Miner operating a front end loader loading material into haul trucks. During the process a fire sparked inside the engine compartment. Miner noticed fire, stopped loader and shut down the engine. Miner then evacuated to the ground and left the immediate area. The fire was isolated to the loader only. No other property damage occurred.

An operator was found unresponsive in the cab of a bulldozer. A lead person drove to the area in which they were operating to pick up for shift change. Upon arrival the area the lead person found the bulldozer parked but wasn't able to get a radio response from the operator. Lead person ascended the bulldozer and found the operator seated in the cab unresponsive.

Pulling 20 ft sections of drill steel pipe from ground after drilling. While stacking one 20 ft section, employee felt a strain in lower back.

While servicing HVAC outdoor unit tripped on refrigerant hoses with the top of unit off and running. Fell into unit with left arm causing a laceration to the left wrist against the blower motor fan blade.

EE completed pre-op inspection on loader. Our Investigation indicates EE fell from standing position on ground prior to ascending the loader ladder suffering injury to right hand, forearm and forehead. Environmental factors (weather, ground) and loader conditions good and non-contributory to this incident. A pre-existing medical condition likely contributed to the fall.
The employee was performing a common task of knocking out rocks from a screen deck, using a sledge hammer. Employee safety glasses began to fog so EE slid glasses down just a little so they were not touching forehead then continued striking the lodged rocks with the hammer when flying debris struck EE in the right eye.
Employee was removing rock from catwalk on a rock crusher. During this process EE lifted a large rock overhead to roll it into the bin. In this process the rock rolled back toward EE as EE attempted to deflect the rock, EE received an injury to right pinky finger.

Employee operating water truck stopped at fuel island to fuel water truck. Upon exiting the cab, the employee was closing the door and had his left hand placed in the door way. While shutting door he slammed door on his left thumb, causing a one inch laceration requiring 10 sutures to close.
EE was dismounting CAT 980 G Loader in prep. to grease machine. During dismount EE's rt. foot slipped off 2nd to last step and fell backwards to the ground. EE extended lt. arm behind him to break impact of his fall. Resulting in a fractured wrist. This will require EE to have surgery on left wrist on 5/16/12.

Employee was using a 1/2" drive air impact wrench with a swivel socket extension to loosen bolts on a wash screen to change screen cloth. Employee failed to wear provided gloves and grabbed the swivel with his right hand and twisted it while applying air to the impact gun. The swivel caused a 3 cm laceration requiring 9 stitches to close
While replacing tail pulley, hand was pinched between shaft & frame work - back of right hand required 3 stitches.

Employee was walking down a ladder and stepped off the step and twisted ankle.
Employee was washing out under station #2 when a rock approx. 7" diameter bounced off scalp screen falling approx 20' striking employee on top left side of hard hat. The force drove hard hat down on EE's head causing bump and laceration to left side of head. Upon being examined at ER the Dr. administered 5 staples to laceration on employee's scalp.

Employee was manually extending the service truck boom out to install cylinder head on 769 water truck. The boom lock did not catch. The boom extension came all the way out and fell onto shoulder pushing him to the ground. The bolts holding boom lock plate were broken causing extension of boom to completely come out.
Mechanic was inflating tire on service truck when side wall blew out forcing rock and grit to right side of employee's face and right eye. Employee suffered facial abrasions and eye irritaion.

Employee had finished a repair on a piece of mobil equipment and was putting his tools away into his field service truck. He was putting a puller rod back into it's protective PVC tube, The rod was sliding down into the tube and he put the end of thumb on the smooth rod and it pulled it into a pinch point and pinched off the tip of his right thumb.
Employee was stepping up from the floor to beam to torque bolts on vib. conv. Heavy rains made work area wet & muddy, the floor had been swept though still wet, along with beams and soles of employees shoes. Employee's fingers slipped off beam, fell approx. 3.5-4' back onto diamond plate floor. Contusion/bruise left elbow. X-rays negative; ER requires visit to Orothopod.

2 employees were attempting to turn a switch tip over in the storage yard. We pulled upward and realized it was to heavy and let it fall back. Unknown to employees there was a large rock under the switch tip. It caught and mashed finger tip
High ambient temperature day. EE, a field service technician, was repairing heavy earthmoving equipment. When EE got home that night he was very tired and sat down and fell asleep. He awoke a couple of hours later with symptoms of heat exhaustion. He went to an emergency room for treatment.
Employee cleaning top shaker screen with chain & plate while standing on shaker flywheel guard housing. Chain fell into flywheel shaker, tightened up, then slapped employee across both legs in shin area. Employee jumped down & got out of immediate area experienceing pain in legs. Gash in right shin.
Driver Error: Driver coming over 20 per cent hill and lost control of haul truck. Haul truck struck berm and rolled over onto the drivers side. No occupational injury to driver. Equipment was badly damages.

Employee was repairing rubber wash down hose, Forced king nipple into open end of hose. Holding one end of hose with one hand and king nipple with other hand. He forced king nipple into open end of rubber hose, he felt pain in his left shoulder. He reported incident, felt he would be ok after a nights rest. Next morning pain persisted he was taken to doctor.
EE WAS CHANGING BOTTOM DECK SCREEN ON STATION 8. HE WENT TO CLIMB ONTO BOTTOM DECK SCREEN WITH HIS LEFT HAND AND FELT PAIN THROUGH HIS ARM AND SHOULDER. EE COULD NOT STRAIGHTEN ARM WITHOUT FEELING PAIN.
EE STEPPED UP FROM WALKWAY TO BACK OF THE ROCK BREAKER. WHEN HE STEPPED UP, PAIN WAS INSTANT IN THE MID TO LOWER PORTION OF HIS BACK. EE THEN NOTIFIED SUPERVISOR. EE SAID HE COULD WORK THE REST OF THE SHIFT. NEXT MORNING EE'S PAIN & STIFFNESS VERY BAD. PLANT MGR. TOOK HIM TO PHYSICIAN: MEDICATION AND PHYSICAL THERAPY FOR 1 WEEK AND WAS RELEASED TO RETURN TO WORK SAME DAY.

EE WAS CARRYING A 48" SCREEN PANEL. HE STATED THAT THE SURFACED WAS FLAT AND HE STEPPED NORMALLY BUT TWISTED HIS ANKLE.
HOISTING MOTOR UP INTO POSITION USING A CHAIN-FALL COME ALONG THE RIGGING. SLIPPED AND THE 250 POUND MOTOR FELL APPROX 2 FT AND HIT THE EE ABOVE THE KNEE AND DROPPED INTO A WHEEL BALLRELL, WHI CH CAUSED THE WHEEL BARREL TO TIP AND HIT THE SAME KNEE AT THE KNEE CAP.

WHILE EXITING A MANLIFT, EE WAS "STOOPING" DOWN TO GET UNDER THE MANLIFT BASKET SAFETY BAR. WHEN HE STOOD UP, HE FELT PAIN IN HIS LOWER BACK. THE DR. DIAGNOSED THE PAIN AS A LUMBAR STRAIN.

EE WAS KNEELING UNDER A HAUL TRUCK TRYING TO INSTALL A STARTER. HE FELT HIS KNEE CAP POP OUT OF POSITION. DOCTOR PUT HIM IN A KNEE IMOBILIZER AND ON MODIFIED DUTY.
EE WAS IN THE BED OF MECHANIC'S TRUCK TO RETRIEVE AN ARTICLE. HE JUMPED FROM THE BED TO THE GROUND AND TWISTED HIS KNEE. RESULTING IN A TORN CARTILAGE WHICH WILL REQUIRE SURGERY & 2 WKS. OFF.

EE WAS HAVING TROUBLE WITH HIS A/C. HE RAISED THE LID ON THE BATTERY BOX TO CHECK HIS WIRES. AS HE MOVED A RUBBER MAT TO CHECK THE BATTIERIES A SPARK OCCURRED & IGNITED THE GAS EXPLODING THE B ATTER. HE RECEIVE 1ST6 & 2ND DEGREE BURNS TO HIS FACE WITH SOME LACERATIONS, NO DAMAGE TO HIS EYES DUE TO HIS SAFETY GLASSES DIVERTING THE ACID.
EE WAS HELPING REPLACE BELT LACES. HE WAS ATTEMPTING TO ROLL THE BELT BACKWARDS BY USING THE V-BELTS. THE BELT HAD SOME RESISTANCE TO IT. WHEN HE TRIED TO LET GO OF THE V-BELTS, THEY CAUGHT HI S THUMB BETWEEN V-BELT AND THE SHEAVE OF THE GEAR BOX, BREAKING HIS RT THUMB.

EMP WAS HELPING A MECHANIC ON D8 DOZER. THEY LIFTED THE BELLY PAN AND HE FELT PAIN. SAID HE DID NOT WANT TO GO TO DR. ON 10-18-95 CAME IN WITH SEVERE PAIN. TOOK HIM TO DR WHERE HE WAS DIAGNOSE D WITH AN UMBILICAL HERNIA.
INJURED WAS TIGHTENING A ROW OF 1" BOLTS ON A 988 LOADER. HE PUT HIMSELF IN A STRAIN AND FELT SOME PAIN IN HIS LOWER BACK. HE DID NOT REPORT THIS TO US UNTIL 5-30-95. HE WAS SEEN BY A DOCTO R ON 5-31 WITH A LOWER BACK MUSCLE STRAIN.
EE WAS HAULING ROCK TO THE PLANT. A MECHANIC PULLED IN FRONT OF HIM AS HE WAS TRYING TO STOP, HIS LEFT TESTICLE LODGED UNDER HIS LEG CAUSING IT TO BE COMPRESSED AND LATER SWELL. HE DID NOT COM PLAIN OF ANY INJURIES UNTIL 2-1-95 AT WHICH TIMEWE TOOK HIM TO THE DR.

EE WAS REPAIRING A TRANSMISSION ON A HAUL TRUCK. HE HAS PUT ONE JACK ON TOP OF ANOTHER INSTEAD OF USING CRIBBING. THE TOP JACK SLIPPED OF THE OTHER CAUSING TAG TRANSMISSION TO FALL AND CATCHIN G HIS FINGER BTWEEN THE MOUNTS.
EE AHD BEEN WORKING IN THE CAB OF THE LOADER AS HE WAS DECENDING THE LADDER HIS FOOT SLIPPED CAUSING HIS LEFT ELBOW & SHOULDER TO BE DRIVING INTO THE LADDER HE DIDNT WANT TO BE TREATED AT THAT TIME BUT HAS HAD PAIN EVER SINCE & WAITED TO HAVE IT LOOKED AT THE DOC EXAMINED HIM ON 09-14-93 DIAGNOSED IT AS A LIGAMENT STRAIN IN THE LEFT SHOULDER & ISSUED AN ANTICOFLAMENTORY DRUG WITH
INJURED WAS TRYING TO REMOVE A PIN IN A HYDRALIC LIFT CYLINDEROF A CAT. HAUL TRUCK. AS HE WAS DRIVING THE PIN OUT A PIECE OF METAL SPLIT OFF AND WENT INTO HIS LEFT FOREARM. REQUIRING REMOVAL A ND STITCHES!
EE WAS CHECKING THE PLANT FOR STARTUP. HE WENT AROUND TO START THE PUMP IN DUST CONTROL AND TURNED HIS ANKLE ON A ROCK. HE THEN WENT UP INTO THE PLANT AND STEPPED IN A HOLE CAUSING FURTHER PAI N TO THE ANKLE. DIAGNOSIS WAS A SPRAIN TO LEFT ANKLE.
EMPLOYEE WAS CLEANING THE SCREENS ON AN A.C. SCREEN: WHEN HE STEPPED OFF THE SCREEN ONTO THE CONVEYOR BELT WET WITH MUD HE SLIPPED & LOST HIS BALANCE WHICH CAUSED HIM TO HIT HIS ARM ON THE BEL T FRAME WHICH BROKE HIS ARM JUST ABOVE THE WRIST. EQUIPMENT WAS LOCKED OUT WHILE MAINTENANCE WAS BEING PERFORMED.

EE WAS CLIMBING THE LADDER UP TO HIS LOADER WHEN HE SLIPPED AND LOST HIS GRIP WITH HIS HANDS CAUSING HIS LEFT HAND TO HIT SOME PART OF THE LOADER AND CUT THE BACK OF IT FAILRY DEEPLY.
