Middle Tennessee Mine: 4 recorded fatalities Metal/Non-Metal

Employee was walking behind a refuge chamber in a stope to use the restroom when EE tripped on a piece of screen that was laying on the ground. The area was flat in nature and employee slipped on screen injuring their right leg.

Employee was attempting to cut a hydraulic hose with a grinder/cut off tool when tool slipped cutting EE's thumb.

Contractor Employee injured left elbow when trying to rotate feed cylinder (still in crate) with pry bar in the mine yard. Contractor Employee was rotating cylinder crate to do a proper hoist with telehandler.

Employee was bending over moving tools when they felt a sharp pain in their abdominal area. After further evaluation at ER, determined that employee had a unilateral inguinal hernia.

Employee was cutting pin from the skip door when the door fell landing on employee's leg.

Blaster was traveling down road in anfo rig when hit a pothole jarring their back. Employee was placed on restrictions for strained back.

Buggy was coming down hill at the fuel bay and while attempting to turn sharply into the fuel area, buggy rolled on its side injuring the passenger. Incident did not become reportable until 12/28/2022 doctor follow-up.

A mechanic was entering the cab of 8172 drill when they lost their footing. When they lost their footing, they grabbed the rail inside of the cab, causing a shoulder strain.

Employee was hanging utilities in roadway when a fist size of rock fell out between screen striking employee in hard hat. On follow up appointment with doctor on 10/20/2022, the incident became medically treated due to prescription to treat neck soreness.

Grouters were in the process of pumping water into the 1st hole in preparation of cement grouting when the packer being pressurized was ejected from the face resulting in an injury to one of the grouters.

An electrician working in the area of West A feeder belt passed a barricade when walking down the catwalk, and fell through to the ground below. The area had previously been barricaded off due to unstable catwalk platforms that needed to be replaced/fixed.

Miner was walking around cleaning the drill with a pressure sprayer wand, when miner fell, in doing so it caused the wand to rotate back to miner's face. The water stream cut a small laceration above miner's eye that required two stitches to close.

The employee was loading anfo into holes on the face when they stepped backwards causing a strain to their L knee.

The employee was standing on a railing to unclog a discharge line, when EE lost EE's balance and stepped down hard on EE's left heel causing a fracture to EE's heel.

Employee was stepping down off of a bolter onto a set of mobile stairs when left foot slipped out from under employee causing employee to twist Right knee. Employee sustained a sprain to MCL/possible medial meniscal tear.

The employee picked up a large rock to move it out of the underground road. While tossing the rock, EE dropped the rock on L foot. The rock struck EE's foot just behind the steel toe guard, causing a fracture to the L great toe.

The employee was using a fin-hoe to clean out lifter holes after drilling them, when EE felt pain in the lower back/left hip.

A cover door on an AD-30 was lifted to access the transmission as the employee leant down to inspect the area. The door fell down crushing both hands, breaking right thumb.

The employee was carrying empty boxes from the powder magazine when the employee dropped one of the boxes, causing employee to trip over the box. Employee fell on outstretched arm. The employee sustained a strain to left wrist and was placed in an immobilizer splint.

The employee was walking down a set of steps into the PM rack when EE slipped and fell. The EE caught self with right arm causing an injury to EE's right shoulder.

EE was walking to the cage to exit the mine when right boot kicked left boot causing them to fall. The EE struck left knee on the ground and sustained trauma with effusion to left knee as a result.

Maintenance tech was tracing leaks on hydraulic cooler of Haul truck. Unit was running, tech was looking to pinpoint leak, when working around fan area, the cooler leak opened up. Tech instinctively stuck hand out to block from being sprayed. The rag in tech's hand was sucked into fan & pulled hand into fan. The tech received a laceration to fingers that required stitches.

Two utility workers were hanging high-voltage electrical cable in the O3-N roadway. They were using the manbasket of a telehandler to lift the cable while hangers were placed when the employee asked the telehandler operator to raise the basket more. As the basket was raised, the employee's hand was pinched between the basket rail and the roof of the mine.

While preforming clean up in the con-shed, the EE pulled on a piece of plywood to remove it from the shed when EE felt a "pop" to the Left lower abdomen/groin. EE was diagnosed with an inguinal hernia.

Employee was walking on a freshly graded road in 29S-6A, up to the face, when left heel slipped in the gravel causing employee to lose balance. While attempting to catch self, employee hyperextended L knee, causing a sprain.

Hanging and bolting full line of 6" HDPE pipe on roof. Pipe resting on pipe roller shifted and fell on miner's left index finger causing miner to receive medical attention and 7 stitches on left index finger.

Employee was closing the door to the panning station when EE's Left index finger was caught between the door latch and the door frame causing a laceration that required #6 sutures to close the wound.

Employee was driving haul truck in the 261 area when they accidentally engaged the E-stop on the truck causing the truck to come to an abrupt stop. As the truck stopped suddenly, the cinching style lap-belt came un-threaded from the male end of the buckle allowing the employee to strike the windshield of the truck.

Employee was helping to change a haul truck tire in the drift when the truck shifted on the cribbing. As the truck shifted, the employee turned to get out of its path and in the commotion was knocked down causing a strain in mid back.

At approximately 4:30 pm August 30th, 2019; A piece of steel was being cut out of the chute with a sheave rope attached to control the downward motion when the piece was cut free. When the piece came free the rope tensioned and kicked toward an employee who was standing too close to the "v" of the rope striking EE's arm and face.

Employees were unscrewing a bowl from the crusher mainframe. Employees were using bars to unscrew the bowl. The injured employee's bar slipped, causing the employee's right hand to come in contact with a hand rail. Pinching the employee's finger between the bar and hand rail.

Miner was in the process of scaling down the loose ground on the rib, as EE moved to avoid falling materials EE twisted left knee.

Laceration to left hand palm. Cutting an avocado during coffee with a razorblade knife.

Fixed maint EE was pulling a cutting-torch cart across uneven terrain when their Right shoulder "popped" and began hurting.

Employee was helping transport pipe from trailer to the rig floor. EE was assisting with one end of the pipe while the other end was being lifted with the elevator. EE's left thumb was caught between the pipe and the rig floor. EE sustained a laceration to left thumb. "The EE was transported for treatment of injury and following medical evaluation, was placed on light duty."

Employee was helping blaster load face. Employee was unaccustomed to the additional heat and physical exertion required for the job. Did not maintain proper hydration. The employee started sweating heavily and went to fresh air. Started breathing rapidly and confused. Shift Lead called and got employee to drink water and transported to surface. Given IV at ER.

EE went to grab and remove last lug nut of LHD tire when the load shifted and the wheel pinched finger between the lug nut and wheel.

Employee sustained an avulsion injury and crush injury to EE's Right 5th finger while attempting to remove a bolt from the anfo tank on the blasting rig, when the bolt slipped smashing the employees Right 5th finger.

EE caught between fuel truck's open door & telehandler when telehandler backed up towards fuel truck. EE pinned at their rib area and was released as the telehandler operator pulled forward. EE taken to local ER, first aid only at the time of the accident. This became reportable to MSHA on 6/20/19 when they received med treatment of root canal and crown to a permanent tooth.

Employee was using a scaling bar to dislodge a rock from the #56 chute when the rock fell against the scaling bar crushing right 5th finger between the scaling bar and the back. The employee's finger sustained a full thickness laceration severing the tendon and dislocating the appendage.

At Approximately 2:10 A.M. a miner was loading roof bolts into the back of a tractor. The stack began to fall against the miner's legs pushing miner to the ground and on the way down miner struck face on the back of the tractor. Originally, treated as a first aid, but later diagnosed with a concussion.

Low back strain-R side-occurred while pulling/jerking on pallet jack lodged under pallet, felt a sharp pulling in lower back. Not immediately reported, thought they could work through it. To medical first aid at 3PM (ice/massage/heat/biofreeze) advised rest on reg. sched. Days off. Sched. With MD on 11/05/2018 when pain cont/ Placed on light duty with pending f/u.

10/11/2018, an operator tripped and suffered a fracture to left ankle. The operator was working in the 261-17 area nipping steel with another operator. They were walking a piece of screen through the stope to load onto a flatbed truck with one individual walking backwards. The individual that was walking backwards tripped over loose rock causing individual to fall.

Employee was scaling heading when employee got out of claw, employee noticed when employee raised arm employee had shoulder pain. Employee did not know of anything employee did that would have hurt it. Employee was carried out for a drug screen and returned to work. Employee came in 9/6/18 about shoulder and was taken to the clinic for evaluation.

On 8/15/2018, ignition of anfo in 229/21 area tore down utilities, damaged glass on blasting rig, and sent 3 miners to the ER to be evaluated. The miners were released to return to work without restrictions.

EE was drilling some metal when the drill hung up causing metal to spin and wrap around bit. Metal lacerated EE's left index finger.

employee was walking back to the rig when they tripped on a rock, causing them to fall, and land on their right arm, c/o R shoulder pain.

EE was cutting a chain, footing slipped causing hand to slide down the handle of the bolt cutters into the closing stop as they closed. The tip of L ring finger was crushed in the closing stop. The bone was broken and a laceration on finger. EE was transported to Medical Center where EE received treatment that included 5 sutures and shots for tetanus and antibiotics.

Employee was stepping off belt structure underground and backed down onto a piece of rebar that was attached to torch cart. Rebar punctured buttocks app 1 inch from anus.

Mechanic in UG shop was assisting employee reversing mobile equipment when mechanic slipped and fell approximately 4 feet injuring R knee.

While driving a bolt with a jack leg a rock approx. 12"x10"x3" dislodge from the adjacent bolt collar came down and struck ee on the outside of the left foot. Rock struck to the rear and the side of the metal guard on boot

17% decline worker was walking down ramp to get a bar. Slipped and lost footing, fell to knees. Right knee landed on sharp rock, cutting the knee.

Employee was operating loader and ran over rock, causing him to bounce and jar his R elbow. Incident happened on 8/26/2015, became reportable on 11/17/2015

They were trying to free the brakes on a haul truck that had a blown motor in order to push into the shop with a CAT Loader. When the truck was pushed it articulated and pinched employee against the rib.

EE's UTV overheated, went to relieve pressure and coolant burned left arm and left upper chest

Ee was walking underground and felt a pop in R knee and had immediate pain. *Injury did not become reportable until 7/9/15.

EE pans for geology, repetitive motion of panning caused R elbow pain. *Injury did not become reportable until 6/24/15.

Operator went to step over the slide of the drill and his foot got caught under slide causing him to fall injuring his elbow.

Employee was in process of taking down a booster fan when he slipped and fell approx. 20 ft. His tie off point wasn't secure and didnt hold.

Employee was on top of man cage attempting to unwrap a hydraulic hose from around some vent bag to get it anchored. The cage was moving about 20 feet a minute when his arm was caught between the steel set and roller assembly.

Riding on a tractor; when the operator stopped suddenly, miner' s neck was injured.

Then the closest end of the bar came up and hit the employee in the face. The resulting laceration above his right eye required 3 sutures.

Operator was in a curve when his foot slipped off the brake and hit the gas causing the truck to hit the rib. When he hit the rib, his elbow hit the cab inside of the truck.

Employee was tightening a hydralic hose leak with a wrench. The wrench slipped off of the connector and the employee fell backwards. He tried to catch himself but his left hand fell onto a rock.

Miner develop stinging and numbness in shoulder while working on the 10-10 screen

Employee slipped and fell from blasting rig, injuring his ankle and tail bone.

Employee was kneeling down. When he stood up he hit his shoulder on a fan, which knocked him down.

While cleaning beneath the fine ore bin by operating a high pressure water hose, employee strained his back. The strain occured when the operator lost control and tried to regain control of the hose.

This employee was pressure washing the center area of a fuel truck when the wand slipped and the water hit his foot cutting the boot and burning his foot.

This employee fell to the same level at the face. As he reached out to catch himself, he pulled a rock off of the rib. The rock struck his right, middle finger tip.

While cleaning muck from a workplace with a loader, a blasting cap from a misfired bottom hole was apparently struck by the loader dipper and detonated. No injuries occurred.

Hoist accelerated into overspeed mode and stopped. The hoist was removed from service immediately and all persons withdrawn from the mine via the Decline. Repairs were made to the electronic controls and the hoist returned to service for the evening shift on 08/10/2008.

This employee stated his back began to hurt after driving a haulage truck on rough roads.

A fire occurred on a road grader. The mine was evacuated with no injury to any person. The fire was allowed to extinguish itself. Air quality checks were made throughout the mine prior to miners being allowed to return underground.

This employee was using a sawzal to cut all thread while installing a pump starter. The sawblade jammed, resulting in the employees left pinkie finger being pinched between the saw caging and a piece of uni-strut.

#3 Hoist computer allowed the hoist to overspeed to more than 300 feet per minute total. The hoist safeguards immediately stopped the hoist. The computer was reset and the fifteen employees within the cage were brought to the surface. After successful function testing the hoist was returned to service.

This employee sliped and fell as she attempted to wash out a chute. She lacerated her left hand.

A rock fell from the face and struck this employee's right knee, as he scooped a sample of drill cuttings.

This employee was sitting in a maintenance utility vehicle passenger seat with his hand in the door opening. As he looked over his left shoulder, the vehicle door closed on his right index finger.

This employee was struck on the head and shoulders by a falling rock as he attempted to change a drill bit.

This employee said he reinjured a repaired hernia while installing a truck tire. After an investigation by the workers compensation insurance carrier it was determined that the injury was reportable as of August 26, 2008.

This employee injured his left, middle finger when he pulled a 5/16" chain from the back of a tractor carrier. The chain hung and jerked his finger when it unexpectedly stopped.

During pre-shift testing, #3 Service Hoist Computer recorded an inadequate brake test. The hoist was removed from service. No persons were underground. Further investigation revealed a failed hoist servo. When repairs are completed and hoist fully tested and found safe, it will be returned to service.

This employee pinched two fingers between a door and door frame on a blasting rig when the door closed.

This employee slipped and fell to the same level as he dismounted a service truck. He twisted his knee on a plastic discharge pipe. He later requested medical evaluation and was placed on restricted duty on April 23, 2008.

Upon entering the mine, the night shift crew encountered smoke and 12 PPM carbon monoxide. The shift boss removed the crew from the mine. No fire was seen. MSHA was contacted as a precaution. After receiving approval from MSHA, the mine was checked for hazards and none were found. A high voltage vacuum breaker blew out, causing the smoke.

Three employees were installing a wear plate in a chute. As they lowered the plate into the chute, it was pulled up by a tramp steel magnet. One employee's finger tip was pinched between the steel and the magnet.

#3 Service hoist was overloaded when a heavy motor was placed in the cage for transport. The safety controls on the hoist prevented conveyance movement. The motor was removed, the cage and rope connections inspected, and a round trip shaft conveyance test successfully performed before the hoist was returned to service at 6:00 PM. The mine crew left the mine via the Decline.

#3 Service Hoist brake did not release during pre-use inspection. The hoist was shut down for 55 minutes to adjust the brake. It was returned to service at approximately 10:15 AM. Mine crews were evacuated to the surface via the decline upon hoist shut down.

Worker was walking down a slope of gravel and clay that had received rain the previous night. The worker's feet slid from underneath her. She reached back to catch herself. This effort resulted in a strain.

As this employee placed a six foot long drill steel in place on a drill boom, another six foot steel fell over, striking his right instep.

As an old electric cable was being removed from a mine shaft, it broke and fell back into the shaft. This employee was observing the task and was struck on the upper lip by the end of the cable. Sutures were required.

A miner operating a Caterpillar TL943 Telehandler climbed under the boom to adjust a mirror. He accidentally contacted the joy stick control with his leg. The boom came down and caught him between the boom and engine cover, injuring his chest.

This employee and another were scaling loose rocks from a manlift basket. He and his partner identified a loose rock. When they began to scale the rock, it loosened over them. As the employee attempted to move the manlift out from under the rock, the rock fell, striking both men.

A miner was using a metal banding tool. Apparently a piece of the metal banding material was protruding out from the tool. The miner lacerated his left forearm on the metal banding.

This employee was changing electrical connections on a fan. He lacerated his left hand with a razor knife when he tried to cut nylon tie wraps. The laceration required six sutures to close.

EE was unscrewing a bad shaft coupling. When he got to the top of the threads, the oil tub had pressure against the shaft, causing it to jump sideways and strike him. This resulted in a deep cut on inside of right index finger.
