Mechanic was using a homemade tool to work on a D11 dozer. The tool broke causing a piece to fly out, striking employee in the forehead.

Coal rib sloughed off striking employee in the left leg.
Employee was crossing behind a shuttle car as it was being loaded. As he was attempting to step on the shuttle car cable he slipped, the shuttle car backed out and struck employee. This resulted in a fractured pelvis.
Employee exited his tire truck and stepped onto packed snow and ice covered ground located at the old truck line. Upon stepping on the ground, he slipped and fell landing on his hip and attempted to catch himself with his hand. This caused a sprain to the right wrist which required a splint.
Employee was struck by a foreign object while pulling conveyor belt in place.
The operator was notified on 12/2/2015 that the employee has been diagnosed with Occupational Pneumoconiosis.
The operator was notified on 12/1/2015 that the former employee (last exposure date worked 5/2/2014) has been diagnosed with Occupational Pneumoconiosis.
On Wednesday, 11/11/2015 the mine operator was notified that the employee had been diagnosed with 16% hearing loss as a result of repeated exposure to noise over his 40 year career. Medical documents support this claim. The employee has been off of work since 8/14/2015 for an unrelated medical issue.
