulley assembly to release horizontally and pin EE's hand.

Employee was removing a steel plate. It fell against employee's left knee and landed on the left foot.

Employee was adjusting a work light on a service truck. As employee dismounted the rear of the truck, employee stepped on a rock. When employee let go of the rear of the truck, employee's left ankle rolled.

Employee was cleaning around base of 7x20 structure using a Bobcat MT 55. Employee hit a rut which caused ee to clench mouth shut quickly. This caused ee to fracture a tooth.

Employee was placing wheel hub on its side to install wheel studs. Employee smashed finger between hub & block of wood.

Employee was in the process of cleaning off top of slurry tank, so a repair could be made to one of the screens. Employee fell through the screen.

Employee was assisting in changing out a winch motor, employee lost balance after stepping up to the maintenance deck on a Drill Tech T35 (Blast hole drilling rig) and lost balance causing him to lose traction on the deck and then fall from the deck to the ground.

EE was attempting to clear the 7x20 discharge chute when he was struck by several rocks ranging from 4" to 8" in diameter. He was struck on left shoulder & elbow.

Employee was changing screen cloth on 6x16 screen structure. Catwalk broke & he fell through, cutting his finger on the structure.

Employee #1 was holding a wrench on a bolt while another employee was using an air impact wrench. Employee #1 had placed his pinky finger of his left hand in a pinch point. As the bolt tightened, it crushed his finger.

Employee was operating a Bobcat walk-behind track loader. Employee was caught by the rubber track & burned right leg.

Employee stepped off a conveyor onto wet material,causing him to lose a firm foothold.Employee was taken to the shop,where his ankle was iced for several hours.Employee stated that he wanted to see a physician.Took employee to a physician, where it was found that he had a sprained ankle.Employee returned to work on 5/2/12 @ 6:30 a.m.

Employee cleaning out a chute on the jaw crusher and when he went to close the door, he accidentially smashed his finger in the door.

Employee walked under the forklift, operator did not see employee when he tilted the forks causing the forks to hit the employee on the head.

Something flew into eye.

Changing out wear liners in crusher, took out a bolt and liner fell out, smashing the ring finger between the liner and a breaker bar. Damage was to the tip of the finger, which broke the tip and took 5 stitches to sew up.

Employee reported on 6/12/2007 that he hurt his back on 6/9/2007 while working on the plant, but only mentioned that his back is sore and that it probably would work itself out. On 6/15/2007 employee told his supervisor that his back still hurt and that he wants to go see a doctor.

Employee reported on 1/25/07 that his knee hurt (gave no reason why it hurt). He requested to go to a doctor on 1/26/07 and told the supervisor that he hurt his knee on 1/22/07 while coming down off a ladder.

EMPLOYEE WAS REMOVING DUST FILTERS UNDER SUPERVISION WHEN ONE OF THE FILTERS FELL DOWN, SMASHING HIS MIDDLE FINGER. WHEN EMPLOYEE REMOVED HIS GLOVE THE FINGER WAS DISCOLORED AND SWOLLEN

EE was working under a piece of equipment when he lost his hard hat. He tried to get up to get the hat and he hit his head on equipment.

EE was removing a stone (appox 90lbs) away from a crusher chain feeder tail adjustment. While moving the stone, another became dislodged, rolling and striking his foot.

EE & co-worker were welding most all shift. EE went home @ end of work day and his eyes started to burn. He believes he may have had a welding flash. He went to his doctor the next morning 2-2-05.

INDIVIDUAL WAS TAKING OFF A BEARING SHAFT ASSEMBLY (WEIGHING 75 LBS.) FROM A TAKE-UP PULLEY IN THE SHOP. HIS RIGHT HAND SLIPPED OFF AND THE BEARING SHAFT ASSEMBLY FELL ON HIS LEFT HAND (RING FINGER). INJURY REQUIRED FIVE STITCHES - NO BROKEN BONES. EE RETURNED TO WORK THE SAME DAY.

PLEASE ADVISE HEARING TEST CONDUCTED 1/22/04 THE EE WAS NOTED AS HAVING A SIGNIFICANT THRESHOLD SHIFT IN RIGHT EAR. REFER TO ENCLOSER 1 FOR BASE LANE, ENCLOSER II FOR ANNUAL & FINALLY ENCLOSURE III FOR RE TEST. SUBSEQUENT EVALUATION SHOWS STS IS BELOW 250D & THEREFORE NEED NOT BE REPORTED AS A PERMANENT LOSS.

INDIVIDUAL WAS DISMOUNTING A DUMP TRUCK, MODEL 773B, WHEN HE STATED HE STEPPED OFF WRONG AND SPRAINED HIS BACK.

EMPLOYEE WAS APPLYING BELT DRESSING TO BELT. HE THEN SIGNALED TO START THE BELT. THE TURN BUCKLE HOLDING GEAR REDUCER BROKE ALLOWING THE REDUCERAND MOTOR TO SPIN BACK AND STRIKE THE EMPLOYEE.

EMPLOYEE AND ANOTHER EE WERE IN THE PROCESS OF CHANGING THE BREAKER BARS ON THE CRUSHER WHEN HE STUCK HIS FINGER IN THE CRUSHER AND IT WAS SMASHED. EE TOLD INJURED NIT TO BUT HE DID NOT HEAR H IM.

EE WAS ASSISTING DRILL OPERATOR CHANGE HAMMERS ON DRILL. TOOLS USED WERE THE HYDRAULIC OPERATED PIPE WRENCH AND A J WRENCH. EE PLACED J WRENCH ON THE DRILL STEEL SO THAT IT WOULD REST AGAINST THE MAST WHILE THE PIPE WRENCH TURNED THE HAMMERWHEN HAMMER BROKE LOOSE, THE J WRENCH KICKED BACK STRIKING THE EE IN THE RIBS.

EMPLOYEE WAS CHANGING BLOW BARS IN CRUSHER. HE WAS REMOVING A SHOE THAT HOLDS THE BLOW BARS WITH A SLEDGE HAMMER. A FRAGMENT OF METAL FROM THE SHOE BROKE OFF AND LODGED IN EMPLOYEE'S RIGHT LEG . EMPLOYEE REPORTED INJURY IMMEDIATELY AND RETURNED TO WORK.

EMPLOYEE WAS CHANGING BLOW BARS IN WELDING CRUSHER. HE WAS REMOVING A SLAG. A FRAGMENT OF METAL FLEW THE SLED BAR AND LOGGED IN THE EMPLOYEES LEFT LEG. (CANNOT READ THE NARRITIVE VERY WELL)
