Employee was welding on the jaw crusher when a piece of metal got into his left eye. Employee was wearing safety glasses and face shield. Continuing irritation required doctors visit to remove object and treatment with an antibiotic prescription.

Contact with hot objects or substances

Employee was cutting a 1/4"" plate with a torch when a piece of slag entered his right nostril. Employee notified supervisor 9 days after the incident as his nostril remained swollen. Medical treatment was sought and an anitibiotic prescription was given.
