Removing small center shaft on loader with come-along. Shaft got hung up on bolt and slid to strike forehead. Cut on forehead, cleaned and placed band-aid on cut. Sent to care center to just have them evaluate. They place two stitches and returned EE to work with no restrictions.

Working on loader and employee felt a slight strain in back at the time. The next day the employee reported that his back was feeling sore. Employee was placed on modified duty starting 6-25-15.

During disassembly of transmission pump, a hammer and punch were used to remove the shaft from the housing. On the third strike a chip flew off the punch and struck the upper lip. Employee requested a band-aid and declined a trip to the doctor.
Employee was doing housekeeping in surplus equipment storage area. He was not wearing gloves while picking up old scrap wood and equipment parts. Employee noticed his wrist itched but was not aware of a bug bite until the next morning when he noticed his left wrist was swollen. We sent employee to the doctor where he received a tetanus shot and a splint to wear for two days.

While cutting steel in a band saw a small amount of steel particles built up close to the blade drive pulley. Employee lift guard and went to clear build up with finger, saw blade caught glove and pinched index finger tip of the left hand on edge of saw frame.

doing a welding repair under an excavator in an awkward position
Employee felt and heard a tearing sound from the left shoulder while working the lever back and forth of a 1/4 ton chain come along. Employee was in the act of removing a rod assembly from a hydraulic cylinder. The hydraulic cylinder was laying horizontally in the shop on the cylinder work bench.
Employee set lunch box down to go load a skid steer loader on an equipment transport trailer. As the employee set his lunch box on the ground he heard two (2) popping sounds and felt extreme pain in right shoulder.
Loader was parked on pressure wash pad, employee was climbing down ladder and slipped

EE was pulling out a piece of steel from yard storage, when he was stung by 5 or 6 yellow jackets on arm and neck. (L) arm was quite swollen.
EE had been typing at desk since start of shift. He turned to look at calendar (right side) and felt neck stiffen. Progressively got worse as day progressed. EE was released to full duty on 9/26/11 but was prescribed physical therapy, which caused incident to become reportable per Denver Office. First appt available was 9/13/11.
While EE was filling 5 gal bucket with soap and exiting an area where other containers were stored, his foot was caught on said containers and caused ee to trip. As he was falling his left forearm contacted a hot muffler on steam cleaner. This caused 2nd degree burns to effected area.

The EE was removing the dust cover over the threaded port hole on a grease barrel and cut his right finger on the metal pull tab. The laceration to the tip of his pinky finger required two stitches.
The EE was exiting a Loader #71-926. As he climbed down the ladder his hand slipped on the handrail. He twisted his left knee. Saw company doctor on 4-30-09. No restrictions that prevented him from doing his job. Returned to doctor on several occasions. Surgery done on 7-29-09. Reportable as of then.

The EE accessed the front end loader Co. #71-485 via the ladder, located on the left hand side, to check fluids. While descending from the right side of the cat 988F loader at 05:45 am, the operators visibility was limited and the EE misjudged the distance from the last rung to the ground, hyperextending his left knee.
EE was working on vehicle 22-311. While working on suspension he heard a popping type sound. He immediately pulled his left arm away from bolts being tightened. The leaf springs may have shifted. Springs were not under load.

He was working on brake assembly for a motor grader. Assembly had been removed from grader for rebuild. While on the bench, he tested the piston seals & retainers blew out & struck him on lip & under chin causing laceration.
The ee was trying to make an adjustment to the table. The handle used to move the table saddle swung around striking the ee (r) tip of index finger-(r) hand. causing the nail to partially come loose.

Jump starting equipment( haul truck). Stepping across from truck to truck, lost footing, grabbed handrail, fell against the truck and fell to the ground.
Prying on a bearing rack with a heelbar inside a gear box. The heelbar slipped which caused my right forearm to hit the gear box housing. The edge of the housing was fairly sharp.
Picking up cages and stacking onto truck trailer. Bent over to pick up another cage and felt pain in back.

Installing safety latch on wire rope hook using screw driver, and screw driver slipped and went across knuckle on left index finger.
(EE WAS OPERATING LATHE.) CHIP WOUND DOWN INTO CHIP PAN, GRABBED THE CHIPS IN PAN AND THE ROTATING CHUCK ON THE LATHE CAUGHT THE CHIPS AND PULLED THEM OUT ONTO EE'S HAND, CAUSING CUT.
