Dozer operator stated that saw flames coming out of the right side engine compartment. EE got out and began using fire extinguisher but was not able to suppress fire. An excavator was then used to cover the fire and put it out. Fire was limited to Dozer only.

climbing down a ladder after working on the frame of the machine under the rear glass when ladder footing slipped out causing him to fall onto the counterweight and then to the ground - employee went for treatment nothing was found to be wrong returned to work with no restrictions - returning to the doctor and was taken off work on 3/1/2016
While reattaching the belly pan on a D-11 Dozer employee was lining the bolts holes up and strained a muscle in his stomach.

EE was changing oil in a dozer, when he slipped on the track. He tried to jump from the track and his left hip hit the open door on the tool box on his grease truck, causing a puncture wound that required stitches.
Employee was operating a loader when he struck a rock in the shot causinng his left knee to strike the safety guard of the steering control. Employee was not taken off work until 1/30/09 when he had surgery.
Truck was coming down bank and driver could not get it back into gear, so he jumped out of truck, causing multiple contusions and abrasions.
Drilling holes with a highwall drill to prepare a shot when one of the holes started having smoke and heat coming out of it. Could not extinguish in 30 minutes.
Building loader pad, rock pushed from above hit his dozer jarring operator causing back strain.

Building road, employee got off of dozer to determine exact path of gas line when he tripped and fell fracturing hand.
Employee lost control of the rock truck he was operating and went off the road, ran over a berm where the truck dropped 2' to 3' down into pit area.
Employee was removing rock from tire on a 789 when a rock dislodged striking him in the right chest & forearm area.
When climbing down from D11R dozer cab, employee stepped from track to push arm when his shirt caught on engine cover door laying on track and when he stepped to the ground, door fell on the back of his leg.
Employee finished fueling excavator and was climbing off when he slipped and fell off machine dislocating his left ankle. Taken off work 4/15/07.

Miner was operating coal loader when the bucket struck a rock, jolting him. Operator stated that he felt something in his stomach ""pop"".
Employee was driving personal vehicle and leaving job site when he stated that he misjudged the roadway and went into a ditch on left side of road.
EE was helping install a cutting edge onto a bulldozer blade, when the lifting eye that was being used to lift up the edge broke. The cutting edge landed on his foot.
Driving a pin into a bull dozer nose cone and a piece of metal flew into his eye.
Operating a front end loader carrying a large rock in the bucket. This caused the loader to bounce allowing the operator to leave his seat and be bounced around in the cab of the loader.

Employee was stepping across a piece of ice and his right foot flew out from under him causing him to fall. His left leg was bent under him, spraining his left knee.
HE WAS STANDING ON THE CAT WALK OF A BULLDOZER, WHEN THE BLADE WAS LET DOWN IT JARRED HIM AND HE FELL DOWN ONTO THE DOZER TRACKS.
He was operating a haul truck, when he ran up on the berm and turned the truck over on its side.
EE was making a repair on the highwall miner when a blast was detonated. A piece of flyrock or a piece off of the equipment struck him on the right lower leg.

EMPLOYEE INFORMED HIS SUPERVISOR AT QUIT TIME THAT HE HAD PAIN IN HIS LOWER BACK & LEFT LEG. THE PAIN STARTED SOMETIME AT THE BEGINNING OF THE SHIFT & AS THE NIGHT WENT ON IT GOT WORSE. HE STARTED MISSING WORK BECAUSE OF THIS ON 9/2/04 & IS STILL OFF WORK.
EMPLOYEE WAS OPERATING A ROCK TRUCK WHEN A ROCK ROLLED OUT OF THE ENDLOADER BUCKET AND STRUCK THE SIDE OF THE ROCK TRUCK BED. THIS BOUNCED HIM AROUND IN THE CAB OF THE ROCK TRUCK.
