Struck by flying object

Employee was pulling hose off dust collector to clean it, pulled off a little quickly and got some dust in right eye. Employee tried flushing it out for a long while, however went to eye clinic to get the remainder out. Received prescription for eye treatment. No lost time.

Caught in, under or between a moving and a stationary object

Employee was trying to set a 75# rock in between granite slab and the quarry wall. The rock slipped and right ring finger tip got caught between the rock and granite slab. Contusion to end of right ring finger. X-ray showed displaced fracture, pressure needed to be relieved from fingertip. Employee returned to work, no lost time.

Struck against a moving object

Employee was ascending driveway with loaded grout truck, it was noticed & communicated that load seemed heavy to the rear. Employee stopped, backed down to reposition load, applying brakes. The truck raised slightly then settled. Second time truck raised quite high dumping the load, the front end slammed to the ground. Jammed EE's back, cut elbow & hurt ribs. Lost time injury.

Fall from piled material

Employee was wedging a block while standing on block already wedged. When the line split, it caused chain reaction of rough backs to fall, causing employee to go over the edge of the block. EE fell 3 to 4 feet, causing dislocated right shoulder & inferior glenoid fracture, also abrasions to both forearms. Shoulder was re-set, employee's arm is in sling. As of now, not lost time.

Struck by falling object

EE was moving the control box while EE was setting up saws. EE was walking backwards, lost EE's footing and fell backwards. When EE fell the control box EE had been carrying landed on EE's hand, breaking EE's thumb. EE went to clinic, was released back to work. Went to follow up appointment Tuesday, May 28th. Has lifting restrictions of 1 lb., continues to work, no lost time.

Employee was chipping ice on saw rail using chisel, turned to step out of the rail and slipped/tripped on the rail, fell forward while holding the chisel bar. The bar hit the saw wire and sprung back at employee as employee fell forward, bar striking mouth/upper lip. Chipped/loose teeth, upper lip laceration received sutures. No lost time.

Employee was using utility knife, putting notches in the end of a plastic pipe to insert a string into a saw cut hole for wire sawing. Utility knife slipped and cut his left index finger from last knuckle to tip of finger. Received six stitches. Released to work, no restrictions. No lost time.

Employee was operating loader, moving slabs in the quarry. Employee alleges the bouncing in the loader gave him lower back pain, right side. Verbally reported injury 6-20-13, filed First Report 6-26-13 and saw a chiropractor on 6-27-13. No known restrictions, no lost time.

Over-exertion in lifting objects

Employee and co-worker were loading 55 gallon barrel into back of pickup. Employee was lifting with left arm, pushing with right arm, felt left shoulder pop, then felt the pain. He continued working. By Monday, 6-21-10, bicep/shoulder area was black and blue so he saw a doctor, was released to light duty. Left shoulder strain. No lost time.

Struck by flying object

Employee was trying to thread two pieces of steel together, when they would not go together, he used his percussion to get them to thread and a piece of steel broke off and got wedged under the skin of his chin. Metal was removed from wound and wound sutured closed.

Employee was running 475 Loader, using bucket to clean up grout. He hit a ledge with corner of bucket and it jarred him. Complaints are of sore neck, left arm, hand goes numb, sore shoulder left side. Employee has been treating with chiropractor, although employer unaware until report received 9-26-05. No lost time, no restrictions employer is aware of at this time.

EE put leg up on saw rail, was stripping a wire during a wire change, knife slipped and cut left leg on inside of knee cap. EE received sutures and returned to work, no lost time.

EE FELT MINOR PAIN IN LOWER BACK WHILE RUNNING LOADER ON 2-21-03. HE SAW A CHIROPRACTOR A FEW TIMES THAT FOLLOWING WEEK, THEN REPORTED INJURY TO COLD SPRING GRANITE ON 3-5-03. HE IS NOW GOING TO SEE A DIFFERENT DR.

EE REACHED DOWN THROUGH JAW GUIDE ON SLOT DRILL TO PULL ON AIR HOSE AND GRABBED THE WRONG LEVER, WHICH CLOSED JAWS ON HIS LEFT ARM, CRUSHING HIS ARM AND BREAKING THE BONE.

EE WAS LOWERING DRILL STEEL INTO HOLE. IT STARTED TO GO FAST SO HE GRABBED IT. A SUDDEN JERK PULLED HIS RIGHT SHOULDER OUT OF PLACE CAUSING PAIN. EE WAS UNABLE TO WORK ONE DAY BECAUSE OF THIS.
