A 4-person shot crew was loading a toe shot on a 147-foot highwall when a large rock in the wall failed and fell approximately 25 feet above the floor. The two people operating the loading hose ran, but rock rolled in the same direction as the IP. As the IP ran, IP tripped over the hose, brushed the rock, and fell. IP sprained right shoulder, elbow and damaged three teeth.

Audiogram revealed an STS, the miner refused a re-test. The operator's interpretation of the standard was the audiogram is invalid without a re-test. This report is filed to terminated citation #9443434 issued on 6/18/2020 under standard 62.175(b).

While removing bottom deck screen panel support on screen #2 with the material handler, the support became wedged. EE was helping with the task using a pry bar to help free the support. When the support became free, the pry bar EE was using moved quickly. When the pry bar moved EE's left hand was pinched between the pry bar and cross over tube in the screen.

Employee was performing maintenance work on conveyor 2 to 3 transfer. As EE was descending the access point, EE's left foot got tangled in a cable causing EE to land awkwardly onto right foot. EE twisted the ankle causing a sprain. The employee returned to work with restrictions.

Employee was attempting to remove a hydraulic hose. Hose fitting was very tight when fitting broke loose the employee felt a sharp pain in his left shoulder.

Employee was carrying the a grease gun down the steps at the primary crusher. As he descended, he slipped and fell backwards, striking his elbow on one of the steps. The resulting impact lacerated Elbert's elbow. Upon investigation of the site the stairway was free of debris and did not appear to be slick.

EE was in the pep screen chute. As he was climbing down the chute, a strong gust of wind came up through the chute and blew sand under his safety glasses and into his right eye. The sand entered his right eye and he was not able to flush all of it out with water. The decision was then made to go to US Healthworks to have his eye flushed.

EE was hauling mud out of the quarry when the load on the truck shifted to the rear causing the truck to stand up on the tailgate. Some of the material fell out of the bed causing the truck to fall back onto the front tires. The jar of the truck falling back onto the front tires caused him to hit his knee on the steering column.
