3 ee where installing conveyor belting over the top of the modules on the JCI screen to protect them from the elements. 2 ee put a piece of pipe through a roll of belting to use as a handle. When picking up the belting and starting to walk around a corner ee felt a strain in his left knee.

While changing batteries in a truck using a socket and butterfly air ratchet, the socket fell off of the air ratchet. When trying to put the socket back onto the rachet, he accidentally bumped the air rachet trigger mechanism, causing the socket to spin lacerating the palm of his right hand resulting in four stitches.

CLEANING THROAT OF CRUSHER ABOVE THE ROTOR WITH A 6 FT BAR. THE BAR SLIPPED AND HIT THE DISTRIBUTOR PLATE AND GOT INTO ROTOR - IT WAS STILL SPINNING. IT WAS STILL IDLING DOWN. SMASHED HIS RIGH T THUMB AND HIT HIM BEHIND LEFT EAR.
CHANGING TOOTH ON DRAGLINE BUCKET. WHILE DRIVING TOOTH OFF WITH A 6 LB HAMMER A CHIP BROKE OFF OF TOOTH AND STRUCK THE RIGHT KNEE. SLIVER OF METAL WAS IMBEDDED INTO THE SOFT TISSUE.
EE WAS USING A FENCE POST HAMMER TO INSTALL A GRUND ROD AT A PORTABLE FEEDER. WHILE DRIVING THE GROUND ROD, HE LIFTED THE HAMMER TOO HIGH AND ON THE NEXT STROKE MISSED THE ROD AND LET THE HAMM ER FALL ON HIS FOOT.

THIS CAUSED TRAILER TO CONTACT REA R OF TRUCK CAB. APPARENT CAUSE OF FAILURE WAS METAL FATIGUE OF TRAILERS FIFTH WHEEL PIN. EMPLOYEE TAKEN TO HOSPITAL OBSERVED AND RELEASED DR SAID HE CAN RETURN TO WORK TO WORK 10-21-94.
GUARD RAIL REMOVED TO USE CABLE TO TIGHTEN CRUSHER. MECH. SLIPPED AND FELL THRU OPENING. FELL TO SAND PILE ON GROUND. BRUISED RIGHT HIP AND SHOULDER. TAKEN TO HOSPITAL FOR X-RAYS. NOTHING BROKEN. STAYED OVERNIGHT FOR OBSERVATION.
