ELEVATOR OUTAGE OCCURRED AT EAST FINLEY PORTAL WHICH LASTED MORE THAN 30 MINUTES. THERE WAS NO ENTRAPMENT OF PERSONS. A HORIZONTAL BAR WHICH OPERATED THE SAFETY DOGS BECAME BENT AND WAS REPLACED.

An ignition occurred on the 2-east off of 3-north mains section at the face of #4 entry. the ignition lasted approx. 15 seconds and contained to the face area. the ignition was extinguished with wash down hose. there were no injurieds and there was no. damage to equipment.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE 5H SECTION @ 7 WALL IN THE #1 ENTRY (BELT). THE FALL MEASURED 12' HIGH, 16' WIDE, 20' LONG.

A ROOF FALL ABOVE THE ANCHORAGE OCCURRED AT #13 1/2 WALL CROSSCUT, #1 AND #2 RETURN ENTRIES 3 NORTH. THERE WERE NO INJURIES OR DAMAGE TO EQUIPMENT. THE FALL WAS APPROXIMATELY 90' LONG BY 18' WIDE AND 15' ABOVE THE ROOF LINE.

A FRICTIONAL IGNITION OCCURRED AT THE FACE OF #11 ENTRY INBY #120 WALL, 102' FROM LAST OPEN CROSSCUT, ON THE 3 NORTH RIGHT SECTION. THE IGNITION LASTED APPROX. 5 SECONDS AND WAS EXTINGUISHED WITH A WASH DOWN HOSE AND WATER SPRAYS ON THE MINER. THERE WERE NO INJURIES OR DAMAGE TO EQUIPMENT.

THE EAST FINLEY ELEVATOR WAS DOWN FOR MORE THAN 30 MINUTES WITH PEOPLE ON THE ELEVATOR. THERE WAS NO DAMAGE TO THE ELEVATOR. THE ELEVATOR OUTAGE WAS CAUSED BY THE LIMIT SWITCH FOR THE GOVERNOR ROPE - HAD TRIPPED BECAUSE THE NEW ROPE HAD STRETCHED.

AN IGNITION OCCURRED AT THE FACE OF #11 ENTRY INBY 120 WALL ON THE 3-NORTH RIGHT SECTION. THE IGNITION WAS EXTINGUISHED BY WATER HOSES FROM BOTH SIDES OF THE MINER. THERE WERE NO INJURIES OR DAMAGE TO EQUIPMENT.

THE EAST FINLEY ELEVATOR WAS DOWN FOR MORE THAT 30 MINUTES WITH PEOPLE ON THE ELEVATOR. THERE WAS NO DAMAGE TO THE ELEVATOR. THE ELEVATOR OUTAGE WAS CAUSED BY TWO BLOWN FUSES (ONE TO THE UPS AND ONE TO THE DRIVE).

AN UNINTENTIONAL ROOF FALL OCCURRED ON THE 4 SOUTH SECTION #6 ENTRY AT 77 + 32. THE FALL MEASURED APPROX. 15' W X 12' H X 25' L. NO INJURIES OR EQUIPMENT DAMAGED.

There was an ignition on the 3-north right section at the face between #8 and #5 entry at #117 crosscut. the ignition lasted 2 seconds and self extinguished by the water sprays on the miner. there were no injuries and no damage to equipment

A ROOF FALL ABOVE THE ANCHORAGE OCCURREED AT 39 WALL ON 3-NORTH # 2 BELTLINE WHICH AFFECTED VENTILATION ON THE BELTLINE. THERE WERE NO INJURIES & THERE WAS NO ENTRAPMENT. THE FALL OCCURRED IN THE INTERSECTION AT 39 WALL & WAS APPROX. 25 FT. LONG BY 20 FT. WIDE AND 15 FEET ABOVE THE ROOF LINE.

AN UNINTENTIONAL ROOF FALL OCCURRED OUTBY 4 SOUTH SECTION IN #9 ENTRY AT 90+11. THE FALL MEASURED APPROX 150' L X 16 1/2 W X 12' HIGH. NO EQUIPMENT OR PERSON INJURED.

A FRICTIONAL IGNITION OCCURRED AT THE FACE OF THE #4 ENTRY WHICH HAD BEEN MINED IN 132 FEET IN THE F-12 SECTION. THE FLAMES FROM THE IGNITION LASTED APPROX ONE MINUTE AND WERE EXTINGUISHED WITH THE WASHDOWN HOSE, 1 1/4 INCH WATERLINE AND A FIRE EXTINGUISHER. THERE WERE NO INJURIES OR DAMAGE TO EUQIPMENT.

THE EAST FINLEY ELEVATOR WAS DOWN FOR MORE THAN 30 MINUTES BECAUSE OF A STRETCHED GOVERNOR ROPE. THERE WERE MEN ON THE ELEVATOR AT THE TIME BUT WERE NOT ON THE ELEVATOR FOR THE MORE THAN 30 MINUTES. THE ROPE STRETCHED BECAUSE OF CHANGE IN TEMPERATURE.

A FRICTIONAL IGNITION OCCURRED AT THE FACE OF THE #4 ENTRY WHICH HAD BEEN MINED IN 35 FEET IN THE F-12 SECTION. THE FLAMES FROM THE IGNITION LASTED APPROXIMATELY ONE MINUTE AND WERE EXTINGUISHED WITH THE WASHDOWN HOSE AND 1 1/4 INCH WATERLINE. THERE WERE NO INJURIES OR DAMAGE TO EQUIPMENT.

THE EAST FINLEY ELEVATOR WAS DOWN FOR MORE THAT 30 MINUTES BECAUSE THE CHANGE IN TEMPERATURE CAUSED THE BRAKES ON THE DRUM TO BE OUT OF ADJUSTMENT AND CAUSING THE BRAKES TO HEAT UP. THERE WAS NO ENTRAPMENT OR DAMAGE TO EQUIPMENT.

THERE WAS AN IGNITION IN F-11 AT THE FACE OF #3 INBY 6 WALL 170'. THE IGNITION LASTED 20 SECONDS AND WAS EXTINGUISHED WITH WATER SPRAYS AND WATER HOSE. THERE WAS NO DAMAGE TO EQUIPMENT OR INJU RIES TO PERSONNEL.

THERE WAS AN IGNITION AT THE FACE OF #3 IN F-9 INBY 6 WALL. THE FACE WAS IN 160 FEET. THE IGNITION WAS SELF EXTINGUISHING. THERE WAS NO DAMAGE TO EQUIPMENT OR INJURIES.

WHILE WORKING ON AN AIR LUBRICATION PUMP ON A WORK BENCH IN THE REFUSE SHOP, EE'S LEFT THUMB WAS SEVERED BETWEEN THE BOTTOM OF THE NAIL & THE 1ST JOINT. WHEN AIR WAS APPLIED TO TEST THE PUMP, THE PISTON STROKED, CUTTING THE END OF HIS THUMB WHICH WAS INADVERTENTLY IN THE CHAMBER.

THE EE WAS OPERATING THE MINER WITH A REMOTE CONTROL WHEN A ROCK APPROX. 10""X 10""X 1"" THICK CAME OUT OF THE ROOF & CAUGHT HIS RT. LITTLE FINGER BETWEEN THE REMOTE CONTROL & THE ROCK. THE END OF THE FINGER WAS AMPUTATED.

Employee was laying on his side, pulling on a 36"" pipe wrench to remove a pipe nipple & he strained his rt. shoulder & upper arm.

EE STEPPED IN A HOLE WHILE DRAGGING A TUBE AND TWISTED BACK AND GROIN. DIAGNOSED AS A STRAIN/SPRAIN. THE INJURED BEGAN MISSING ON 1/13/05.

WHILE KNOCKING OUT A STOPPING WALL, A CONCRETE BLOCK FELL & SMASHED HIS FINGER. 2ND FINGER RT. HAND.

The employee had just completed welding on the panline in E-10. As he was crossing the handrail to get into the walkway, he struck his head on the shield tip. His first day off work because of the incident was 12/13/04.

EMPLOYEE WAS CHECKING THE CHARGE ON A SET OF BATTERIES IN 3I. HE RAISED THE LID ON THE SCOOP BATTERIES & WHILE HE WAS CHECKING THE CHARGE THE LID FELL ON HIS RT. HAND.

Ee was changing a broken break away pin on #85 shield, F-10 longwall face when the relay bar suddenly jumped up and caught the left middle and ring fingers of his left hand between the top of the pin and bottom of the panline cable trough, resulting in lacerations requiring 13 sutures.

At 8:45 he was pulling a discharge line off the head of the miner and heard a pop in the middle of his left shin area. He did not slip on anything nor did he step on anything to roll his ankle. Investigation was completed on 11-11-04.

EE WAS OPERATING PAN AT THE REFUSE AREA. A DRAG WAS BEING PULLED TO LEVEL ROADWAY. THE DRAG CABLE CAME OFF & HE HAD TO PULL IT OUT OF THE MUD. AFTER REATTACHING ROPE TO PAN, HE FELT PAIN IN RIGHT SHOULDER. LOST WORKDAYS BEGAN 1/20/05

Employee was operating loead motor, pulling a supply trip. As he approached #4 crossover he saw the track switch was for the curve, his motor struck air lock doors and an omega block fell out and struck back of his neck.

Employee was dismounting a mantrip, while getting out of mantrip he struck right knee on a link of chain that was welded to side of mantrip. Lost work days began 11/3/04.

5H MAKING BELT SPLICE, AFTER INSTALLING BELT CLAMPS, BELT DROPPED ON CO-WORKER'S ARM. HE HURRIED, GOT A BAR TO LIFT BELT AND PULLED A MUSCLE IN HIS LOWER BACK.

2i SECTION, RECOVERING RAILS WHILE UNHOOKING CLEVIS, RAIL SHIFTED AND PINCHED TIP OF MIDDLE FINGER RIGHT HAND CAUSING FRACTURE AND LACERATION.

Pushing against a chute on July 7, 2004. A coworker freed the chute with a pry bar, the full weight of the chute was being held by the employee. After this situation, Employee's ankle continued to bother him, but he continued to work until 9/22/2004 when he reagrevated the previously unreportable injury.

3I section, after breaking up rock on top of miners, he was getting down of mines as he went for the step, his pit belt caught on a bolt which caused him to miss the step and fell landing on his left ankle. lost days began 9-21-04

#2 mother belt at C-2 crossover overcast- injured was descending outby ladder on overcast when a piece of rock fell and caused him to slip and loose his balance on the steps and fall striking his left hip and leg on the steps.

7 H SECTION WHILE WALKING PAST THE LOAD CENTER, HE STEPPED ON THE RUBBER SHOCK MAT WITH MUDDY BOOTS, SLIPPED & FELL TWISTING HIS RIGHT KNEE.

While advancing the ventilation tubing in the 3I section the tubing fell, striking the employee on the head and shoulders. Lost work began 9/3/04.

Installing a stainless steel drip pan. Employee tried to move/adjust pan section; pan moved and jerked employee around.

6H section, beltman was separating belt stands when a stand fell over & struck injured on the side of his foot causing fracture. Lost work days began 8/17/04.

EE WAS DRAGGING OIL CANS OFF OF SHEARER & FELT A PAIN IN RIGHT SHOULDER, RT CHEST & NECK. CONT'D TO WORK WITH REPEATED TRAUMA TO SHOULDER FOR 5 MOS. WHEN EXPLORATORY SURGERY WAS PERFORMED TO DETERMINE IT THERE WAS ANY KIND OF INJURY TO SHOULDER. DIAGNOSIS IS UNDETERMINED AT THIS TIME.

Employee was climbing down off the right side of the continuous miner in e-11 section and his foot slipped. causing him to strike his left knee on the conveyor motor. Ee began missing work due to the injury on 8-9-04.

OPERATING MANTRIP WHEN HE PULLED BRAKE HANDLE & ROD CAME OUT OF MASTER CYLINDER & MANTRIP FAILED TO STOP. IT STRUCK AIR LOCK DOOR & TOP OMEGA BLOCK FELL OUT & STRUCK HIS NECK & MIDDLE BACK.

EMPLOYEE WAS ROOF BOLTING WITH A COBRA DRILL AT #72 SHEILD F-9 LONGWALL FACE WHEN A ROCK THAT MEASURED APPROX. 1'THICK X 2 1/2'WIDE X 15'LONG FELL & DISLODGED THE ROOF JACK THAT WAS SET. AS THE EE WAS MOVING OUT OF THE WAY. THE ROOF JACK STRUCK HIM ACROSS THE LOWER BACK CAUSING HIM TO FALL HITTING HIS NECK AGAINST THE RAIL OF PANELING. EE RECEIVED 3 FRACTURED VERTEBRAE.

While bolting on the miner in 6H miner secton, an auger got stuck. When employee pulled on the stuck auger he felt pain in neck. LTA began 7-29-04.

6H section, while walking he stepped on a flat rock, slipped and fell twisting his left knee and fell onto both knees.

Struck by rolling or sliding object

The employee was rotating the impellar of a pump, while assembling the unit. The pump rolled over - landing on top of the employee's LEFT foot.

Fall from machine

Prep plant refuse area. EE was dismounting pan after re-fueling. he missed the bottom step and fell landing on left foot leg. re-injury to right leg fracture when he fell at home on ice.

6H section, helping make splice on belt and adding a piece of belt, as he was lifting and pulling piece of belt into place and pulled a muscle in his groin area. diagnosised as hernia lost work days began 7-26-04.

PUTTING FIRST TOP CRADLE ON BEDRAIL, TOP CRADLE FELL. HIS FINGER WAS BETWEEN CRADLE AND BEDRAIL.

OPERATING SHEARER E-10 LONGWALL SECTION CUTTING THE TOP AT MIDFACE TAILGATE TO HEADGATE, HE STRUCK HIS HEAD ON A LOW SHIELD AND JAMMED HIS NECK CAUSING HIM TO FALL BACKWARDS TO THE MINE FLOOR.

EMPLOYEE WAS STANDING ALONG SIDE THE MINER OPERATOR'S COMPARTMENT OBSERVING ROCK BEING CUT & LOADED OUT FROM AN OLD ROOF FALL AT #8 WALL, #1 NORTH MOTHER BELT. THE HEAD OF THE MINER CAUGHT A 5'DRILL STEEL THROWING IT BACK & IT STRUCK HIM ON THE RT. FOREARM & UPPER ARM CAUSING A FRACTURE TO BOTH THE ULNA & RADIUS BONES.

WHILE SHIVING A ROLLER, WHEEL HIT HIM IN THE CHEST KNOCKING HIM DOWN. UPON HITTING THE GROUND, HIS HEAD HIT A COBRA DRILL.

EE WAS MOVING A ROCK FROM BENEATH THE TIRE OF A TRAILER BEING UNLOADED. EE PULLED ON THE ROCK, ROCK MOVED, EE'S HAND SLIPPED ALLOWING THE ROCK TO ROLL BACK INTO ORIGINAL POSITION, PINCHING EE'S RIGHT MIDDLE FINGER AGAINST ANOTHER ROCK. THE LACERATION REQUIRED SUTURES.

EE WAS USING TIE WIRE TO HANG VENT TUBE. AS EE PULLED DOWN ON ONE END THE OTHER END OF THE WIRE CAME UP AND WENT BEHIND HIS SAFETY GLASSES AND PUNCTURED HIS LEFT EYE. LOST DAYS BEGAN 6/23/04.

While walking in #5 entry the ee stepped in a hole and felt a pain in his lower left back. the injury was diagnosed as a strain.

Ee was pulling shields behind shearer at midface F-9 longwall section going from headgate to tailgate and struck his head against a low shield and felt pain in his neck and right shoulder. The ee began missing work due to the injury on 7-6-04.

At the end of his shift he had completed taking a shower, as he was exiting the shower stall, he tripped on a ledge used to keep water in the stall. He fell onto his right knee and twisted his back.

2I HEAD ROLLER ATTENDANT. EE RECEIVED A CALL TO CHECK BELT FOR MISSING MONO-RAILS. SHE WALKED BELT LINE AND TRIPPED OVER A FLAG ANCHOR AND FELL. FRACTURED 2 RIBS AND TWISTED KNEE. LOST WORKDAYS BEGAN 6/14/04.

FELT PAIN IN LEFT SHOULDER AND LOWER BACK. AREA WAS WET AND MUDDY. EE BEGAN MISSING WORK ON 6-4-04. DIAGNOSED AS A ROTATOR CUFF TEAR ON 6-4-04.

EE WAS IN #2 ENTRY HELPING TUBER TAKE DOWN A TUBE WHEN THE RIB ROLLED OUT FROM BEHIND THE TUBE AND STRUCK HIM ON THE LEFT LEG AND ANKLE. THE INJURY WAS DIAGNOSED AS A FRACTURE OF THE LEFT FOOT .

3I section during a belt move, he reached down to grab and pull a bottom roller, which knocked a stand over. The stand came down (fell) and struck his left hand little finger causing laceration and fracture lost days began 3-24-04.
