Fall from ladders

Miner finished adjusting the tension on the feeder belts from a ladder. He was descending the ladder with his back to the ladder when a button on his coveralls hung up on the motor mount. He stepped up a rung to free the button and his foot slipped and he fell approximately 3 ft. to the ground below.

Struck against a moving object

Haul truck driver suffered a back and neck strain during operation of a haul truck and at the time of being loaded with rockery rock from a front shovel. After loading the truck with 2 large rocks, the shovel operator started to swing and a 3rd unexpected rock rolled out of the bucket and struck the bed of the haul truck jolting the driver.

Fall to the walkway or working surface

While walking to the shop to attend the morning tool box talk a miner suddenly collapsed due to an unknown personal medical condition and hit his head on the concrete floor. The miner sustained a laceration which required 6 stitches to close.
