Accident type, without injuries

EE was operating a 2004 Volvo 220 front end loader clearing snow off the haul road when he heard a loud pop. EE noted smoke & flames were coming from under the engine cowling. EE dropped bucket, set brake, turned machine off & exited the vehicle. After briefly trying to extinguish flames with hand held extinguisher, the fire dept was called. They extinguished fire. No injuries.

Rubbed or abraded

Employee had sand in eye resulting in scratched cornea. Employee unsure when sand got in eye, it may have been during clean up around the wash plant or having rubbed his eye with a dirty glove.

Struck by... (Not Elsewhere Classified)

Employee was grinding in a manlift. Protective eye wear slipped down and he got a small splinter in the eye.

Fall to the walkway or working surface

Employee was getting ready to dismount truck and slipped on the catwalk and cut his leg requiring stitches.
