Employee was checking their surroundings while reversing the loader. When they turned to look beside them, they lost feeling in their lower extremities.
While using a hammer to hit a punch, the employee missed striking left thumb. This resulted in 9 stitches and a fractured bone at the end of the thumb.

While stepping out of a pick up truck the employee twisted ankle. X-ray showed a broken 5th metatarsal bone on left ankle.
While pouring concrete, it started to rain. EE was on EE's hands and knees screening concrete and had concrete on EE's pants. The rain helped soak it into EE's pants and on EE's skin causing a rash.

Removing back of the plate from the Seco screen. Employee lost grip on the handle and the backer plate pitched EE's finger between the backer plate and the screen frame. The injury required sutures to close.

Employee was injured while the haul truck was being loaded. Their left shoulder/lower neck injured when they were jostled around in the truck cab while rocks were being loaded in the bed. Type of injury is yet to be determined, but appears to be a type of strain.

Employee was releasing a quick coupler on the pressure washer hose. Residual pressure in the hose caused it to whip when disconnected. The hose and fitting struck the employee in the face, cutting his lip.

Employee called supervisor on radio and said he needed to go to the hospital and have his neck checked. The injured employee told his supervisor that the loader operator had slammed his truck with a big rock, causing injury to his neck.
East 42" Conveyor went down due to being overloaded. EE had conveyor belt de-energized and was sitting on belt shoveling out "pant leg chute" that feeds conveyor. Employee reported, after shoveling, that his back was hurting. This was not a reportanle injury until Feb 6, 2006 when employee under went surgery and was taken off work.

WAS SETTING UP CHAIN FALL TO LIFT A 12 INCH PIPE INTO PLACE. WAS STANDING ON A CONVEYOR BELT AND LADDER AND WENT TO HOOK UP CHAINFALL. I MISSED THE CHAIN AND HAD A CHOICE EIGHTER FALL ON WHERE I WAS STANDING OR CATCH MYSELF AND DROP FALL. ILET THE CHAIN FALL ON TOP OF MY LEFT FOOT WT 40 LBS OF SOLID STEEL.
