THE FALL WAS 20' X 30' X 0 TO 6' THICK. THE ONLY DAMAGE WAS TO THE BOLTER CABLE.

NO PERSONNEL OR EQUIPMENT WERE IN THE AREA & VE NTILATION WAS NOT DISRUPTED. PASSAGE WAS NOT IMPEDED. THE TIMNE OF THE FALL IS NOT KJNOW, IT WAS FOUND ON 3-1-99 AT APPROX. 1345.

WHE HE STARTED THE SCOOP, THERE WAS A SUDDEN MOVEMENT WHICH CAUGHT INJURED BETWEEN THE SCOOPS FRACTURING BOTH TIBIAS & DAMAGING BLOOD VESSELS IN HIS LEFT LEG.
