3 recordedWHILE CHANGING A TRACK ON A DOZER, THE CHAIN HOOK SLIPPED OFF THE BOGIE & STRUCK THE EE IN THE HEAD AREA RESULTING INS SIGNIFICANT TRAUMA TO THEHEAD AREA. THE EE WAS TRASNPORTED TO THE UNIVERS ITY OF UTAH MED CTR. EE PASSED AWAY 9/25 AS A RESULT OF THE INJURIES SUSTAINED IN THIS ACCIDENT.
A TEMPORARY SUPPORT STRUCTURE PLACED BETWEEN THE MAST AND BOOM OF A DRAGLINE PARTIALLY FAILED. THE TOP CROSS MEMBER OF THE STRUCTURE STRUCK THE EMPLOYEE WHO WAS IN THE MAN-BASKET OF A AERIAL L IFT. THE EMPLOYEE WAS FATALLY INJURED WHEN HE WAS CAUGHT BETWEEN THE SUPPORT STRUCTURE AND THE RAILING OF THE MAN-BASKET.

High winds blew the doors on the back of the Drag Line house open. The employee and two others decided to close the doors. They successfully closed the right door and were attempting to close the left door when a gust of wind hit the door and flung it open blowing all three of them back with the door. The employee was pinched between the door and the guarding around the Gen-set.

The 103 Dragline at Ramp 53 contacted the cable handler #768 and was struck by the left rear house of the dragline while the dragline was in full swing. As a result, the cable handler was tipped onto its side. The cable handler operator was transported to the Dr. Office for further medical evaluation. On 10/13/25 the MRI confirmed that the employee would need surgery.
Employee was climbing off the dozer that was located on the deck of the lowboy trailer while stepping backwards when they fell off the trailer approximately 3 to 4 feet to the ground onto their back. This resulted in pain to the right elbow, and the wind was knocked out of them. The employee was transported to a higher level of care for further evaluation.
While positioning a barrel of solvent in the lube room of the 102. The employee's finger got caught between the barrel and the air compressor bank pinching the Left hand pinky finger crushing it between the barrel and the compressor bank.
An employee stopped for lunch break at the 2 high wall rock run. Employee climbed down the ladder grabbed both chock blocks, was walking to set one chock block on the left front wheel. The employee slipped on the ice and landed on their arm.

Employee noticed a hose blew apart down in Ramp 58. EE went down to the pump, shut the pump off and started to disassemble. The hose was not repairable, so EE went to get a different hose to replace it. When starting to pull the hose apart EE felt a poke through the glove. After removing the glove, the employee found a laceration on their left middle finger.
Employee was performing an oil change on 1774 after removing the first oil filter. Employee began to remove the second filter the employee felt 3 pops in their right arm between the bicep and forearm. Employee stated that the filter could have been stuck or had been put on too tight. MRI was done and a torn muscle was found.
Employee was carrying a receiver plate and tripped on a mixed gas hose and fell with the plate and the plate contacted the employee on the left eye brow. This caused a laceration over the left eye brow needing stitches.

Employee was opening the back doors of the dragline and placed hand on the lower rail of the door. When the door swung open, EE's left hand was located in a pinch point between the door and the back house fan.
Employee was doing audio test on 2-17-2022 and has showed employee had sustained a STS in left ear. Employee was scheduled to re test on the 2/23/22 and it confirmed that the employee has sustained a permanent STS in left ear, from the baseline in 2017.
Employee was working on 308 drill outside the shop. EE was standing on the mast loosening a hydraulic hose, after they broke the hose loose, they stepped down with right foot, left foot was caught under another hydraulic hose and right foot landed on uneven footing causing them to roll the ankle. Resulting in a small fracture to the right foot.

Employee was changing out a transfer chute wear plate. The employee was directing the crane when the wear plate became hung up. The employee freed the wear plate and as the plate settled the employee's right wrist was caught and bent backwards. The employee was referred to University of Utah. A diagnosis from the employee was received on 03/29/2021 after EE's surgery.

The Skid Steer bucket slipped off the attachment shoe edge and fell approximately 1.5 foot to 2 feet to the floor striking the employees foot behind the hard toe.
Employee arrived at R1 drill pad work area. Exited 2470 pickup, went around to get lunch box out of the passenger side. Grabbed lunch bucket with left hand, holding the door with right hand. The wind pushed the door shut and right middle finger was struck between the door and the frame. The finger was lacerated resulting in sutures.
Employee was climbing up the off side boarding ladder on 222 track hoe when, they slipped hitting right shin on a rung of the ladder causing a laceration on shin which required sutures.

Employee was lifting hood off of D11 CAT Dozer, Hood shifted and pinched left index finger between the hood and the holding bracket for the hood. Broke index finger.
While fueling 442 Haul Truck at Ramp 53 fuel island, and was putting chock blocks in holder and jammed left middle finger resulting in an extensor tendon tear.
While fueling 4117 pump in ramp 60, employee went to grab EE's radio out of 1771 lube truck. While climbing up steps employee slipped and hit EE's left side on steps.

Employee was sitting in the cab of Company #443 haul truck a 785 Caterpillar haul truck, being loaded with dirt. An unseen rock in the bucket struck the truck bed jolting the employee.

Employee was drilling a 5/8" holes in 3"x3" angle iron with a Wilton drill press. The piece pulled up out of the vice and grabbed EE's welding glove on EE's left hand. The drill twisted EE's hand and arm into the bit until it pulled off the glove and the bit broke. This accident resulted in a hairline distal fracture of the left ulna.
Welder was standing on a platform, driving a wedge setting the sidewall on a dragline bucket in the bucket shop. The employee lost balance and put left hand on the dog while swinging the 6 lb. Sledge. The sledge glanced off the wedge and struck the employee's left ring finger.

The employee had right hand on the tugger rope just above a snatch block pulley. The employee wanted to put slack in the tugger roper but hit the in button on the controller by mistake pulling right pinky finger into the snatch block pulley resulting in a fracture to right pinky finger.

Employee was taking off used dragline teeth and setting them in the dragline bucket and disrupted his right long head bicep tendon.
Employee was removing a fuel filter from a haul truck with a chain wrench when the wrench slipped and struck the employee on the right side of his jaw chipping his tooth.
Employee was removing a guard on a dozer, when the employee picked up the guard he felt a pain in his stomach. The employee was seen by a physician and scheduled for surgery. During the surgery it was determined that the hernia was caused the injury that the employee sustained on 8/8/15. The company received the surgical report and final diagnoses on 9/29/15.
Employee was moving cones for a scheduled blast, he slipped and fell on some loose material at ramp 60. The employee used his right arm to break his fall and heard a pop in his right wrist area. Resulting in a sprain to his right wrist and elbow.
Employee stepped out of 1768 flatbed truck turned and began to walk through the mud to the dragline bucket. Were he strained his left Achilles tendon.
Employee was kneeling, cutting a 4 inch rubber hose with a box knife. When he stood up he punctured his right thigh above the knee with the open box knife blade.
Employee slipped on ice as he fell to the ground he fractured his left Fibula.
Employee slipped on ice and fell to the ground. As the employee fell backwards he turned to his right, landing on his right hip resulting in a contusion and injuring his Sacroiliac Joint.
Slipped on ice beside shop building and tried to catch himself with his right hand, straining his right shoulder.

The employee opened a 5 gallon bucket of Foray fire extinguisher chemical with a box cutter knife. The bucket turned and so did the knife causing the knife blade to lacerate his left ring finger. Six sutures were used to repair the wound.
Employee was operating a haul truck on graveyard, being loaded by LeTourneau #243 in ramp 2. The operator's statement indicates she felt pain in her shoulders and neck while being loaded at approximately 1:30 am.
EE slipped and fell injuring his right shoulder, while pulling a barge pump to sure for repair at ramp 60 pond. His right rotator cuff was torn during the fall.
Employee was installing a bucket rigging pin in the upper hoist chain on the company #103 dragline when he felt a pop in his left elbow.
Employee was trying to get the deck bushing turned with pry bar supporting it. Bar slipped out and bushing came down and smashed right hand and fingers middle 4th and little.
Employee was climbing off a D 11 T Caterpillar dozer and slipped off the right side draft arm step causing him to twist his right knee.
Employee was operating 625 scraper when he ran over a large rock that was barely covered with dirt. After he passed over the rock, the right front tire of the scraper fell off the rock and dropped suddenly. Employee was thrown upward, and then slammed down, bottoming out the seat. He then experienced severe pain in his lower back.
Employee picked up one end of a thread saver for an overburden drill and felt his left calf muscle pop behind left knee
Employee was walking to the back of company #749 water truck and slipped on some ice. As she began to fall she grabbed a hose on the water truck with her left hand causing her to strain her left shoulder.

Employee was mounting company #103 dragline and felt a pop in his left shoulder
Employee tripped while stepping over dragline power cable and fell on his left hand and left ribs. The employee was seen by the on shift ERT. The employee did not go to the hospital. On 01/10/2014 the employee seen his doctor and it was determined that the employee received an Acute, non-displaced, fracture of his left 8th 9th 10th ribs.
Employee fell backwards from an elevated platform and hit his head on the ground causing a concussion and laceration to his head.
Employee was opening a man door in the shop wash bay. The door was jammed so the employee pulled hard on the door and felt a sharp pain in his left shoulder.
Employee was exiting a ladder and over extended his left leg on the last step and felt pain in his lower back.
Employee was removing a grate over a sump in order to clean the sump. Employee used a 1/4 inch round stock hook approx. 3 feet long with a 1 inch 90 degree bend on the end of the hook. The grate slipped off the hook and struck the employees right foot just behind the steel toe of his boot resulting in a fracture to his Medial Metatarsal of his right foot.

While lifting power cable onto the tub hooks of a dragline employee felt pain in his left bicep.
Employee was removing the muffler from a F250 pickup. The employee removed the brackets from the section next to the muffler and was in the process of removing the remaining brackets on the tail pipe section. When the muffler came loose, the bracket pinched the employee's left middle finger against the truck frame.

Employee was walking across the shop when he slipped on the track rails. As he caught himself he felt pain in his lower back.
Employee removed razor sharp steel shavings from a lathe with his right hand lacerating his right index finger
Employee was digging a ditch using a pick to loosen the material. The employee swung the pick and felt a pop in his middle/lower back.
Employee was putting lifting slings into a storage box in the back of a mechanic truck when the lid fell onto the employees left forearm lacerating the top of the forearm. The employee received four sutures.

The employee was helping install half of a hydraulic to the other half of the hydraulic tank by lifting one side of the tank so a fellow employee put bolts in the tank. While lifting the side of the tank the employee felt a pop in his lower back.
EE was working on Tub of a dragline. He was standing on the 3rd step of a 6' ladder. As he went to step down off the ladder, his foot slipped on the 2nd to bottom rung. When his foot slipped, he struck his chest on the Tub Manway Portal in The Tub.
Employee slipped off of the lower step on a fuel truck and landed on the door threshold, injuring his right back rib area.

Employee was getting into # 1585 service truck when he heard a pop noise and felt pain in left knee.The employee did not have surgery on his left knee until 03-24-2009 this is the reason for reporting now.
Employee was moving a steel shaft with a forklift. The employee got off the forklift to adjust the cribbing, the left fork kicked up and struck the employee in the left shin. The employee recieved eight stitches.
Stepped up on step stool when installing windshield. Step stool slid out from under my feet. Caught self from falling and hurt back.

Employee was moving a 55 gallon barrel and his right ring finger tip was pinched between the barrel and dragline swing gear case. Employee recieved two stitches.
The employee was attepting to open the door of a water truck and the wind blew the door closed on her right index finger tip. Employee was taken to the hospital and suctures were required to close the wound. She subsequently missed two (2) work days and returned to light duty on 6/11.
The employee was diagnosed with Carpal Tunnel Syndrome in his right wrist. He underwent surgery to repair this condition on March 16, 2007. This resulted in a Lost Time Occupational Illness.

The employee was diagnosed with Carpal Tunnel Syndrome in his right wrist. He underwent surgery to repair this condition on December 15, 2006. This has resulted in a Lost Time Occupational Illness.
The employee was attempting to fuel a piece of equipment when the rear door of the service truck lowered striking him on the back of the neck. Initial examination and xrays were negative, however after returning from scheduled days off the employee complained of pain. The employee was examined by a physician and placed on restricted duty.
The employee was attempting to attach a nylon strap on a dump socket wedge for a dragline bucket rigging when the wedge slipped causing it to graze their left hand. Upon medical evaluation, it was determined that there was a non-displaced linear fracture of the second metacarpal on the left hand. The employee was placed on restricted duty.
The employee was dismounting the shoe of the #103 dragline when he stepped on uneven ground and twisted his left ankle. Upon medical evaluation, it was determined that he had strained his left ankle. This accident has resulted in a restricted duty occurrence.

Employee wiped his right eye and felt pain in the eye. His eye was flushed, but no foreign material could be found. The eye was red. Employee was prescribed and used antibiotic drops.
The employee was moving along the edge of the left track of a D11 Dozer when he lost his balance and fell to the floor. Upon medical evaluation, it was determined that he had strained his left ankle. This accident has resulted in lost work time.
The employee was diagnosed with bilateral Carpal Tunnel Syndrome. He has underwent surgery for this on 11/16/05. This has resulted in a lost time occupational illness.
EMPLOYEE'S HAND WAS CAUGHT BETWEEN POWER POLE & BUCKET TRUCK. RESULTING IN TWO BROKEN FINGERS.
The employee was walking on the deck of a lowboy trailer when he slipped off the edge and fell to the ground. He landed with his left leg under his body. Upon medical evaluation, it was determined that he had strained his left knee. This accident has resulted in a restrictive duty occurrence.
Employee stepped down an ~6" step with his right foot and stepped on a 2" piece of angle iron. He suffered a sprained right ankle. This injury was a First Aid case until 01/20/06. It became reportable due to Physical Therapy Treatments employee received on 01/20/06. Note: This employee is a TXU Mining employee who was on company business at the specified mine site.
The employee has been diagnosed with Carpal Tunnel Syndrome in both wrists and underwent surgury for this condition on one wrist, October 19, 2005. He will have the other wrist operated on, November 02, 2005. This has resulted in a Lost Time occupational illness.
At the time of the incident employee was using an air torque wrench to remove sprocket segments. As he was removing one of the bolts holding a segment in place the wrench slipped from his grip. When he attempted to catch the wrench he accidently squeezed the wrench trigger, activating the wrench and causing the torque arm to swing into and smash the left ring finger.
The employee was reaching for an item above his head, lost his balance and fell to the floor on his right side causing a fracture to a rib on his right side. The employee was released to full work duty.
The employee died of apparent natural causes. The autopsy has not been finalized.

The employee was cutting an envelope open with a pocket knife when he cut through the envelope and caused a laceration to the inside of his right wrist. This injury required three (3) stitches.
This employee has been diagnosed with Carpel Tunnel Syndrome and underwent sugrey for this condition on November 8, 2004. This had resulted in a lost time occupational illness.
The employee was decending the ladder of a haul truck when he slipped and fell to the ground. He fell onto his left hip. Upon medical evaluation, it was determined that he had a fracture in the left pelvis region. This accident has reasulted in lost time.
This employee has been diagnosed with Carpel Tunnel Syndrome in both wrists. He has underwent surgery for this condition on his left wrist on 11/29/04 and surgery for his right wrist on 12/08/04. This has resulted in a lost time occupational illness.
The employee was dismounting a dozer when his foot slipped off the bottom step. At the same time, the employee was holding onto a grab iron. When the employee's foot slipped, he felt a sharp "HOT" pain in his back. This resulted in a strain / sprain in the employee's lower back. The employee was put on restricted duty.
As the employee was removing a chock block from the holder on the tractor, he felt a pain in his left shoulder. Upon medical evaluation, the employee was diagonsed with a strain / sprain in the left shoulder. This resulted in restricted work activities.
EE WAS WALKING ACROSS A DIRT PAD WHEN A PIECE OF DIRT ENTERED HIS RIGHT EYE. THIS MATERIAL BECAME IMBEDDED IN THE CORNEA AND WAS REMOVED WITH A COTTON SWAB.
At the time of the incident Employee was observing a co-worker hoisting a brake accumulator out of a CAT 988 wheel loader. As the accumulator was being lifted out of the loader it suddenly rotated on the end of the hoist cable and struck employee's right middle finger, pinching it between the accumulator and a handrail.
EE BENT OVER TO INSPECT AN OIL LEAK. AS HE SIMULTANEOUSLY STRAIGHTENED UP AND TURNED AROUND, HE FELT A SHARP PAIN IN HIS BACK. UPON MEDICAL EVALUATION, THE EE WAS DIAGNOSED WITH A STRAIN IN THE LOWER BACK. HE WAS PLACED ON RESTRICTED DUTY.
WHILE STEPPING FROM BED OF TRAILER TO THE TONGUE HIS FOOT SLIPPED ON CONTACT WITH TRAILER TONGUE AND HE FELL TO THE GROUND. RESULTING INJURIES REQUIRED MEDICAL ATTENTION AND SUTURES.

EE REPORTED PAIN IN LEFT WRIST/ HAND. UPON MEDICAL EVALUATION, THE EE WAS DIAGNOSED WITH CTS IN LEFT WRIST. EE UNDERWENT SURGERY ON NOV 10, 2003.
EE LOST CONTROL OF A HAUL TRUCK AND SLID INTO THE TOE OF THE SPOIL. UPON IMPACT, HE STRUCK HIS LEFT SHOULDER ON THE DOOR OF THE TRUCK. UPON LATER MEDICAL EVALUATION, THE EE WAS RESTRICTED FROM ALL WORK DUTIES. (OCTOBER 30, 2003). THE EE HADSURGERY ON NOVEMBER 4, 2003
THE EE NOTED PAIN IN BOTH ARMS AND HANDS. UPON MEDICAL EVALUATION SHE WAS DIAGNOSED WITH CTS. SHE BEGAN SURGICAL REPAIR ON SEPTEMBER 17, 2003 AND HAD A SECOND SURGERY ON SEPTEMBER 29, 2003.
EE WAS MOVING A BARRELL MOUNTED GREASE PUMP. WHEN THE PUMP WAS LIFTED THE EE FELT A PAIN IN HIS RIGHT SHOULDER. THIS RESULTED IN A STRAIN TO THE EE S RIGHT SHOULDER AND HE WAS PUT ON RESTRICTE D DUTY.
THE EMPLOYEE WAS DIAGNOSED AS HAVING CARPAL TUNNEL SYNDROME IN BOTH WRISTS. HE UNDERWENT SURGERY ON JULY 31, 2003 AND AUGUST 11, 2003. HE HAS SINCE RETURNED TO FULL DUTY.
WHILE CLIMBING A LADDER TO THE MACHINE CAB, THE EMPLOYEE STEPPED ON THE EDGE OF HIS RIGHT BOOT WITH HIS LEFT FOOT. WHEN HE ATTEMPTED TO MOVE HIS RIGHT FOOT AGAIN, HE FELT A PAIN IN HIS RIGHT H IP/LEG. THIS RESULTED IN A STRAIN TO THE MUSCLES AND HAMSTRING IN THE RIGHT LEG. THE EMPLOYEE'S WORK DUTIES WERE RESTRICTED.
EE WAS WALKING ON SNOW COVERED GROUND WHEN SHE SLIPPED AND FELL. THE EE FELL TO HER BUTTOCKS. THE IMPACT WITH THE GROUND CAUSED A BODY REACTION THAT AFFECTED HER LEFT EAR. UPON MEDICAL EVALUAT ION SHE WAS DIAGNOSED AS HAVING CONTUSIONS AND SYMPOTOMS OF VERTIGO.
UPON MEDICAL EVALUATION, THE EE WAS DIAGNOSED WITH CTS. THE EE IS UNDERGOING SURGERY FOR BOTH WRISTS ON DECEMBER 2, 2003.
EE WAS LIFTING A BOX OF PRIMERS IN THE EXPLOSIVE MAGAZINE. HE FELT A SHARP PAIN IN HIS LOWER BACK. UPON MEDICAL EVALUATION, THE EE WAS DIAGNOSED WITH A STRAIN IN THE LOWER BACK. THE EE WAS PLA CED ON RESTRICTED DUTY.
THE EE REPORTED THAT HE HAS REPETATIVE MOTION INJURIES WITH BOTH WRISTS. THIS WAS CONFIRMED BY A NEUROLOGY EVALUATION. THE EE HAD SURGICAL REPAIR OF THE LEFT WRIST ON 4-24-03, AND SURGICAL RE PAIR OF THE RIGHT WRIST ON 4-14-03. THERE WAS NOLOST TIME DAYS. EE IS ON RESTRICTED DUTY.
EE WAS ATTEMPTING TO REMOVE A HYDRAULIC FITTING WHEN HE FELT A PAIN IN HIS RIGHT SHOULDER. AFTERCONSIDERABLE MEDICAL EVALUATIONS, THE EE WAS DIAGNOSED WITH AN R.A-C JOINT INJURY. HE UNDERWE NT SURGIGAL REPAIR ON 10/7/03 AND REMAINS OFF WORK.

HE WAS PLACED ON RESTRICTED DUTY AFTER FURTHER EVALUATION WHEN HE RETURNED FROM VACATION.

EE SLIPPED & FELL ON ICE. HE FELL ONTO HIS LEFT SIDE & SUSTAINED CONTUSIONS & STRAIN TO THE LEFT SHOULDER, HIP & ELBOW. UPON FURTHER MEDICAL EVALUATION, HE WAS SCHEDULED FOR SHOULDER SURGERY.
THE EE REPORTED LOWER BACK AND NECK PAIN AFTER OPERATING A PIECE OF EQUIPMENT. UPON MEDICAL EVALUATION THE EE WAS DIAGNOSED AS HAVING A STRAINED LOWER BACK. THE EE RETURNED TO WORK ON RESTRIC TED DUTY.
THE EE WAS ATTEMPTING TO MOVE SOME PLASTIC PIPE.HIS FOOT SLIPPED CAUSING HIS BODY TO REACT IN REGAINING HIS BALANCE. THE EE FELT A SHARP PAIN IN HIS LOWER BACK AND ABDOMEN. UPON MEDICAL EVALUATION THE EE WAS DIAGNOSED WITH A STRAIN TO THE LOWER BACK AND ABDOMEN. THIS RESULTED IN RESTRICTED DUTY.
UPON EXITING THE CAB OF THE WATER WAGON, THE EE SLIPPED ON THE DECK AND FELL TO THE GROUND. THISRESULTED IN THE EE SUSTAINING A RUPTURED SPLEEN.

WHILE WORKING IN A FIELD AREA, THE WIND BLEW A PIECE OF DIRT INTO THE EE S LEFT EYE. THIS MATERIAL BECAME IMBEDDED INT HE CORNEA AND WAS REMOVED AT ON SITE MEDICAL DEPT. PERSCRIPTION DRUGS WER E USED DURING THE REMOVAL PROCEDURE.
EE WAS TRANSPORTING #401 HAUL TRUCK FROM THE FIELD TO THE SHOP AFTER REMOVING THE DUMP BED. THE STIFF SUSPENSION ON THE TRUCK CAUSED HIM TO STRIKE THE BACK OF THE SEAT WITH HIS LOWER BACK. THI S RESULTED IN A STRAIN TO HIS LOWER BACK. EE WAS PLACED ON RESTRICTED DUTY.
EE WAS ATTEMPTING TO DISMOUNT A 789 HAUL TRUCK UTILIZING THE LADDER AND STEPS. AS HE STEPPED FROM THE BUMPER TO THE RUBBER STEPS HE PLACED HIS RIGHT FOOT ON A NUT AND BOLT INSTEAD OF THE RUNG OF THE STEP. WHEN YIS WEIGHT WAS TRANSFERRED TO THE RIGHT FOOT HE FELT A POP. XRAYS REVEALED A FRACTURED BONE IN HIS RIGHT FOOT.
EMPLOYEE WAS PARTICIPATING IN EMERGENCY RESPONSE TRAINING. WHILE RESPONDING TO A SIMULATED ACCIDENT, THE EMPLOYEE GOT DIZZY AND FELL BACKWARDS. AS HE FELL, HE STRUCK THE BACK OF HIS HEAD AGAIN ST A TRAILER HITCH. THIS CAUSED A LACERATION THAT REQUIRED 6 SUTURES TO CLOSE.
EMPLOYEE WAS IN THE PROCESS OF CHANGING THE CONNECTOR LINK ON THE BOOM ENS OF THE BUCKET CYLINDER ON 757 PAGE DRAGLINE. WHILE ATTMEPTING TO GUIDE THE CONNECTOR LINK AND CYLINDER AS IT WAS LOWE RED INTO THE BOOM CHANNEL, THE CONNECTOR LINK ROTATED AND SMASHED THE EE'S LEFT RING FINGER AGAINST A FITTING ON THE CYLINDER. CAUSED LEFT RING FINGER TO BE LACERATED.
EE REPORTED PAIN IN THE LEFT WRIST. MEDICAL DIA\GNOSIS WAS CARPAL TUNNEL SYNDROME. EE HAD SURGERY TO CORRECT THE PROBLEM.
THE EE WAS HOLDING THE BOTTOM OF A GROUND ROD DRIVING TOOL TO HELP ANOTHER EE, WHO WAS HOLDING THE TOP, DRIVE THE GROUND ROD IN. THE EE'S LEFT HAND SLIPPED OFF THE BOTTOM OF THE TOOL DURING IT S UPWARD MOTION. THE TOOL CAME OFF THE ROD. DURING THE DOWNWARD MOTION, THE EE'S LEFT INDEX FINGER WAS LACERATED WHEN CAUGHT BETWEEN THE TOOL AND THE ROD. THIS RESULTED IN THE FINGER REQUIRIN
THE EE WAS HOLDING A 5 1/2" SOCKET. ANOTHER EE WAS HOLDING A 4# SLEDGE HAMMER TO HIT THE SOCKET TO SEAT A BUSHING. THE HAMMER DEFLECTED OFF A NEARBY PIECE OF METAL STRIKING THE EE'S RIGHT THUM B. THIS RESULTED IN EE'S THUMB BEING FRACTURED. ALTHOUGH THE ACTUAL EVENT TOOK PLACE ON 5-5-02, IT WAS NOT DETERMINED THAT A FRACTURE EXISTED UNTIL 5-6-02 WHEN THE EE OBTAINED XRAYS.
EE WAS WORKING ON A COAL HOPPER. THE WIND BLEW A PIECE OF COAL DUST INTO THE EE'S LEFT EYE. THIS RESULTED IN THE REMOVAL OF A CORNEAL FOREIGN BODY AND AN ABRASION TO THE CORNEA OF THE EE'S LEF T EYE. IT IS NOTED THAT THE EE WAS WEARING PROPER EYE PROTECTION AT THE TIME OF THE INCIDENT.
EE WAS CLEANING THE TRACKS ON A DOZER WHEN HE FELT A PULL IN HIS LOWER BACK. AS MEDICAL TREATMENT, THE EE RECEIVED DIATHERMY TREATMENT.

WHILE CARRYING A COVERALLS AND WALKING BETWEEN THE SHOP AND OFFICE BUILDINGS, THE EE FELT A SHARP PAIN IN HIS LOWER BACK. UPON MEDICAL EVALUATION, THE RESULTING INJURY WAS CLASSIFIED AS A STRA IN/SPRAIN IN THE LOWER BACK. THE EE S WORK ACTIVITY WAS RESTRICTED BY THE ATTENDING PHYSICIAN.
THE EE WAS UTILIZING A HAMMER TO REMOVE A BACKUP WRENCH. AS HE STRUCK THE WRENCH IT FLEW UP AND STRUCK THE EE IN THE MOUTH. THIS RESULTED IN A LACERATION TO THE UPPER LIP.
EE WAS OPENING THE DOOR TO THE WAREHOUSE BARN. THE DOOR CAUGHT ON A CONCRETE PAD. SHE FELT A POPIN HER RIGHT WRIST RESULTING IN A STRAIN/SPRAIN.UPON MEDICAL EVALUATION SHE WAS PLACED ON RESTRI CTED DUTY.
THE EMPLOYEE WAS SPRAYED WITH DIESEL WHEN THE FUELING CONNECTION STUCK OPEN AS THE FUELING NOZZLE WAS REMOVED AFTER FUELING. SOME OF THE FUEL ENTERED THE EMPLOYEE'S EYES. THIS RESULTED IN A CORNEAL BURN AND REQUIRED TREATMENT WITH PRESCRIPTION MEDICATION.
THE EE REPORTED FEELING AN IRRITATION IN HIS LEFT EYE. THE EYE WAS FLUSHED SEVERAL TIMES AND NOTHING WAS FOUND. THE DAY AFTER THE INITAL REPORT, THE EE CONTINUED TO EXPERIENCE IRRITATION AND S AW THE ON-SITE PA-C. A RESULTING CORNEAL ABRASION WAS TREATED WITH PERSCRIPTION MEDICATION.

WHEN CHANGING A TRACK ON THE DOZER, THE CHAIN HOOK SLIPPED OFF THE BOGIE. THE EMPLOYEE'S ARM WAS CAUGHT BETWEEN THE BOGIE AND THE TRACK. THIS RESULTED IN A SIGNIFICANT CONTUSION TO THE EMPLOYE E'S RIGHT FOREARM. THIS INJURY RESULTED IN RESTRICTED DUTY FOR THE EMPLOYEE. HOWEVER, EE WORKED 4 ON/4 OFF SHIFTS & 4 SHIFT CANCELLED. HE RETURNED NEXT SHIFT AT FULL CAPACITY.
EMPLOYEE WAS STEPPING OFF OF A SMALL TRAILER WHEN HE STUMBLED FALLING TO THE FLOOR. EMPLOYEE DISLOCATED HIS RIGHT ELBOW. HE RETURNED TO WORK ON HIS NEXT SCHEDULED SHIFT.
EMPLOYEE WAS PLACING PILLOW BLOCKS ON A PALLET WHEN HIS LEFT RING FINGER WAS PINCHED BETWEEN TWOPILLOW BLOCKS RESULTING IN A LACERATION. THE EMPLOYEE RECEIVED THREE SUTURES AND RETURNED TO WOR K THE FOLLOWING SHIFT.
EMPLOYEE WAS IN THE PROCESS OF ATTACHING A CYLINDER TO A MOUNTING BRACKET ON A TOW-HAUL TRAILER HITCH. WHILE ATTEMPTING TO LINE UP THE CYLINDER AND BORE HOLE ON MOUNTING BRACKET, THE EMPLOYEE PLACED TWO FINGERS INTO THE BORE HOLE TO CHECK FOR ALIGNMENT. THE TOE OF THE HITCH SUDDENLY DROPPED, WHICH CREATED A PINCH POINT. THE TIPS OF THE EMPLOYEE'S RIGHT HAND MIDDLE AND RING FINGE
THE EE WAS LIFTING LARGE WOOD BLOCKS WHEN HE FELT A POP IN HIS BACK. THE EE CONTINUED TO WORK REGULAR DUTIES OVER TIME. THE INJURY WAS LATER DIAGNOSED AS A RUPTURE DISK. THE EE UNDERWENT SURGE RY ON JANUARY 10, 2001, TO REPAIR THIS INJURY. THIS WAS THE FIRST DAY OF MEDICAL TREATMENT AND THE FIRST DAY MISSED A RESULT OF THE INJURY.
EMPLOYEE WAS OPERATING A WATER WAGON WHEN THE STEERING FAILED (HYDRAULIC HOSE FAILURE). THIS CAUSED THE WATER WAGON TO LEAVE THE ROADWAY. WHEN THE WATER WAGON CAME TO A STOP, THE EMPLOYEE STRU CK THE SIDE WINDOWS, WHICH WAS RESULTED IN A HEAD LACERATION. THE EMPLOYEE ALSO INCURRED TWO MINOR RIB FRACTURES ON THE LEFT SIDE OF HIS CHEST AREA. WAS WEARING HIS SEAT BELT.
EE WAS REMOVING AN EMULSION HOSE WHEN IT CAME FREE AND STRUCK HIS NOSE RESULTING IN A HAIRLINE FRACTURE.
THE EE WAS ATTEMPTING TO LOOSEN A BOLT. THE HAND TOOL BEING USED SLIPPED AND THE EE'S LEFT KNUCKLE STRUCK AGAINST AN ENGINE CASTING. THIS RESULTED IN A LACERATION TO THE LEFT KNUCKLE, WHICH RE QUIRED SUTURES.
THE EE WAS HOLDING A SLUGGER WRENCH ON A NUT. ANOTHER EE WAS USING A 10# SLEDGE HAMMER TO HIT THE WRENCH . THE HAMMER DEFLECTED OFF A PAD EYE AND STRUCK THE EE'S LEFT INDEX FINGER. THIS RESULT ED IN THE EE'S FINGER BEING FRACTURED.
EE WAS PLACING A DOOR COVER ON AN ELECTRICAL CABINET WHEN IT SLIPPED AND HIT HIS RIGHT HAND JUST ABOVE THE KNUCKLE OF HIS LITTLE FINGER RESULTING IN A LACERATION. THE EE RECEIVED 2 SUTURES AND IMMEDIATELY RETURNED TO REGULAR WORK DUTIES.

THE EE WAS PERFORMING MAINTENANCE ACTIVITIES IN THE SHOP AREA WHEN A BRAKE DRUM FELL AND STRUCK HIM ON THE RIGHT FOOT. THIS RESULTED IN A CONTUSION TO THE EE'S RIGHT FOOT. AFTER MEDICAL EVALUA TION THE EE WAS RELEASED FOR WORK ON RESTRICTED DUTY.
EMPLOYEE WAS ATTEMPTING TO CLEAR DEBRIS FROM THE NOZZLE OF A PRESSURIZED CANISTER WHEN HE WAS SPRAYED IN THE LEFT EYE WITH SOLVENT. HE IMMEDIATELY FLUSHED THE AFFECTED EYE. THE RESULTING CHE MICAL BURN TO THE EYE WAS TREATED WITH PRESCRIPTION MEDICATION.
WHILE WORKING IN THE SHOP, THE EE'S FOOT SLIPPED. THE EE REPORTED A POP AND PAIN IN THE GROIN AREA. THE EE CONTINUED WORKING AND HAS NOT LOST ANY TIME UNTIL NOW. THE EE CONTINUES TO HAVE PROBL EMS. UPON MEDICAL EVALUATION AND MRI TESTS (12-14-99) THE EE WAS FOUND TO HAVE A RUPTURED DISK.
A TEMPORARY SUPPORT STRUCTURE PLACED BRETWEEN THE MAST AND BOOM OF A DRAGLINE PARTIALLY FAILED. THE TOP CROSS MEMBER OF THE STYRUCTURE STRUCK THE EE WHO WAS IN THE MANBASKET OF AN AERIAL LIFT. THE EE INCURRED INTERNAL INJURIES WHICH REQUIREED SURGICAL REPAIR AND THE EE'S RIGHT WRIST WAS DISLOCATED AND FRACTURED.
WHILE THE EE WAS PLACING DRILLS TAKES, THE WIND BLEW SOME DUST INTO HIS EYE. THE EE, LATER THAT EVENING, WENT TO THE HOSPITAL AND RECEIVED AN EYEWASH TREATMENT. PRESCRIPTION MEDICATIONS WERE A DMINISTERED. IT IS NOTED THAT THE EE RETURNED TO WORK ON HIS NEXT SCHEDULED SHIFT. NO TIME WAS LOST.
THE EE REPORTED FEELING AN IRRITATION IN HIS LEFT EYE. THE EYE WAS FLUSHED & NOTHING WAS FOUND\ 2 DAYS AFTER THE INITIAL REPORT THE EE CONT' TO EXPERIENCE IRRITATION & WENT TO AN OPTOMETRIST A RESULTING CORNEAL ABRASION REQUIRED RX MED
THE EE WAS POLISHING A SHAFT ON THE LATHE. WHEN THE EMERY CLOTH WAS PULLED UNDER THE SHAFT, THE EE'S JERSEY GLOVE WAS ALSO PULLED INTO THE ROTATING SHAFT. THIS RESULTED IN THE EE'S RIGHT MIDDL E FINGER(JUST ABOVE THE MIDDLE KNUCKLE) TO BE PULLED COMPLETELY OFF HIS HAND.
THE EE WAS REMOVING THE FRONT TRANSMISSION/DIFFERENTIAL ASSEMBLY FROM A SCRAPER. THE ASSEMBLY WAS SUPPORTED BY AN OVERHEAD CRANE. WHEN THE LIFTING EYES ON THE ASSEMBLY FAILED, A PRY BAR BEING USED TO SEPERATE THE ASSEMBLY FROM THE HOUSING STRUCK THE EE'S RIGHT HAND. THIS RESULTED IN A FRACTURE TO THE EE'S RIGHT HAND.
EE WAS PERFORMING MAINTENANCE DUTIES ON A D-11 DOZER USING A CUTTING TORCH. AS HE WAS CUTTING ON A TRACK PAD A PIECE OF METAL FLEW UP & STRUCK HIM ON THE FOREHEAD RESULTING IN A LACERATION THE LACERAION REQUIRED 5 SUTURES. THE D-11 DOZERWAS DONW FOR REPAIRS & WAS NOT BEING OPERATED
EE WAS WORKING ON POSTS. WHILE WALKING TO THE TRUCK SHE SLIPPED ON SNOWY GROUND AND FELL CAUSING MULTIPLE LACERATIONS TO HER FACE. THESE REQUIRED SEVERAL SUTURES.
EMPLOYEE WAS PERFORMING MAINTENANCE ACTIVITIES USING A HAMMER AND CHISEL. DURING THIS PROCESS THE CHISEL BOUNCHED UP AND STRUCK HIM ON THE LEFT SIDE OF HIS MOUTH. THIS ACTION CUT THE LOWER LI P REQUIRING SEVERAL (4) SUTURES.
THE EMPLOYEE SLIPPED AND FELL WHILE WALKING THROUGH THE EMPLOYEE'S PARKING LOT. THIS RESULTED IN INJURIES TO THE EMPLOYEE'S RIGHT WIST, NECK, AND LOWER BACK. THIS EVENT OCCURRED PRIOR TO THE BEINNING OF THE EMPLOYEE'S SHIFT. IT MUST BE NOTED THAT THE EMPLOYEE WAS TAKEN OFF WORK BY HER PHYSICIAN EFFECTIVE 4-27-99.
THE EMPLOYEE WA PERFORMING FUELING ACTIVITIES AND SLIPPED AND FELL WHICH RSULTED IN A SLIGHT FRACTURE TO THE RIGHT WRIST. THE FALL WAS TO A DIFFERENT LEVEL. AT THE TIME OF THE SLIP AND FALL, THE EMMPLOYEE WAS FUELING A MOTOR GRADER WHICH WAS NOT BEING OPERATED.
THE EE WAS PUSHING A WHEELBARROW ACROSS THE YARD WHEN SHE SLIPPED AND FELL. SNOW IN THIS AREA HAD RECENTLY BEEN REMOVED; HOWEVER, THE SURFACE WAS STILL WET. THE EE RESUMED NORMAL WORK ACTIVITI ES AT THE TIME OF THE ACCIDENT. FURTHER MEDICAL EVALUATION RESULTED IN SURGICAL REPAIR OF THE MENISCUS ON 6-24-99.
EE WAS ATTEMPTING TO PURGE A NEW PROPANE GASE LINE. HE DID NOT BELIEVE HE HAD PROPANE IN THE LINE, WHEN HE ACTUALLY DID. AN ELECTRIC WATER HEATER IN THE ROOM APPARENTLY CYCLED, SPARKING AND IG NITING THE PROPANE WHICH WAS LAYING ON THE FLOOR, RESULTING IN FACIAL AND HAND BURNS.

THE EE WAS REMOVING A DUST COLLECTOR HOSE. WHEN THE HOSE CAME LOOSE FROM THE CONNECTING FITTING. THE EE LOST HIS BALANCE AND FELL TO THE GROUND. THE EE LANDED PARTIALLY ON HIS WRIST. THIS RESU LTED IN A SPRAIN AND CONTUSIONS TO THE LEFT WRIST.
EE SAID THAT HE WAS CARRYING REBAR FROM LAYDOEN AREA, TO A SITE WHERE MATS WERE BEING TIED FOR SLAB PALCEMENT. AS HE WAS WALKING, HE SAID THAT HE FELT SOMETHING POP IN HIS BACK. AT THE TIME HE SAID HE THOUGHT NOTHING OF IT AND SAID NOTHING. HE CONTINUED TO WORK THE REST OF THE DAY. LATER IT STARTED TO CAUSE HIM PAIN. DIDN'T REPORT IT TIL MONDAY. WITNESSES KNEW NOTHING.
EE WAS STRIPPING CONCRETE WALL FORMS. WHILE PRYING ON INSIDE CORNER OF WALL FORMS, HIS PRY BAR SLIPPED, CAUSING A LACERATION TO HIS RT FOREARM ON A WALL TIE. HE RECEIVED SUTURES.
EE ARRIVED EARLY FOR WORK AND WAS CURLING HER HAIR IN THE LOCKER ROOM. SHE DROPPED THE CURLING IRON AND IT FELL ON HER EYEBALL AND BURNED IT. IT WAS HER RT EYE. SHE WAS SEEN BY THE ON-SITE PA AND THEN SENT TO THE ER. SHE THEN WENT TO AN OPTOMETRIST.
EMPLOYEE WAS DISECTING A HYDRAULIC OIL FILTER. DURING THIS PROCESS, THE EMPLOYEE STRUCK HIS RIGHT HAND AGAINST A RAGGED EDGE ON THE FILTER. THIS ACTION CUT THE EMPLOYEE'S RIGHT HAND REQUIRING SEVERAL (15) SUTURES. EMPLOYEE WAS DOING MAINTENANCE DUTIES AT THE TIME OF THE ACCIDENT.
BASED UPON MEDICAL EVALUATION THE EE WAS DIAGNOSED AS INCURRING CARPAL TUNNEL SYNDROME ON BOTH THE LEFT AND RIGHT WRISTS. CORRESPONDENCE FROM THE EE'S PHYSICIAN STATES THAT HE BELIEVES THIS CO NDITION TO BE JOB RELATED. THIS LONG TERM CONDITION IS CATEGORIZED AS AN OCCUPATIONAL ILLNESS.
WHILE MOVING A CUTTING EDGE,THE MATERIAL FELL TO THE FLOOR AND BOUNCED ONTO THE EMPLOYEE'S RIGHT FOOT. THIS RESULTED IN INJURIES TO THE EMPLOYEE'S BIG AND SECOND TOE. SURGERY WAS REQUIRED TO R EDUCE A DISLOCATION OF THE BIG TOE. ADDITIONALLYTHE SECOND TOE OF THE RIGHT FOOT WAS FRACTURED DURING THIS ACCIDENT.

EMP WAS SPRAYED WITH DIESEL FUEL WHEN A CONNECTION NOZZLE POPPED OFF FROM FUEL TANK CONNECTION. SOME OF FUEL SPRAY ENTERED EMPS EYES. THIS RESULTED IN A CHEMICAL BURN IN EYES WHICH REQUIRED TR EATMENT WITH PRESCRIPTION MEDICATION.

WHILE TURNING A LARGE GEAR ASSEMBLY OVER TO INSTALL A BEARING ON THE OTHER END, THE EMPLOYEE'S FINGER WAS CAUGHT BETWEEN A 2" NYLON SLING BEING USED TO LIFT THE ASSEMBLY AND THE GEAR. THIS RES ULTED IN A FRACTURE, A LACERATION-REQUIRING SUTURES- AND CONTUSIONS TO THE LITTLE FINGER ON THE EMPLOYEE'S RIGHT HAND.
WHEN THE EMPLOYEE EXITED THE WORK AREA, A GUST OF WIND BLEW FOREIGN MATERIAL INTO HIS EYES. A PIECE OF RUST LODGED IN A PUPIL OF THE EMPLOYEE'S RIGHT EYE. THIS NECESSITATED THE REMOVAL OF A CORNEAL FOREIGN BODY.

AFTER THE SHIFT, EE EXPERIENCED MORE DISCOMFORT AND EVENTUA LLY HAD A FOREIGN BODY REMOVED FROM HIS LEFT EYE. THE EE DID NOT KNOW SPECIFICALLY WHEN OR HOW THE MATERIAL ENTERED HIS EYE.
WHILE BACKING A PICKUP TRUCK, EMP STRUCK AN OUTRIGGER OF A CRANE. EMP PULLED FORWARD AND LEFT THE PICKUP TRUCK TO SEE WHAT HE HAD HIT. PICKUP ROLLED BACKWARD AND EMP GOT PINCHED BETWEEN BUMPER OF TRUCK AND CRANE OUTRIGGER, VERY BRIEFLY, AND PUSHED HIMSELF AWAY. EMP INCURRED ABRASIONS, CONTUSIONS, AND INTERNAL INJURIES INVOLVING THE L.KIDNEY, PANCREAS, SPLEEN, BOWEL, AND L. LUNG.

THE RESULTIN G INJURY INCLUDED A FRACTURE TO THE LEFT WRIST, SPRAINED RIGHT WRIST, A SMALL LACERATION TO THE LEFT SIDE OF THE FOREHEAD AND AN ABRASION TO THE LEFT KNEE.

THIS RESULTED IN CONTUSIONS A ND ABRASIONS TO HIS RIGHT ARM, RIGHT LEG, AND RIGHT SIDE OF HIS RIBS. ADDITIONALLY, THE EMPLOYEEINCURRED A BROKEN TOOTH. SUTURES WERE REQUIRED UPON REMOVAL OF THE TOOTH.
WHILE TROUBLE SHOTTING AN ELECTRICAL PROBLEM IN THE CONTROL BOX OF A CRANE INSIDE THE DRAGLINE HOUSE THE EE CONTACTED AN ENERGIZED CIRCUIT WITH HIS HAND. WHEN THE EE PULLED HIS HAND AWAY FROM THE CIRCUIT HIS HAND STRUCK AGAINST A SHARP OBJECT IN THE CABIENT. THIS RESULTED IN A LACERATION WHICH REQUIRED STITCHES.

THE RES ULTING INJURY WAS A FRACTURED METACARPAL OF THE LITTLE FINGER ON THE RIGHT HAND. EQUIPMENT WAS NOT OPERATING AT THE TIME OF THIS OCCURRENCE AND IT WAS NOT A CONTRIBUTING FACTOR.
WHILE ARC GOUGING IN A TUB COMPARTMENT, DEBRIS FROM THE ARCING BLEW INSIDE EMPLOYEE'S WELDING HOOD AND BEHIND HIS SAFETY GLASSES.
EMPLOYEE WAS ADJUSTING TIE ROD DRAG LINK. HE WAS USING A COLD CHISEL AS A WEDGE IN THE COLLAROF THE DRAG LINK. THE CHISEL FLEW OUT STRIKING THE EMPLOYEE IN THE UPPER LIP. RESULTING IN A LACERATION AND BREAKING A TOOTH. THE LACERATION REQUIRED SUTURES.
EMPLOYEE WAS MOVING A SMALL PUMP FROM A WORKBENCH TO A SHELF UNDER THE WORKBENCH WHEN HE FELT A POPPING SENSATION IN HIS RIGHT GROIN AND UPPER ABDOMEN. EMLOYEE CONTINUED TO WORK HIS REGULAR JOB ASSIGNMENT AT THAT TIME. ON 8-10, EE WAS EVALUTED BY A DR AS HAVING INCCURRED A HERNIA. SURGICAL REPAIR WAS CONDUCTED ON 8-11 WITH DAYS AWAY FROM WORK BEGINNING ON 8-11.
WHILE WELDING IN TUB COMPARTMENT #22, THE AIR HOSE CAME LOOSE FROM FITTING ON AIR CHIPPER. THE HOSE BLEW TRASH AND DUST IN EE'S RIGHT EYE. EE WAS WEARING HIS SAFETY GLASSES AT THE TIME OF INCI DENT.
EMPLOYEE HAD BEEN GRINDING AND WELDING DURING SHIFT.CONDITIONS WERE DUSTY AND VERY WINDY.EMPLOYEE FINISHED SHIFT,CLEANED UP AND WENT HOME.HIS EYE THEN BEGAN TO BOTHER HIM.UPON RETURNING TO WOR K,EMPLOYEE HAD PA LOOK AT HIS EYE.PA REMOVED A CORNEAL FOREIGN BODY FROM LEFT EYE AND TREATED.
THE EMPLOYEE WAS ATTEMPTING TO ADJUST THE DOZER SEAT WHEN THE SEAT ADJUSTMENT ALLOWED THE SEAT TO DROP TO THE LOWER POSITION (APPROX. 6" ). THIS CAUSED A BODILY REACTION WHICH RESULTED IN AN A BDOMINAL HERNIA.
THE EMPLOYEE WAS PULLING ON A SLING ATTACHED TO THE DRAG ROPE (PULLING THE DRAG ROPE INTO THE PROPER LAY OF THE DRUM). AT THAT TIME, THE EMPLOYEE FELT A PAIN IN HIS KNEE. THIS ACCIDENT RESULTE D IN DAMAGE TO THE MUNISCUS IN THE LEFT KNEE, WHICH NOW (1996) REQUIRES SURGICAL REPAIR.
WHILE DRIVING A LUBE TRUCK, THE EMPLOYEE HIT AN ICY PORTION OF ROADWAY. THIS CAUSED THE TRUCK TO SLIDE & EVENTUALLY TO ROLL OVER [3/4 TURN]. THIS ACCIDENT RESULTED IN A CONTUSION ON THE RIGHT HIP OF THE EMPLOYEE.
EMPLOYEE WAS OPERATING A HAUL TRUCK AND WAS IN THE PROCESS OF BEING LOADED BY A BE SHOVEL. WHILE BEING LOADED, A LARGE ROCK FELL FROM THE SHOVEL BUCKET AND HIT THE BACK OF THE HAUL TRUCK BED J ARRING EMPLOYEE IN THE CAB.
