Employee was working on a water clamp and had his right hand resting on the lifting arm next to the canopy post. The other operator came over to assist and started to lift the drill head higher. This mashed his fingers and resulted in fractures of his middle, ring, and pinky fingers and stitches.

Employee was taking miner water line apart. When the line came apart it was twisted and sprung around hitting him on the elbow and forearm. He went to the doctor on 11/16/12 and an x-ray revealed a fracture on the end of the bone (spur broke completely away from the bone).

A roof fall occurred at spad 463 between the #3 and #4 entries on the 2 Southwest Mains. This area was mined 5/21/2006. No one was injured, there was no equipment involved and there were no airways or travelways blocked. The fall measured approx. 15' long, 18' wide, and 3.5 to 4' thick.

A roof fall was found at approximately 2:20pm in the primary escape way 1 Break inby #4 tail piece measuring 40' long, 16'wide and 8' thick. This area was bolted with 4' fully grouted roof bolts and had 2 12' cable bolts in between every other row. There was no indication this area needed any additional support.

Severe thunderstorms were moving through the area of Deep Mine #35 on the evening of May 10th 2011. During on of these storms a bolt of lightening struck the mine office causing a fire at the mine office, with area wide power outages and no phone communications. The mine was evacuated and there were no injuries to any of the mine employees related to this incident.

An unintentional roof fall was discovered on 2 SW Mains in #6 entry(intake) S.S.1201, approx. 2500' off the working section. The fall measured 40'L x 8'H x 18'W. The cause of the fall appeared to be a slip with a coal rider seam and shale roof. This fall does not interfere with ventilation or with escape travel and will not be cleaned up at this time.

A fall of the roof 90' X 18' X 8' fell in the # 6 entry 90 feet inby SS # 1865, 5600 feet outby the face. 4' fully grouted glue bolts were on pattern in this area with 12' cable bolts as supplemental. Cause of the fall was a coal/shale slip.

The miner was de-energized. EE was moving cables to prepare to cut a corner.He put the rope on the bolt on top of the J-hook and went outby the miner.He started the miner and the rope slipped off and he leaned in to pick the rope up and was pinned against the rib with the boom of the miner.He freed himself and walked to the front of the miner.

A roof fall occurred 10,600 feet outby the working section in the #6 entry near spad # 485. The fall was 40' long and 18 feet wide and 5 feet thick. 4 ft 3/4 glue bolts and 12 ft. cable bolts had been installed in this area. This was a transitional area from sandstone to shale.

Employee was using a 5/8 high strength chain hooked to his shuttle car to try to pull a continuous miner. As the shuttle car started pulling on the miner the chain broke. The chain whipped around hitting the employee on the left side of the face above the eye and right cheek causing lacerations and a fracture of the cheek.
