THE OPERATOR HAD ALREADY BACKED THE CONTINUOUS MINER BACK AND DROPPED HIS CABLE. AS HE WAS TRAMMING TO SET UP FOR THE END CUT OF THE NO. 6 BLOCK HE WAS CAUGHT BETWEEN THE CONTINUOUS MINER AND THE RIGHT RIB CAUSING FATAL INJURIES.

Employee was tightening bolts on a shuttle car wheel with a 3/4 drive ratchet and socket. The socket slipped off the bolt causing him to lose his balance and twist his right leg, causing a fracture.

Employee was prying a pump out of a 21 shuttle car when the corner broke off causing him to loose his balance and put all of his weight on his left knee. Employee began loosing time on 12/10/04 due to swelling in the knee.

EMPLOYEE WAS BRINGING STEEL OUT OF HOLE. AS HE WAS SEPARATING THE STEEL HE REACHED OVER TO LOWER THE HEAD TO GIVE MORE ROOM TO GET THE STEEL OUT. WHEN HE ENGAGED THE HEAD HE WENT THE WRONG WAY, CATCHING HIS FINGER BETWEEN THE TWO PIECES OF STEEL, CAUSING A LACERATION AS WELL AS A BROKEN BONE.

DURING WEEKLY EXAMS, AN AREA OF ROOF WAS DISCOVERED TO HAVE FALLED OUT BETWEEN RIB AND FIRST ROW OF BOLTS. APPROXIMATELY 3 TO 6 BOLTS WERE PULLED AND 7 TO 10 WERE SHEARED. THE AFFECTED AREA WA S APPROXIMATELY 26'LONG, 4 TO 6 FT WIDE X 4 TO 6'THICK. THE COMPANY DOES NOT BELIEVE THIS WAS REPORTABLE.

HE WAS TRAVELING BACK TO THE #3 ENTRY IN REVERSE IN LOW COAL CONDITI ONS WHEN HIS HEAD CAME IN CONTACT WITH THE MINE ROOF AND THE SEAT BACK REST. IT APPEARS THE EDGEOF HIS HARD HAT CAUSED A LACERATION TO HIS HEAD.
