Accident type, without injuries

An unintentional roof fall occurred on 12-2-10. The fall occurred on old 48 belt, #18 crosscut, #4 entry (spade #2324) to #2 entry (spade #2318. 2 crosscuts our.) No injuries occurred from the fall. Length 140' X width 20"" X thickness average average. Support 6"" fully grouted bolts.

Accident type, without injuries

An unintentional roof fall occurred on 10-16-10. The fall occurred on #8 belt, #8 entry, left return at pad #3180. No injuries occurred from the fall (length 40' X width 19' X thickness 4'-6'). Support 4' fully grouted bolts.

An unintentional roof fall occurred on 6/7/10. The fall occurred on #3 belt, #6 entry - intake between spad #1668 and 1679. No injuries occurred from the fall (length 60', width 20', thickness 2 1/2') Support 6' fully grouted bolts w/timbers and jacks just inby.

An unintentional roof fall occurred on 5/11/10. The fall occurred on #3 belt mains, #5 entry, Bk #10, spad #1277. No injuries occurred from the fall (length 30'-60', width 10', Thickness 6' plus - above anchorage) (support 6' 3/4"" Torque tension, 10' cable volts, and cribs).

An unintentional roof fall occurred on 3/3/10. The fall occurred on old #2 unit at spad #2241. No injuries occurred from the fall (length 60' to 80', width 20', and thickness - above anchorage) (support 6'3/4"" resin bolt).

Caught in, under or between (Not Elsewhere Classified)

EE was setting up tooling to press out the king pin on the Rock Truck and while hooking up the chocker cable to re-position the 100 ton ram sitting on the spindle the ram fell over and the cable hooked on his left thumb. First aid was adm. and EE was transported to hospital for treatment. EE was referred to a orthopedist by the ER Dr.

Struck by flying object

EE was in the process of removing a sprocket with another mechanic. EE stepped back to observe the sprocket while the other EE struck it with a 8 lb sledgehammer. A piece of metal came off and struck EE in his right forearm. First aid was given and EE was transported to ER. Dr. referred EE to a surgeon.

Struck against stationary object

On April 17, 2010 EE was rock dusting the face of No. 1 Entry by hand and struck the backside of his left knee against the battery end of the scoop. EE felt pain, however completed his shift and went to Summersville Memorial to have it checked. X-Rays revealed a sliver of metal approx 1.25"" long and it was removed on 4/20/10. Stiches were required on incisions made to remove metal.

Struck against a moving object

On March 10, 2010 EE was rock dusting w/scoop and duster. To get out of the dust, he backed-up the scoop and ran over a rock resulting in pain in his lower back. He completed his shift and went to have it checked. He was released and returned to work on his next scheduled shift. He returned to Dr on 4/19/10 due to continued pain & was placed off work. An MRI was performed.

Over-exertion (Not Elsewhere Classified)

On 3/8/10, EE was in process of removing the Miller Welder from #7 belt head. EE set the welder on some hanging cables. The welder started to fall and EE grabbed the welder w/his right hand to prevent it from hitting ground. EE heard his shoulder pop and felt pain. EE was referred to Dr where he was placed off work on 8/17/10, 1st day of work missed.

Struck by falling object

Removing tailgate when a bolt broke in tailgate causing tailgate to fall unexpectedly and knocking him from elevated position on ladder and falling on him.

Caught in, under or between a moving and a stationary object

EE was removing brake pack from Komatsu loader using a 1"" drive impact wrench with a made up socket keeper (made from welding rod) while operating wrench the keeper caught his glove and cut his left hand index finger.

On 9/21/10 ee was in the process of installing a hydraulic bake steering pump in the CAT 992D loader and while winching into position the pump lodged and when it came free it struck and mashed ee right hand against the transmission mount. First aid was administered and ee transported to Summersville Regional Hsp for treatment. EE was treated and released to RTW.

Struck against stationary object

EE was walking to the launch and while going around the pan loader he rested his arm on the back upright. When he pulled away a sharp burr of metal cut his right forearm. Seven stitches were used to close the laceration.
