Inundation of gas after miner cut into an active gas well that was not supposed to be there. All State & Federal agencies were notified immediately.

An electric meter blew up while ee was troubleshooting a tram breaker and burned his hands. Started missing work 9/6/2006.

EE strained back while pulling on belt (converyor). Started missing work on 9/1/06. He did not report injury until 9/14/06.

EE was shoveling head drive and rock fell from top, striking him in lower back.

Tool box lid fell while ee was reaching into box and broke ee's right forearm. He started missing work 7-25-2006.

EE slipped off of a rock he had stepped on and fractured his right ankle. Worked light duty on 3/21, 3/22, 3/23 and 3/24 and started to miss work on 3/27/2006.

Turned water valve on and valve top blew off and hit EE under his right eye, causing a contusion and got grit in his eye. Antibiotices were perscribed for the eye.

Walking through mandoor - knocked hat off and hit head. Required 2 sutures.

EE was going thru mandoor & a rock hit his left ring finger. His finger was broken & cut.

Employee was removing rock from the continuous miner and cut his right wrist and hand. Required sutures - no lost time.

Employee was using acetylene torches and a piece of hot metal got into his right eye.

Scattering bolts and rock fell, hitting employee in the right shoulder. *Employee has not called back or contacted anyone on this injury. They have terminated him due to lack of cooperation & poor work.

Employee walked into a parked trailer and cut his head, requiring three sutures. It happened in the parking lot after his work shift.
