Caught in, under or between a moving and a stationary object

Employee was standing on a temporary platform just below a jaw crusher attempting to remove two toggle seat wedges from the pitman. Shortly after removing the two bolts holding the wedges in place, the pitman unexpectedly moved fatally crushing the employee.

Over-exertion in lifting objects

Employee strained tendon in his left leg as he was working on changing a conveyor belt.

Worker was lifting steel door into the syntron of the pit run tunnel when he strained his back. Worker is receiving therapy and on prescription medication.

Fall onto or against objects

Employee was attempting to loosen a drain plug underneath a front end loader. As he was pulling on the wrench, it suddenly loosened, and his momentum caused him to hit the back of his head on a tire rim. Employee was knocked unconscious and taken to the hospital for observation. Tests were negative for any head injury.

Over-exertion in wielding or throwing objects

Employee was swinging a 12 pound hammer to remove the main shaft from a water pump when he recieved a partial tear of his right biceps tendon.

While using a hole punch to punch holes in a piece of rubber, employee hit finger with hammer, requiring stiches.

Over-exertion (Not Elsewhere Classified)

During a trip to Oregon for work-related training, worker stepped awkwardly on the side of a curb. Doctor diagnosed it as a foot sprain and prescribed pain medication.
