Slipped on wet steps resulting in a compression fracture to the back
Employee suffered from a 3/4 of an inch laceration on EE's left hand.

Employee was attempting reposition a ceramic shoot for reassembly of the lid covers. The employee kicked the chute causing damage to the metal clamp that secured the shoot onto the piping that ran from over head. This allowed the chute to fall striking the employee on the right leg. Employee was examined at the local hospital and placed on light duty.

Employee was operating loader on haulroad to spread gravel when he drove over a rough patch in the road making the loader bounce. When this happened, the employee felt a sharp pain in his upper back area.

Employee was prying a slab of slate to get it to fall thru breaker when it moved, kicking the pry bar causing it to pinch his finger on his left hand against the edge of the cover resulting in a cut requiring stitches.
Employee was replacing metal siding. Metal fell and cut index and middle finger on left hand.

Employee picked up drive shaft, the shaft slipped catching left ring finger under drive shaft.
EMPLOYEE WAS ON FOURTH FLOOR OF THE PREP PLANT AND COLLAPSED.
Employee was lifting a piece of metal and felt a pain in his lower back. Started missing work 3-8-2005.

Employee was lifting a wearplate. The wearplate slipped catching his middle finger of right hand.
Employee was working on discharge line in plant on cyclone pump. The next section of pipe was in place when employee hit the pipe with hammer and the come-a-long rolled around catching his left little finger.
Employee was changing top roller on feed belt and the roller turn over causing a jagged edge to cut forearm. (6 stitches)

EMP WAS CUTTING OLD HINGES OFF ACCESS DOOR. HEAD CHUTE, FIRE AND SLAG SPLATTERED BACK GOING UNDER EMPLOYEE'S GOGGLES.

THIS EMPLOYEE STATED THAT HE TWISTED HIS BACK ON 5-13-96. HE REFUSED TO GO TO THE DOCTOR. THE EE SOUGHT HELP FROM A NON-CERTIFIED PROVIDER. THE COMPANY REQUESTED THE EMPLOYEE TO SEEK SERVICE F ROM A CERTIFIED PROVIDER AND HE DID. EMPLOYEE WAS GIVEN MRI WITH RESULTS INDICATING NO EVIDENCE OF BULGING OR HERNIATION.
THIS EMPLOYEE STATED THAT HE HURT HIS BACK ON 4-25-96, HE WORKED UNTIL 7-22-96 AT WHICH TIME HE UNDERWENT AUDIOGRAPHY AT CENTRAL BAPTIST HOSPITAL. AFTER THE RELEASE TO RETURN TO WORK, HE UNDER WENT MRI EXAM AT WHICH TIME HIS DOCTOR REQUESTED HE BE EXCUSED FROM WORK.

THE EMPLOYEE WAS RUNNING A 988B LOADER AND SAID THE SHAKING AND JARRING INJURED HIS BACK.
HE WAS ATTEMPTING TO ROLL A 55 GALLON DRUM OF STONE HARDENED BY HIMSELF AND HIS HAND WAS CAUGHT BETWEEN TWO DRUMS CUTTING MIDDLE FINGER ON RIGHT HAND. FIRST AID WAS ADMINISTERED AND AFTER SHIF T ENDED, WENT TO DOCTOR. THE WOUND WAS STITCHED UP WITH SIX STITCHES.
EMPLOYEE WAS ASSISTING IN REPAIRS IN THE PREP PLANT WHEN A STEEL PIPE WHICH THEY WERE SLIDING INTO PLACE PRESSED AGAINST HIS FINGER MASHING IT AGAINST A STEEL WALL CAUSING THE FINGER TO BE FRA CTURED.
EE HAD JUST FINISHED CLEANING THE BOTTOM FLOOR OF THE PREP PLANT BY SPRAYING WATER ON THE FLOOR WHEN HE RETURNED INTO THE ROOM TO RETRIEVE SOMETHING SLIPPERY ON THE WET FLOOR AND FALLING TO TH E FLOOR.
EE WAS ATTEMPTING TO FREE COAL THAT HAD FROZEN ON A BELT LINE WHEN HE SLIPPED AND FELL ONTO THE GROUND.

EMPLOYEE WAS LIFTING A H. M. CYCLONE PUMP, WHICH APPARENTLY CAUSED A STRAIN ON HIS BACK, WHICH BECAME WORSE OVER TIME.
EE WAS ASSISTING EE IN CONNECTING A WASHER CHAIN BY DRIVING A PIN INTO THE CHAIN EE WAS LOADING THE CHAIN WHILE EE WAS HITTING THE CONNECTING PIN A SMALL CHIP FROM THE PIN EE WAS HAMMERING STR UCK EE IN THE LEFT EYE
EE WAS HELPING LOWER A CHAIN USED FOR LIFTING A PIECE OF EQUIPMENT ON THE MUD AUGER WHEN HE LOST HIS BALANCE AND BEGIN TO FALL GRABBED A HOLD OF A 4 INCH PIPE TO CATCH HIMSELF AND STRAINED HIS ARM AND SHOULDER.
INJURED WAS SHOVELING COAL OF A CHUTE SHEN HE STATED FEELING ILL AND WAS OVER COME BY HEAT EXHAUSTION. TEMPERTURES WERE 100 DEGREES THAT DAY AND VERY HUMID.
REACHED OVER HEAD TO PULL DOWN A VALVE TO CLOSE OFF THIS VALVE IS EASY TO TURN OFF OR ON BUT DOES REQUIRE REACHING OVERHEAD.,
EMPLOYEE WAS REFILLING A SMALL BIN, INSIDE THE WASHER PLANT, WITH PERCOL 368, A CHEMICAL DESIGNED TO MAKE THE MUD IN THE WATER STICK TO THE BOTTOM OF THE TANK SO THE MUD CAN BE REMOVED. AS HE WAS REFILLING THIS BIN FROM A 50 POUND BAG A SMALL AMOUNT OF DUST FROM THE BAG BLEW UP FAN THE BIN INTO HIS EYES.
