EMPLOYEE WAS STRUCK BY A PIECE OF DRILL STEEL AS HE WAS ROOF BOLTING. HE SUSTAINED A FRACTURE TO HIS LEFT WRIST. EMPLOYEE BEGAN MISSING WORK ON 05/05/05.

EMPLOYEE WAS DRILLING HOLE IN ROOF OF MINE, DRILL HUNG UP, HE STUCK HIS HAND UP TO UNHANG IT, SLIPPED, AND WITH THE OTHER HAND HE HIT THE CONTROL LEVER ON MACHINE AND THE HEAD OF THE MACHINE MASHED HIS HAND.

Inhalation of radiations, caustics, toxic and noxious substances

An ex-employee filed a claim for black lung benefits on 11-7-2005 as per a notice from aig workman's comp. The injury date was his last day of employment with this company.
