Employee was loosening a nut which broke loose suddenly and pinched his middle finger between the wrench and angle iron frame, resulting in a fracture to the middle finger of his right hand.

The employee was pushing the feed box on the #1 dryer when the #1 centrifuge plugged off and sent slurry over the feed chute resulting in burns to the employees right side including his right forearm.

Employee stepped in a hot soda ash pile which entered his boot resulting in burns to his foot.

Employee was putting a 4 inch valve on a pallet with 6 inch valves and pinched her finger between the handle stems resulting in a laceration and 5 sutures.

Employee was checking a leak near an oil plug on the C boiler ID fan in the Powerhouse when he reached up under the coupling gaurd and came in contact with the RPM sensor resulting in fractures and lacerations to his left hand and fingers.

Employee was taking the drive belts off of a circulator and pinched the top of his little finger between the belt and sheave. The right little finger required sutures.

A small piping hole was observed near slot 3 on the downstream slope of D cell.

As an employee was bending conduit, a piece of bent conduit fell and struck the employees finger. On April 3, 2013 the employee received an X-ray which revealed a fracture on the left middle finger. The employee immediately returned to work with no restrictions.

Employee was sliding a piece of metal plate when it slipped out of her hands landing on her toe and breaking it.

Employee got "trona rash" on his legs from the #2 mud tank.

Employee strained his shoulder operating the north feed water valve on E boiler. Employee returned to work and worked normal duties until he went off on surgery on 1/10/13.

Employee was priming a pump when the pump caught the prime and blew water causing him to jump back and land in hot water that went inside his boots causing 2nd degree burns to his foot.

Employee opened a prime valve on pump when hot water sprayed on his face.

Employee was cleaning ducon when steam blew out and burned his left side of face and left arm.

Employee was walking under dryer when he stepped on piece of grading and fell on the floor.

Employee was crawling in cable tray pulling fiber optic cable and upon completion he felt some stiffness in his knees and felt he may have hyper extended his knee. On March 26, employee began to experience more discomfort in his knee. He went in and got an MRI and was diagnosed with a partially torn meniscus.

Upon receiving a modification of a K-Order that was issued on D-Cell, we began filling D-Cell with water and tailings from the process plants. After we began discharging into D-Cell, the recently repaired area of the impoundment began to leak again. We immediately took action by discharging the water and tailings elsewhere.

The affected experienced a hernia while pushing on a heavy pulley while installing the pulley.

Employee was lifting gasket from expansion joint when he cut his pinkie.

Employee was pulling on hub when it popped off hitting him on left leg and he tripped over bearing box.

Employee was walking up stairs when slurry was draining towards him, employee then jumped down five stairs to get away from it and broke his left foot.

Employee was opening a steam valve when he felt a burning on the right side of his abdomen. Employee went on MLOA on 12/26/2011.

Employee was on the forklift when witness observed him slump over. Witness immediately got him off of the forklift and laid him on the ground where rescue member began treating him. Employee was the transported to local hospital where he later died of natural causes.

Employee was walking when he stepped in a hole and felt pain in his right knee. Employee went on MLOA on December 23,2011.

While contractors were removing damaged slope plate out of #5 product bin soda ash sloughed from an unknown location into bottom of bin. All personnel were evacuated with EE in the bin thigh high in material for several minutes and another EE assisted out right away w/another EE. 7000-1 submitted electronically on or about 11/30/11 but unable to locate submitted 7000-1.

EE inhaled soda ash, was treated by EMT onsite and released. EE's reaction to exposure required follow-up treatment at hospital. After treatment, EE was given full release to work. EE quit job before follow-up treatment was sought.

Employee was opening a valve on a filter when he felt a cramp. The next morning his hand was swollen and sore. Employee was put on restricted duty on 9/13/11.

Employee was operating a pipe threader when it caught his right middle finger between the pipe and pipe threader.

Employee was cutting handrail when sawzall kicked and cut his index finger on left hand.

After parking, employee got out of his vehicle and rolled his ankle.

Plant personnel noticed an increase in flow from the seepage area that was reported on 1/8/2012. Upon discovery it was immediately reported to MSHA and we instructed MSHA at the 1-800 number that this had been previously reported to MSHA but we were reporting the increase in flow.

Plant personnel noticed increased seepage from d cell. Upon investigation a boil was noticed on the downstream slope along with a small sinkhole. Upon discovery it was immediately reported to MSHA as having a potential to be an unstable condition at an impoundment.

EE slipped on stairs landing on his left arm and shoulder.

Employee was inspecting bull gear and pinion when he lost his balance and smashed his left hand in between the dryer tire and the trunnion.

Contractor was filling oil tank on the 2nd floor of 6RR when the tank overflowed and spilled on #5 calciner causing the oil to ignite the fire.

Employee injured his left shoulder while beating on ore bin on Aug 9, 2010. Employee went off on MLDA for shoulder surgery on 1/19/11.

Employee was driving forklift through double doors when the door blew shut. Employee tried to stop it but got his hand caught between the door and the cab of forklift.

While bolting bearing box to table, employee took a bolt out of the bottom which caused the box to come loose and pinned his finger between box and table.

EE was cutting belt skirting, when his knife slipped, cutting his left thumb.

EE was replacing block on rotex screen when the block slid into place smashing his fingers between block and frame. He received stitches.

Employee was working on a calciner when the wrench he was working with slipped off the nut and he smashed his left index finger wich required stiches.

Employee was performing work inside of dryer. When he was climbing out he felt a pop in his right shoulder and stated that it didn't bother him until the next morning. Employee went on medical leave of absence on 1/21/10 for shoulder surgery.

Employee was attempting to unplug dryer discharge chute. When he applied water to the chute, the ash flashed causing 1st and 2nd degree burns to both arms.

Employees work area got flooded out. He then was unplugging extension cord and moving welding lead from area. He stepped on grating over trench, when grating flipped over causing his left foot to go in trench and burning it.

EE was doing area check when his left foot got caught in grading and twisted his knee.

EE was getting a water hose when he tripped and fell going from an incline to floor level and hurt his right hand.

EE was on scaffle opening valves, when the operation handle he was using slipped off the valve, causing him to lose his balance and fall and land on the floor.

Employee was lifting a guard and strained his back. This is ongoing from a previous injury dated 11/22/07. He missed 5 working days and has returned with restrictions.

EE was checking a pump guage. He turned and stepped over a 3' line. IN doing so, his left foot got caught on the line, causing him to trip and land on his L ankle and breaking it.

Employee was placed on medical leave of absence on 5/29/09 for hand surgery for carpal tunnel syndrone. This illness is continuel from his last surgery on 1/20/09 when he had the some procedure. This injury has been on going since 12/27/05.

After inspecting rotation on H09 transfer screw, he was removing a rag from inside the inspection door when the transfer screw caught his glove resulting in partial amputation to his index, middle and ring fingers of his right hand.

While prying cone out of classifier, pry bar slipped out of seam of cone striking employee on the upper lip creating a laceration which required stitches.

Employee got something in his eye from dust while he was working. Employee has infection in right eye.

While attempting to close the overhead steam valve on #4 dryer with a wrench while standing on a roll-away platform employee twisted his left knee.

Employee fell into hole while inspecting impoundment and was entrapped for approximately 20 minutes.

From repetitive motion with his hands over the years employee has developed carpal tunnel syndrome in his left hand, wrist and fingers. Injury is ongoing from 12/27/05

EE was walking to work on the road west of tunnel, when he slipped in the mud, falling on his right hip. He continued to work and perfomed his job duties that day.

Employee was closing a double door after he had walked through it and got his fingers on his left hand slammed in the door jam. Had swelling and bruising and had to have blood drained from thumb nail.

Employee was pulling on chain fall when he felt a strain in his left arm.

Employee worked all day doing normal duties. He stated he woke up the next morning and couldn't get out of bed. He thought he re-aggravated his back from a reoccured injury.

Employee stepped off of manlift in the basement floor of the crusher. When he stepped off he twisted his ankle causing it to break.

Employee was walking on top of rail car inside loading building. Employee appeared to have fallen between two rail cars on south side of train scale.

During belt project, belt was being pulled on; dust from upper structure got between face and dust mask causing burning feeling to face.

EE was trying to close #3 dryer overhead valve. The valve was very hard to operate. He hurt his right shoulder while struggling to close the valve. He was using a 36" pipe wrench as per usual.

While turning valves employee felt a pull to right side of belly. Employee restricted to no overtime for two weeks on 9/9/08.

Employee stepped down from Cat dozer and turned around when he felt a "pop" in his back, hip, or knee. Employee restricted to desk work only. Scheduled for MRI on 8/21/08.

Employee was descending west stairs in GR2 when he slipped on the bottom step and twisted his ankle. Employee restricted to no overtime on 8/12/08.

Employee was moving the choke on a pump when his left handcame in contact with the ait filter which was hot. This resulted in a second degree burn to his hand.

Employee was shoveling the JD606 conveyor at product loadout when she got hot and felt dizzy. Employee was observed in the clinic for three hours, re-hyrdrated, and restricted to working in a cool environment for the rest of the shift on 6/26 and on 6/27.

Employee worked on #2 Merrick belt shoveling and operated a 30" steam valve in GR3. On the ride home from work the employee's back became sore and locked up. Restricted to no overtime for two weeks on 6/26/08.

Employee burned himself with hot condensate after turning on a hose with a loose connection, 1st and 2nd degree burns to left leg, groin and arm.

Employee pulled on railroad car lid and felt pain in lower back.

Employee developed carpal tunnel, syndrome over the years from unknown causues, Scheduled for surgery and on MCOIA on 5/8/08.

EE was putting belts on FD101 screen in the crusher, when he felt a pull in his somach and groin area. EE restricted to no overtime.

EE was struck on the forehead by a piece of conduit, while cleaning #2 centrifuge.

Two employees were working together to clean up dust piles. EE was shoveling and other ee was using a pick. Other accidently stuck ee in the right forearm with the pick. The result in a puncture wound and sutures.

Employee was pulling wires to VFD and sustained a strain to his left shoulder. Restricted to no overtime on 5/1/08.

EE was coming out of the MCC on the 3rd floor of GR3 plant and looking up, when he tripped over a pump/motor that is welded to the floor. He fell on his right side. He was off 3/10,3/11, and 3/12 due to his injury. Returned to work with dr note 3/14.

While attempting to get #3 calciner to feed, employee swung at #3 bin bottom with hammer and felt pain in his back. Employee restricted to no overtime on 3/19/08.

Employee was carrying a 6" hose and slipped on another hose buried in snow. He fell and hurt his shoulder. He restricted to no overtime for 2 weeks on 6-18-08.

EE was picking up a pump and twisting when he heard a pop in his right knee. EE restricted to no overtime until injury is resolved.

EE was removing bolts out of the drive end of the #6 dryer feed screw on GR3 EE was using a bar to keep screws from turning, when he hit the bolt the screw moved causing the drive pin to push finger up against screw housing. This act resulted in sutures to the 4th digit on the left hand and a fracture to the left wrist.

An increase in seepage from the west embankment of our D cell trilings pond was observed. Upon further investigation, a void near the toe of the downstream slope on the west embankment was discovered; resutling in corrective action-including pumping, trenching, diking and diverting water from the plant. Cause still under investigation.

EE was installing a screen in the GR2 Deca centerfuge, when he cut his finger on the screen, which required sutures.

EE strained muscle in addomen while trying to get on Cat. Restricted to no over time for 2 weeks on 2/12/08.

2 mechanics were in the process of repairing floculent scres in GR3 process plant when they ignited guar gum dust. No injuries were sustained by either employee.

Employee was using a pry bar on a railroad car when his foot slipped and he fell backward onto packed snow. Employee had pain to lower back, right hip, and right side of head. (Restricted to no overtime until resolved on 1/24/08).

EE was reading clipboard, when he slipped and fell on the ice. EE struck his right hip on the ground and indicated that his clipboard struck his ribs (left front). EE limited to no overtime for 2 weeks.

Oiling gearbox for cooling tower fan in GR3, he fell from a ladder straining his right arm and shoulder. Restricted duty: no overtime.

EE stained his back while taking a screen out at product load out. Restricted to light duty on 12/6/07 (2 weeks).

Employee was bending down to place parts on a pallet when he felt his knee cap move causing him pain.

EE lost footing and hit hand rail on left side.

EE was walking down stairs and twisted his ankle. EE restricted to no overtime on 9/14/07.

Empoyee pinched finger while installing filter leaves requiring sutures.

Employee was pulling a feed tube funnel and pinched his hand between the funnel and the feed tube. EE's hand continued to bother him so he went to the doctor on 8/20 and they took x-rays. They determined he had a fractured 5th metacarpal.

Employee jammed left wrist while barring centrifuge restricted to no overtime for 10 days.

Employee cut finger on sharp edge of glass in door.

Employee was helping change out belt on conveyor and snub roller fell and hit him on the leg. He was given a restriction of no overtime for 10 days, making this injury reportable.

Employee was tying a hand brake on a single rail car when he lost his grip and fell backwards. He landed on his back on the rail.

Employee smashed his thumb while installing a race ring in a centrifuge.

EE was hitting a feed chute with a sledge hammer and felt a pull in his abdomen.

Original Report: Employee was pulling on a hose while cleaning the showers and felt a pull in his hip.**Amended Report: Employee was pulling on a hose and fell down, injuring his shoulder. Employee continued to work his regular job until 3/22 when he went off for surgery.**

Employee was climbing onto cat and slipped on the step straining his left quadrisep muscle.

EE was opening valve using electric valve wrench and pinched his finger, causing a laceration requiring sutures.

Employee was walking out the door and slipped on the ice and hurt his neck. He is restricted to no overtime.

Employee slipped and fell onto grating and lacerated his hand.

Employee was clearing a blockage in the feeder and his hand was struck by a rock.

Employee slipped on a stair and hit his elbow requiring sutures.

Employee stepped onto railcar and hit the side of his foot on door latch, causing fracture.

Employee was setting down an electrical cabinet and set the cabinet down onto his right middle finger causing a laceration requiring (2) two sutures.

Employee was diagnosed with carpal tunnel and went in for surgical repair on 10/25/06. Incident occurred over an extended period of time - no specific incident.

Employee was dropping conveyor belting to the ground and belting whipped him in the leg causing a friction burn abrasion.

Employee was performing maintenance on a calciner bull gear and was struck L. the ankle by a piece of falling metal.

On 8/11/06 employee reported that he has been experiencing some pain & tenderness in his right wrist. He was diagnosed with Carpal Tunnel Syndrome-no specific incident or causation.

Employee was removing worn flights out of a screw and cut his hand on a sharp flight requiring sutures.

Employee was cleaning up using a shop vac and strained his lower back while stretching the hose out.

Employee was opening a dump valve and got splashed with hot liquid, causing 2nd degree burn on ee's left arm.

EE walked through hot ash and burned his right foot.

Employee was removing an impeller off of a pump and cut his finger on a sharp vain on the impeller.

EE was removing a drain plug and pipe wrench slipped causing EE to hit his forearm on a piece of tin resulting in a laceration requiring sutures.

Customer light fixture fell from ceiling and im- pacted right index finger on platform.

Employee was walking and stepped on uneven surface and twisted his ankle. His ankle continued to bother him so he went to see a doctor. Employee had previous problems with his ankle and the doctor felt it warranted surgery. Surgery scheduled for 6/20/06. No restricted duty or lost time to this point.

Employee was lifting pipe and felt strain in his abdomen. This case is being disputed due to the fact that employee had a pre-existing hernia. Worker's comp. accepted this hernia as an exascerbation of a pre-existing condition on 5/24/06 determined to be work-related.

EMPLOYEE STATED HE WAS SWINGING A SLEDGE HAMMER ON #1 ORE BIN. HE FELT PAIN IN LOWER BACK.

Employee strained his lower back while pulling bags on Hivac or running 6" hose on Hivac.

Employee picking up shaft and cut finger in key way.

Employee was closing Warman valve using an electric valve wrench and got his finger pinched between the trigger on the valve wrench and a 6" pipe.

Man was working on the 4th floor of a surface mill when he was asked to go to the bottom level and act as a hole watch. Although the stairs were right next to the work area, he went approx. 60' to a manlift belt. He caught his right heel on the upper floor grating as the platform went down, tripping him back and causing him to fall off the platform about 7' to the deck below.

Not attributed to a single incident - due to repetitive use of left arm over time. Employee is restricted to no overtime.

While opening valve using a valve wrench, Employee lost footing and fell onto tailbone.

EMPLOYEE WAS WALKING DOWN STAIRS AND HOOKED TOE OF HIS BOOT ON STAIR GRATING CAUSING HIM TO TWIST HIS KNEE. HE REPORTED THE INJURY ON 8/24

EE WAS WALKING AND SLIPPED ON WET FLOOR INJURING RIGHT ANKLE

While working on dryer housing, employee got his leg pinched.

Ee was lowering extension ladder & caught left ring finger between two of the rungs.

EMPLOYEE WAS REMOVING THE NOZZLE TO THE ALUMINUM PIPING ON THE HI-VAC TRUCK WHEN HAND SLIPPED AND EMPLOYEE STRUCK LEFT THUMB AGAINST EDGE OF PIPE

Employee was performing maintenance on filter and filter leaf support bar fell onto ee's neck and shoulder area.

EE CAUGHT LEFT HAND BETWEEN SHAFT AND GUARD WHILE REMOVING FAN AND SHAFT OUT OF MOTOR

EE was walking and fell. When ee attempted to catch himself, he injured his shoulders.

Employee caught his rt. boot under grating & twisted his rt. knee. Incident occurred on 3/6/05 & EE did not see a Dr. until 3/14/05.

EE felt pain in upper left arm and shoulder area when lifting.

EE was swinging sledge hammer and hit his left foot.

EMPLOYEE WAS LOOSENING BOLTS ON FLOW METER. AS HE PULLED ON THE WRENCH, THE BOLT SUDDENLY BROKE LOOSE & HURT HIS LEFT SHOULDER.

WHILE PULLING WIRES, EMPLOYEE BEGAN TO FEEL PAIN IN HIS LEFT SHOULDER. REPORTED 1/3/05.

Employee slipped on some ice and twisted his back.

Employee was climbing/working inside precip and felt a pain in groin area when he climbed out.

EMPLOYEE WAS WALKING ALONG RAILROAD CARS TO SET BRAKE ON THE SCAMP AND TRIPPED ON UNEVEN SURFACE, TWISTING LEFT KNEE.

EMPLOYEE WAS WALKING AND SLIPPED ON FOAMY WATER, SPRAINING RIGHT ANKLE.

EE was bent over replacing a valve on #1 filter and strained back. Reported 10-14-04.

EMPLOYEE WAS USING A HAMMER WRENCH TO LOOSEN BOILER DOOR & HIT LEFT HAND WITH 8LB. SLEDGE HAMMER.

EE WAS OPENING DRAIN VALVE AND AS HE FINISHED HE TURNED TO WALK AWAY AND BUMPED INTO A WATER LINE. EE HAD A 1/2 INCH LACERATION TO HIS LEFT EYEBROW.

Ee bent over to unlatch hatch on railcar and felt pain in back and hips. Pain persisted while squatting & standing. Ee originally thought injury was a kidney problem and later reported it as a back injury after ee seen his personal physician.

EMPLOYEE WAS REMOVING PIECES OF BROKEN CERAMIC BALLAST AND CUT HIS RIGHT HAND ON THE SHARP EDGE OF THE CERAMIC WHEN ADJUSTING IT TO LIFT. EMPLOYEE WAS WEARING GLOVES AT THE TIME AND EDGE CUT THRU GLOVES.

EE WAS ATTEMPTING TO RELEASE A MANUAL HANDBRAKE ON A RAILROAD CAR. THE BRAKE WAS SET TO TIGHT AND EMPLOYEE PULLED/STRAINED MUSCLE IN GROIN WHEN HE PULLED THE BRAKE LEVER.

EMPLOYEE WAS WORKING ON A PUMP MOST OF THE DAY. HE WAS LOSSENING TIGHT BOLTS AND HAD PAIN IN LEFT ELBOW FROM OVER USE.

Employee was opening a feed valve with a valve wrench. when he felt pain in his groin area.

Ee was re-installing a guard, and when tightening the bolts, wrench slipped causing ee to strike hand against corner of guard resulting in a laceration of appx 1 cm, requiring sutures.

Employee stated he was removing a double nut locking system with a wrench when nut broke free sending employee backwards.

EE STATED HE WAS GOING TO CHANGE ROOM AND WHILE OPENING THE DOOR CAUGHT HIS RIGHT HAND BETWEEN THE HANDLE ON THE DOOR AND THE ANGLE IRON STRUCTURE.

EE STATED WHILE HE WAS PULLING OLD LINERS OUT TO PUT ON A TWO WHEEL CART, FELT A STRAIN IN LEFT SHOULDER, AND ARM.

Employee and another ee were removing a guard from a motor shaft when they raised the guard over shaft the guard started to shift and ee tried to stop the shifting and caught/ pinned his right little finger.

EE stated he stepped in some mud when he tried to pull his boot out he twisted his lower back.

EE STATED HE WAS CLIMBING LADDERS ALL DAY AND EITHER PULLED OR STRAINED HIS KNEE. EE CONTINUED TO WORK - FOUND OUT EE HAD SURGERY ON 6/3/04.

EE STATED HE WAS WALKING FROM CHANGE ROOM TO CRUSHER BUILDING TO GO TO WORK WHEN HE SLIPPED ON ICE IN ROAD AND FELL.

EMPLOYEE STATED HE WAS STEPPING OVER A CONDENSATE PIPE WHEN HE FELT A POPPING SOUND IN HIS KNEE. EMPLOYEE WORKED UNTIL 3-1-04 WHEN HE WENT INTO THE HOSPITAL FOR SURGERY.

EE WAS SWINGING A SLEDGE HAMMER ON BIN TO REESTABLISH FEED TO BELT. EE LATER COMPLAINED OF A SORE NECK.

EE WAS STANDING ON LADDER, WHILE ATTEMPTING TO COVER A LEAKING PIPE-EXTENSION LADDER SLID OUT FROM UNDER HIM CAUSING EE TO FALL.

EMPLOYEE STATED HE WAS INSPECTING THE FEED BOX ON #5 DRYER WHEN ONE OF THE SPRINGS CAME OUT STRIKING EMPLOYEE ON LEFT CHEEK CHIPPING A TOOTH AND BRUISING CHEEK.

EMPLOYEE STATED THAT AFTER HAMMERING ON BINS FOR ABOUT FOUR HOURS THE NEXT DAY HIS LOWER BACK WAS HURTING. EMPLOYEE CONTINUED TO WORK UNTIL 3-24-04 WHEN HE WENT OFF FOR BACK SURGERY.

EE WAS INSTALLING METAL INSULATION COVERINGS. HE HAD ONE SECTION OF COVER THAT WAS PARTICULARY TIGHT SO HE WAS PUSHING HARD ON COVER WHEN HE FELT HIS LEFT SHOULDER MUSCLE PULL. EE CONTINUED WORKING UNTIL 3/29/2004 WHEN HE WENT OFF FOR SHOULDER SURGERY.

EE WAS SCRAPING STICKER OFF EXTINGUISHER & RAZOR BLADE SLIPPED & CUT MIDDLE LEFT FINGER.

EE WAS CARRYING GARBAGE IN OUR GEIII PLANT. HE MISSED THE BOTTOM STEP AND CAUGHT THE HAND RAIL RE-INJURING HIS BACK. WENT TO OUR COMP-ANY PHYSICIAN AND WAS GIVEN NO OVERTIME.

INJURY WAS REPORTED AS FIRST AID CASE AND EMPLOYEE CONTINUED WORKING DOING HIS REGULAR JOB. ON APRIL 6, 2004 EMPLOYEE NOTIFIED COMPANY OF SURGERY ON HIS RIGHT SHOULDER ON APRIL 27, 2004. IT WAS CONFIRMED BY OUR COMPANY DOCTOR TO BE WORK-RELATED.

EE STATED WHILE HE WAS SHOVELING ON THE 2ND FLOOR OF CRUSHER BLDG. HE FELT A POP IN HIS LEFT SHOULDER. EE NOTIFIED COMPANY ON 11-17-03.

EMPLOYEE STATED HE WAS WORKING ON GR III IMPACTOR CHANGING ANVILS WITH AN IMPACT WRENCH. THE SOCKET SLIPPED OFF OF SWIVAL AND STRUCK LEFT MIDDLE FINGER CAUSING LACERATION REQUIRING SUTURES.

EMPLOYEE WAS OPENING A SLUICE VALVE ON #7 FILTER IN OUR GRII PLANT. AFTER HE OPENED THE VALVE HE COULD NOT BRING HIS ARM DOWN. HE WENT TO AN ORTHOPEDIC SURGEON AND SURGERY WILL BE DONE ON THE SHOULDER ON 9-17-03. HE BEGAN MISSING TIME ON 9-15-03.

EE WAS USING A HAMMER TO CLEAN DRYER BOXES. HE FELT A POP AND A SLIGHT PULL IN HIS BACK, BUT DID NOT THINK ANYTHING ABOUT THE DISCOMFORT. HE HAS A HISTORY OF BACK PROBLEMS. WHEN HE WENT HOME T HE PAIN WORSENED TO THE POINT HE COULD NOT GET OUT OF BED.

EE WAS CLEANING A DUCON W/ A SLEDGEHAMMER WHEN HE STRUCK HIS RIGHT INDEX FINGER BETWEEN A PIPE HIPPLE AND THE BOTTOM OF THE HANDLE, RECEIVING A FX OF THE FINGER. THIS REPORT WAS FILLED OUT AS SOON AS WE WERE NOTIFIED OF THE ACCIDENT.

EMPLOYEE WAS WORKING ON A PUMP, BENDING ALL DAY. HE SAID WHEN HE GOT OUT OF BED THE NEXT DAY HE HAD PAIN. HE WENT TO THE HOSPITAL AND WAS GIVEN LIMITED DUTY FOR THREE TO FIVE DAYS.

EE WAS TROUBLE-SHOOTING CENTRIFUGE MOTOR CUBICLE WHEN THE PRIMARY BUS STAB PHASE LEADS SHORTED PHASE TO PHASE.

EE WAS TIGHTENING BOLTS ON THE 101 PAN FEEDER. THE SOCKET SLIPPED OFF OF THE NUT. THE EE SLIPPED STRUCK AGAINST THE PAN FEEDER. HIS SAFETY GLASSES JAMMED AGAINST HIS HEAD CAUSING A LACERATION REQUIRING SUTURES.

EMPLOYEE WAS PLACING A SECTION OF GRATING ON GR-2 BURNER DECK. HE FELT A PULL IN HIS BACK. HE WENT TO A CHIROPRACTOR WHO PLACED HIM OFF WORK.

EE WAS CLEANING #3 DRYER SCREW WHEN THE HANDLE ON THE LEVER OF THE DUCKON BROKE CAUSING THE GATE TO SLAM SHUT. THIS CAUSED STEAM TO EXIT THE DOOR WHERE EE WAS STANDING, CAUSING 2ND DEGREE BURN S ON THE NOSE & FACE.

EE WAS PULLING ON A HOSE & MOPPING. HE WENT TO OUR COMPANY DR & WAS DIAGNOSED WITH TENDINITIS OFTHE SHOULDER. ACTIVITY AS TOLERATED NO OVERTIMEEE HAS HAD SHOULDER PROBLEMS IN THE PAST. DEG 7

EE WAS STANDING ON THE STAIRS BY THE ID FAN IN THE BOILER HOUSE (C) SIDE WHEN HE SLIPPED & FELL. HE REACHED TO CATCH HIMSELF ON THE HANDRAIL BENDING HIS ARM BACK & UP. FX TO LT. UPPER ARM.

EE WAS PUTTING COVERS ON THE 409 A&B SCREW CONVEYOR. AS HE WAS DOING SO, HE FELT A PAIN IN HIS LEFT ABDOMINAL REGION. THE DR HE WENT TO STATED NO LIFTING OVER 26 LBS. OR TWISTING NEXT 3 DAYS EE DID NOT RETURN TO HIS REG JOB

EMPLOYEE STATED LEFT SHOULDER PAIN. EE STATES DOES A LOT OF REPETITIVE MOTION. EMPLOYEE DX TENDONITIS RIGHT SHOULDER. EE RETURNED TO REGULAR DUTY WITH EXCEPTION OF NO OVERTIME.

EE STATED HE WAS WORKING ON CALCINER AND HIT LEFT HAND INDEX FINGER WITH PIPE WRENCH WHILE TRYING TO HOLD PIPE.

EE STATED THAT WHILE COMING DOWN STAIRWAY AND STEPPING ONTO 3RD FLOOR LANDING, EE FELT HER RT. FOOT TWIST AND HER ANKLE ROLL.

EE WAS PULLING ON A CHAIN OPERATOR ON #5 PRODUCT BIN & FELT A STRAIN IN HIS SHOULDER. HE HAD SURGERY ON HIS SHOULDER AND STARTED MISSING TIME ON 5/28/03.

EE WAS EXITING THE CONTROL ROOM IN POWERHOUSE #1 WHEN HE FELT A PARTICLE IN HIS EYE. HE WENT TO THE FIRST AID KIT AND FLUSHED IT WITH EYE STEAM. HE THEN WENT TO HIS LUNCH BUCKET AND TOOK OUT A BOTTLE OF WITCH HAZEL, USED FOR ANOTHER PURPOSE, WHICH HE BROUGHT FROM HOME AND PUT THIS IN HIS EYE CAUSING A CHEMICAL BURN.

EE WAS CHANGING THE HOSES ON THE WAREHOUSE FORK-LIFT. HE HAD TAKEN THE HOSE CLAMP OFF AND WAS TRYING TO PRY OFF THE HOSE WITH A SCREWDRIVER WHEN IT SLIPPED, STRIKING HIM IN THE RT. THUMB, WHICH CAUSED A LACERATION REQUIRING SUTURES.
