Ee went to get product from the truck. the bench was muddy and he had mud on his shoes. he stepped on the bumper of the truck with his left foot and it slipped off the bumper. he hit his knee and fell backwards landing on his back
THE OPERATOR HAD CLEANED THE FLOOR OF THE DRAGLINE WITH MINERAL SPIRITS AND WIPED WITH RAGS. HE WAS GETTING OFF THE MACHINE TO DISPOSED OF THE RAGS WHEN HE SLIPPED. HE GRABBED THE HANDRAIL POS T WHICH HAD A SAFETY CHAIN HOOK WELDED OUT THIS LEFT HAND AT THE HEEL AND RIPPED TO THE BASE OF HIS LITTLE FINGER REQUIRING MULTIPLE STITCHES.
HE WAS WASHING UNDER THE SCALES AND HIT HIS LEFT KNEE ON SCALE.
