Fall onto or against objects

While descending, Employee fell from the lower rung of the access ladder on a 16H motor grader and landed on the moboard resulting in fractured ribs and a minor laceration to employee's liver and spleen.

Caught in, under or between a moving and a stationary object

Employee suffered a laceration to the middle finger of the left hand when their finger was caught between the dragline bucket tooth and the truck bed they were transferring the tooth to. Stitches were required to repair the wound.
