Accident type, without injuries

Found a roof fall during weekly exam-fall is at break #84, #1 entry of left fork mains old works fall is 20' wide x 20' long x unknown height. This is a continuation of another roof fall.

Installing roof bolts and shoulder started hurting. Started missing work 12/18/2007.

Struck by falling object

Employee moved bolt machine up, reached to get bolt, and rock struck him on the back.

Fall from machine

Employee was climbing up on rail runner to work on it and fell off, landing on his lower back.

Employee's arm was slammed in mantrip door.

Taking lid off scoop. The lid started falling and employee grabbed lid, resulted in pulling a muscle in his left shoulder.

Bolter wrench dropped out of hole and struck him on the left thumb. Sutures were required.

Employee was operating bolt machine, he was swinging head and canopy caught the top, causing rock to fall, hitting on left ring finger.

Employee was shoveling belt line and shovel got caught on the belt, pulling his arm into the belt and burning his right wrist.

Employee was drilling hole when top started working loose. He tried to get out of the way by grabbing tray on pinner to pull himself out when a rock fell on hand.

A piece of rock struck employee on back and shoulder. Diagnosed with a fractured clavicle and 2 vertebrae fractures. Started missing work 2-2-07.

Bolter head hung on roof bolt, when he lowered boom, it fell on foot.

A piece of draw rock fell from between the roof bolts and struck employee on head and neck. Diagnosed with only a contusion. Started missing work 1/12/2007.

EE was pulling a curtain from out of some muck and back started hurting. Started missing work 1/31/2007.

Employee was getting a rock off of the bridge conveyor. The rock fell out of the conveyor and struck his left leg. Started missing 1/8/2007.

Tramming bolter, he caught his right ankle/foot between rib and the deck of bolter. He is working restricted duty.

Employee was walking in low top and his back started hurting. He is working at a modified duty.

Unloading overcast panels from forklift and 1 panel hit his finger. Sutures required, no lost time.

Taking canopy off scoop, chain came loose and he cut his left middle finger. Sutures required - no lost time.

Employee was operating bolt machine, pushing bolt into hole and the bolt hit crack and bent sharply and the plate struck him on left side of his face, injuring left cheek.
