Accident type, without injuries

An unintentional roof fall occurred on March 3, 2008. Fall occurred on #1 entry, across #3 belt way over in crosscut 3 to 4 Spad no. 1748 on #2 entry. Length of fall 110', width 30', height 6' . No injuries occurred from the fall. (3/4"" 6', 4' resin bolts)`

Accident type, without injuries

An unintentional roof fall occured on 3/3/08, fall occurred on #2 entry across #3 halfway over in crossout 3 to 4 spad #1748 #4 entry. L110""xW20'x6"". No injuries occurred from the fall. 3/4"" 6""x4"" resin bolts.

Struck against a moving object

While working a steep backfill slope doing reclaimation work, the dozer operator was unable to maintain control, resulting in rollover of dozer approx 4-5 times. Although wearing seatbelt, a broken back and swelling of C-spine resulted, causing loss of sensation and paralysis.

Security officer saw someone on mine property. As he was running to security vehicle, he felt pain in upper left leg. No fall involved.

On September 28, 2008, employee was on top of the miner checking the center gear case when the grease hose started to reel up, catching the blast shield lever. This resulted in the blast shield lowering down on employee's head causing fractures.

Fall from machine

EE mounted the coal loader on the operator side rear ladder steps. While attempting to walk around the rear of the loader to check oil on the offside, he lost his footing which caused him to fall to the ground.

On 9/16/08, Ee was cutting steel banding from a 1000 foot roll of 48"" belt. After cutting the banding, the end of the banding cut his right elbow. First aid was adminstered by cleaning and wrapping and He finished his shift. He was admitted to summerville memorial hospital on 9/17/08.

On 8/22/08 EE was pulling on the water line that was falling out of the car when the car dropped off the pad. His left foot was positioned under the car when the car dropped off the pad, crushing his left foot.

Employee was walking up toward face #2 near corner of 2 right, one cut inby spad #170. The shuttle car was in face of no. 2. The shuttle car started to take up cable as employee straddled the cable and the cable struck him on the inside of his right leg just above the ankle.

While putting car on, EE was putting miner cable in trough of Addacar. Car dropped off pad and his right foot was smashed/crushed under car.

Struck by rolling or sliding object

Employee had just completed bitting miner and was walking along the side of the miner when the rib rolled out on his foot. Rock was approx. 80"" long, 38"" wide and 8"" thick. Employee was transported to the hospital with his left foot broken in 3 places.

ON June 19, 2008, EE was putting bolt in corner of No. 1 entry and corner rock rolled out, off rib, striking EE on back of his head. Rock measured apprx 24"" X 12"" X 12"" thick.

Struck by flying object

EE cut a tree. It fell on a tree that was previously cut when it had been logged. A stick from the tree on the ground flew up and punctured a hole in his shield on his hard hat and went into his eye.

Struck against stationary object

On 6/9/08, EE was walking up No. 2 entry to fork lift chagrer and stepped on a rock in road way, twisting right knee. He was referred to a specialist on 6/16/08 and specialist took him off work on 6/17/08.

Fall onto or against objects

On 4/18/08, EE was climbing up on miner and slipped, hitting right knee on rub rail. Note: EE was injured on the 18th of April, he finished his shift and worked on the 19th. He went ot ER at SMH on 20th. They took him off work starting the 21st.

Fall to the walkway or working surface

EE was walking down launch on highwall miner, slipped on ice and fell to ground approximately 4 1/2 feet hitting left knee, then was transported to hospital.

Employee was stemming holes, twisted his back.

Employee was conducting pre-trip inspection when he strained his back lifting the hood.

Employee was checking bench when he slipped in ice/snow into a hole (not a drill hole) and injured his back.

EE was using a cutting torch to cut the wear bars off the 5500 excavator bucket. The wear bar kicked back and hit him in the jaw and chin, cutting his face.

Operator was backing up a dozer and hit a rock causing him to break his tooth.

Employee was in the process of putting a wheel cover on a 988 coal loader. While trying to line up bolts with the bolt holes on the cover, the cover slipped off and landed on his finger.

On August 15, 2008, Injury was dismounting the 992 G Loader to re-check the oil while the tire man was finishing up changing a front tire. He was walking down the steps when he twisted his ankle and felt a sharp burning pain in his ankle. He had 3 points of contact and did not fall down.

On 7/14/08 EE was in process of haging a line curtain and cut his right hand pinky finger on the roof bolt plate.

Employee was removing a rock from HWM and it fell and caught his index finger between the rock and metal on the car.

Struck by falling object

On May 7, 2008 employee was walking from section tail to head along #5 belt to shut main water coming to section off so his co-worker could install a fire valve at belt # 7. A piece of rock fell out of top hitting the belt line bouncing off striking employee in right rib cage.

Struck by flying object

Employee was using sledge hammer to drive trailroller on conveyor chain. When hammer struck chain, piece of metal flew off hitting employee in lower lip.

Struck by flying object

On March 31, EE was pushing in shot and when he turned on air conditioner, particles came out and went into his eye. He was inside cab of dozer and didn't have eye protection on.

Fall down stairs

Employee missed last step coming out of the office trailer. Turned his ankle.
