Inundation of gas after miner cut into an active gas well that was not supposed to be there. All State & Federal agencies were notified immediately.

Had an unintentional fall at #6, setting seals in front of the #3 seal

Had an unintentional roof fall between break #2 & #3 on the #18 belt line. Fall was 10' high x 20' wide x 45' long.

Unintentional roof fall on #12 beltline between breaks #94 and #95 Fall was 20'widex20'longx10' high.

An untintentional fall on the track entry at #131 break parallel with #20 belt fall was 25'x30'x10' high.

Had and unintentional roof fall at break #131, parallel with #20 belt on track entry. Fall was 25' wide X 30' long X 10' thick.

Had an unintentional roof fall approx 9 breaks from the main outside track portals. Fall is 100' long x 10' high x 20' wide.

Had unintentional roof fall in the #3 entry of the 011 / 012 MMU supersection. Fall was 60' L x 8' high x 20' wide.

Employee was struck by a rib roll and suffered a fractured ankle. Started missing work 11-27-06.

Kneeling beside bridge carrier & got on belt to clear bridge & belt ran him down to lolo dolly.

Walking down belts & tripped over roller, hit forehead on structure.

Employee was hooking up mantrip to cars. The mantrip operator trammed forward and caught employee's middle finger between the couplings.

Haning Hi-line and stained lower back. Started missing work on 9/25/06.

An electric meter blew up while ee was troubleshooting a tram breaker and burned his hands. Started missing work 9/6/2006.

EE strained back while pulling on belt (converyor). Started missing work on 9/1/06. He did not report injury until 9/14/06.

EE was shoveling head drive and rock fell from top, striking him in lower back.

Tool box lid fell while ee was reaching into box and broke ee's right forearm. He started missing work 7-25-2006.

EE stood up from kneeling position and hurt lower back. Started missing work 5/25/06.

Replacing bottom rollers, slipped in mud and twisted knees (both). Started missing work on 5/11/06.

Employee was laying beside track and mantrip hit him.

ee was removing rock from lolo and his finger was struck cutting finger.

Employee stepped out to throw switch and fell, hitting a rock and cutting his finger.

EE was checking cylinder when he slipped on oil on the floor.

EMPLOYEE OPERATES A ROOF BOLTER & HIS KNEE BEGAN TO GET SORE.

Employee was carryng a can of hydraulic oil and strained back. He started missing work on 4/13/2006.

HOOKING UP 20 TON DIESEL TO SUPPLY CAR & GOT THUMB CAUGHT BETWEEN COUPLING & TONGUE.

EMPLOYEE WAS PUSHING ROLLER IN PLACE, THE ROLLER FELL STRIKING HIM ON THE WRIST.

EMPLOYEE WAS BOLTING TOP AND WAS REACHING TO GET ROOF BOLTS AND TWISTED HIS KNEE.

Steel hung in hole, fell out striking his right hand.

Employee was hanging miner cable and strained lower back.

EE was shoveling belt line and pulled muscle in lower back

Making a belt splice, he reached to move knocker and finger was hit by hammer.

EE was putting a pin in a rail motor coupling and caught his right 5th finger. He received stitches.

EE slipped off of a rock he had stepped on and fractured his right ankle. Worked light duty on 3/21, 3/22, 3/23 and 3/24 and started to miss work on 3/27/2006.

Turned water valve on and valve top blew off and hit EE under his right eye, causing a contusion and got grit in his eye. Antibiotices were perscribed for the eye.

Walking through mandoor - knocked hat off and hit head. Required 2 sutures.

Cutting with a utility knife & knife slipped and cut leg. Sutures required.

EE was going thru mandoor & a rock hit his left ring finger. His finger was broken & cut.

Taking a cover off of the miner & caught his left 5th digit finger. Required Sutures-no lost time.

Making a belt splice & Cut his rt hand with knife. Required sutures. No lost time.

He raised his head and struck a beam accross the track and cut his forehead. He received sutures.

Employee was cutting belt with a utility knife & cut his right leg. Had 6 sutures, no lost time.

Belt stand hit him in the head; required sutures.

Drilling a hole in the top and a small piece of rock hit his left 5th finger. Fractured finger and sutures.

EE was knocking a pin out of an ATRS arm in the shop and hammer slipped off pin and hit his hand. Received 4 sutures.

Employee was removing rock from the continuous miner and cut his right wrist and hand. Required sutures - no lost time.

Employee was using acetylene torches and a piece of hot metal got into his right eye.

Employee was using a slate bar and the bar slipped and hit him on the left side of his chin. Required stitches.

Scattering bolts and rock fell, hitting employee in the right shoulder. *Employee has not called back or contacted anyone on this injury. They have terminated him due to lack of cooperation & poor work.

Employee walked into a parked trailer and cut his head, requiring three sutures. It happened in the parking lot after his work shift.

EE WAS CUTTING A CABLE. KNIFE SLIPPED AND CUT HIS LEFT HAND. 4 SUTURES REQUIRED.
