Fall to the walkway or working surface

A contractor employee tripped while engaged in rail road track repair. The employee was working at ground level when he fell. He was diagnosed with a hip fracture.

While dismounting a piece of equipment an employee twisted his ankle. He was released to restricted duty.

A mechanic grabbed a structural support beam while climbing into the center section of the machine. In doing so, he dislocated his right shoulder. He was not released to return to work making this a lost time incident.

Fall onto or against objects

While dismounting a dozer an employee slipped and hit his safety glasses on a handrail. The safety glasses caused two lacerations on his face which required stitches, making this incident reportable.

The employee reported to his supervisor that he was feeling ill. He was transported off-site for evaluation. Later that day he was diagnosed as having heart attack.
