Accident type, without injuries

Found an unintentional roof fall in #5 entry of #16 return at break #52. Fall is approx. 12' long x 20 wide x 8' high.

Had an unintentional roof fall at break #22 on the #21 belt. Fall is 20' x 20' long x 10' high.

Mine had an unintentional roof fall on th e#9 belt line at break #41 & # 42. Fall was 30'L X 18'W X 5' H.

Pulling rock out of bridge conveyor and cut right 5th digit finger. Started missing work 10/20/2010.

Employee was walking down the #3 entry, slipped on a loose rock and twisted his rt. knee. Started missing work 8-6-2010.

EE let his drill canopy down onto him too quickly and it knocked him down. Has contusions on his leg and neck. EE started missing work 4/29/10.

EE caught his foot between 2 waterlines and fell backwards. He fractured his right foot and ankle.

Struck by powered moving object

He stepped out of rail motor and the supply car hit him on the ankle. Fracture ankle. Started missing work 1/19/2010.

Stacking concrete block in a brattice and caught thumb between the top and a block. Fractured end of right thumb.

Struck by falling object

Drill steel fell from hole and struck hand. Fractured bone in hand.

Drill steel hung in hole and then fell and struck his left hand. Diagnosed with a fracture in his left hand.

Struck against stationary object

EE was pulling up cable slack and cut his left arm on a piece of rock, 8 sutures were required.

Drill steel fell out of hole and hit his hand and cut his left thumb. Sutures were required.

Caught in, under or between running or meshing objects

He was drilling a hole in the mine floor with hand drill and caught his finger on the drill steel keeper pin. His right ring finger was cut. 9 sutures required.

Fall onto or against objects

He was going up ladder, his foot slipped and he fell forward and hit his forehead. His safety glasses cut his forehead at the eyebrow. Sutures were required - no lost time.

Inhalation of radiations, caustics, toxic and noxious substances

EE started missing work due to self-sickness. We rec. notification on 11/16/10 that he has filed for Occupational Disease - Coal Work Pneumoconioses
