Accident type, without injuries

A roof fall occurred in #7 head drive, 25 feet inby spad 1723. The fall was 4.5 feet thick, 18 feet wide and 30 feet long.

A roof fall occurred at spad 1169. The fall is approximately 50 ft. long, 19 ft. wide and 5 ft. thick.

Making return air course and found a roof fall which had occurred. It was approx 30' long X 19' wide and 7' thick. I t was just inby spad 417 at the mouth of that entry. This was approx 5400' outby the face.

Miner was hit in head by fly rock from blast causing his death.

Drilling hole when finger got caught between drill pot & pinner steel amputating tip of left ring finger, fracturing middle finger & lacerating indes finger.

Over-exertion in lifting objects

EE stated that he felt a sharp pain in his groin when lifting a sample and twisting to load into a truck. He did not report the inury until 1/8/08. The company and the workers compensation carrier investigated and determined that the injury was compensible on 2/27/08.

Fall to the walkway or working surface

Building road, employee got off of dozer to determine exact path of gas line when he tripped and fell fracturing hand.

Struck by falling object

Employee was carrying 25 lb rock when he slipped and rock fell on hand injuring his right middle finger.

Over-exertion in lifting objects

Employee was lifting pump out of sump hole to reposition, pump slipped off of chain hook causing him to pitch forward, he used his left arm to catch himself straining his left arm, his neck and his back. He is currently off work effective 8/27/07.

Struck against stationary object

Operating shuttle car when he drove into a hole causing his head to hit canopy injuring his head and neck.

Employee lost control of the rock truck he was operating and went off the road, ran over a berm where the truck dropped 2' to 3' down into pit area.

Struck by falling object

Operating roof bolter putting up middle bolt when a piece of rock fell striking him on left leg. Continued working until physician took him off work on 8/3/07.

Employee was climbing up 10 ft berm. When he grabbed limb for support it gave way causing him to lose balance and fall injuring his back. He began missing work due to this injury on 7/14/07.

Dozer topped short incline; when front of dozer came down on other side it jarred operator.

Operating a utility truck when another vehicle rear-ended his vehicle. Miner was taken off work effective 6/29/07.

Employee was removing rock from tire on a 789 when a rock dislodged striking him in the right chest & forearm area.

Employee was helping mechanic to loosen bolt, when he slipped and twisted his left knee.

Operating scoop, hit head on top then hit right elbow on scoop also injuring shoulder. Employee started missing work due to this injury on 7/16/07.

When climbing down from D11R dozer cab, employee stepped from track to push arm when his shirt caught on engine cover door laying on track and when he stepped to the ground, door fell on the back of his leg.

Fall from machine

Employee finished fueling excavator and was climbing off when he slipped and fell off machine dislocating his left ankle. Taken off work 4/15/07.

Over-exertion in lifting objects

Employee lifted box of glue, turned and strained his back. MD took him off work effective 4/9/07.

Fall from machine

Dozer was being serviced with blade safety stands under push arms. Operator was climbing back up on dozer when he slipped & fell fracturing left wrist.

Employee stated that he was loading scoop when he slipped in bucket injuring his right ankle. Employee went to Dr. two days later and was taken off work beginning 2/27/07.

Miner was lifting starter in place for 773D Cat Hauler when he felt pain in groin area. Injury required surgery for hernia and he did not miss work until 3/16/07.

Employee was walking from mantrip to miner in #4 heading, hit head against top and fell injuring neck and back.

Bent down to move miner cable and strained his right hip while lifting cable. Placed on restricted duty on 10/22/07.

Lifting rock off of continuous miner when he felt burning in his groin area. He was placed on restricted duty on 10/15/07.

He was picking up jacks and twisted his ankle.

Lifting jack with bar, when pin came out and jack fell hitting bar, causing the bar to hit EE's head which resulted in a laceration. EE had stitches, no lost time.

Piece of rock fell from roof stricking ee's middle finger on right hand resulting in a laceration that required stiches. no lost time.

Struck against stationary object

Employee was walking out of portal and stood up too soon striking head on beam, cutting above right eye on brow and his nose requiring stitches.
