AN UNINTENTIONAL ROOF FALL OCCURRED IN THE 8H SECTION. THE FALL WAS LOCATED IN THE RETURN ENTRY. THE FALL MEASURED 20 FT WIDE X 20 FT LONG X 12 FT HIGH. LOCATED @ 30 WALL.

AN UNINTENTIONAL ROOF FALL OCCURRED ON THE 4 SOUTH RT SECTION AT 56 1/2 WALL. THE FALL MEASURED APPROX 35' X 25' X 10' TO 12' HIGH. NO INJURIES AND NO EQUIPMENT DAMAGED. TRACK ENTRY.

An ignition occurred at #142 shield, F-11 longwall face. The ignition lasted 2 to 3 seconds and was self-extinguished by the water sprays. There was no damage to equipment and there were no injuries.

THE ENLOW HOIST WAS DOWN FOR MORE THAN 30 MINUTES BECAUSE THE DOOR WOULD NOT FUNCTION. DURING THE TROUBLESHOOTING PROCESS ALL THAT WAS NEEDED WAS TO HIT THE RESET BUTTON AND THE HOIST FUNCTIONED PROPERLY.

Employee was on headgate end of shearer at #90 shield while the machine was backing up toward the headgate cutting up rock between the drums. The spray bar sprung out over the cable though and his right little finger was caught between the handrail and end of spray bar. Diagnosed as a fracture.

He stepped over balony skin; his foot caught the top of the balony skin. He tripped and fell on his left ankle. Hairline fracture to ankle.

EMPLOYEE WAS PULLING A SAFETY CHAIN OUT FROM UNDER STRUCTURE ON A DOLLY AT ENLOW BOTTOM AS ANOTHER EMPLOYEE WAS USING A SLATE BAR TO RAISE STRUCTURE AND FELT A POP IN HIS RIGHT SHOULDER. EMPLOYEE CONTINUED WORKING AND THEN BEGAN MISSING WORK ON 12/27/05.

EMPLOYEE WAS USING A SLIDE HAMMER TO REMOVE A TRAM SHAFT ON THE F-12 LONGWALL SHEARER AND PINCHED HIS LEFT RING FINGER BETWEEN THE SLIDE AND SLIDE STOP CAUSING A FRACTURE AND LACERATION TO HIS FINGER, REQUIRING 10 SUTURES TO CLOSE.

Employee was on #4 shield E-12 longwall face kneeling while pushing the panline out. The horizontal pin came out of the ram bar clevis allowing the clevis to roll catching his left lower leg between the clevis pin and clevis rail. EE sustained a fracture to the tibia and fibula of the left lower leg.

EMPLOYEE WAS CARRYING A SWING ARM FOR A SHUTTLE CAR IN #3 ENTRY E-14 SECTION WHEN HE STEPPED IN A HOLE, FELL DOWN AND FELT A PAIN IN HIS LEFT GROIN. HE CONTINUED TO WORK UNTIL 11-28-2005 WHEN HE WENT TO A HOSPITAL AND WAS DIAGNOSED WITH A HERNIA.

EMPLOYEE WAS OPERATING THE E-12 LONGWALL SHEARER CUTTING BOTTOM HEADGATE TO TAILGATE PASS WHEN THE SHIELD SRB WAS INITIATED THE WRONG DIRECTION. #20 SHIELD ADVANCED, KNOCKING HIM DOWN AND PUSHED HIM INTO THE PANLINE CATCHING HIS RIGHT LEG BETWEEN THE PANLINE AND SHIELD, CAUSING A FRACTURE TO THE TIBIA AND FIBULA OF THE LOWER RIGHT LEG.

Employee was tramming a shuttle car over a bridge when it came off the bridge and onto solid bottom. The frame bottomed out, causing her to hit her head on the canopy jarring neck/back/jaw.

EMPLOYEE WAS CLEANING CORDS AND A PIECE OF DOG CHAIN THAT WAS WRAPPED AROUND THE TAILROLLER SHAFT AT THE 4I TAILPIECE. THE BELT WAS TURNED OFF. WHILE ATTEMPTING TO REMOVE THE OBSTRUCTION, THE BELT UNEXPECTEDLY DRIFTED CAUSING HIS HAND TO BE CAUGHT. HIS REACTION WAS TO PULL HIS HAND OUT. THIS CAUSED THE LACERATION.

7H CAN LINE. WHILE TRYING TO GO THOUGH A MANDOOR AIR PRESSURE CLOSE DOOR ON HIS LEG CAUSING HIM TO FALL AND TWIST HIS LOWER LEG BACK. HE ALSO HAS HAD PRIOR BACK PROBLEMS.

While unloading a miner pot off a jeep onto a scoop, the employee pinched his left little finger between the pot and scoop bucket EE was removing the chain from the pot that was sitting on the bucket when it shift. Diagnosed as a fracture to the tip of the left little finger.

Employee was watering tram roads in the 3 north right section and when he went to pull up slack in 300' of hose he felt a pain in his groin. He began missing work due to injury on 18/18/25 after diagnosed with a hernia.

EMPLOYEE WAS WALKING IN #2 TO #3 ENTRY CROSSCUT AT #16 WALL, F-13 SECTION. HE TRIPPED AND FELL JAMMING HIS LEFT ARM CAUSING PAIN IN HIS LEFT SHOULDER AND NECK. EMPLOYEE CONTINUED TO WORK UNTIL 12/27/05 WHEN HE BEGAN MISSING WORK FOR SURGERY.

Struck by falling object

Employee was walking along panline on headgate side of shearer between #174-#175 shield F-11 longwall face. He had his right hand on the handrail and a piece of rock, 6"" x 10"" x 18"" fell from between the shields striking him on the right hand, causing a fracture to the right little finger. Employee began missing work on 10-05-05, and returned to work on 10-07-05.

Was shiving belt motor when chain broke-EE wasn standing approimately 40 feet away-1 link of the chain flew and hit him between his eyes.

INJURED WAS LIFTING CONCRETE BLOCK OFF BOTTOM WHEN A BLOCK FELL FROM TOP OF LIFT STRIKING HIM ON LEFT HAND CAUSING A BRUISE.

7H section using a sledge hammer to remove belt structure, he accidently hit the belt, causing the hammer to deflect and strike his right shin. causing abrasion which became infected.

WHILE EE WAS ATTEMPTING TO CLEAN THE SUCTION ON THE RIGHT SIDE BOLTER CHUCK OF THE CONTINUOUS MINER IN F-TAILGATE, HE WAS STRUCK IN THE RIGHT HAND BY A DRILL STEEL THAT WAS PREVIOUSLY STUCK IN THE ROOF. DIAGNOSED AS A FRACTURED RIGHT MIDDLE FINGER.

EMPLOYEE WAS WALKING TOWARD THE BOLTER IN #8 ENTRY 3 NORTH RIGHT SECTION WHEN HE STEPPED INTO A WATER HOLE CAUSING HIM TO FALL BACK AND TWISTED HIS LEFT KNEE. EMPLOYEE BEGAN MISSING WORK DUE TO THE INJURY ON 10/27/2005.

While operating mantrip the pole came off the wire. Employee had brake held with right hand and tried to pull the pole down with left hand and felt a pain in left shoulder. Diagnosis: sprain in left shoulder.

Employee received a hairline fracture to his ribs as a result of being picked up by a co-worker. He continued to work until 9/7/05.

Employee was using a battery powered cart to deliver a part to his jeep. After unloading the part, the battery was out of power. He pulled the cart out of the way and felt pain in abdoment. Diagnosed with hernia and lost work days began 9/7/05.

EMPLOYEE WAS LIFTING A BOTTOM ROLLER AT #8 WALL CROSSCUT F-13 BELT TO TRAIN THE BELT AND FELT A PAIN IN HIS LEFT GROIN. EMPLOYEE CONTINUED TO WORK. LATER DIAGNOSED AS A HERNIA AND BEGAN MISSING WORK ON 11/1/2005.

Over-exertion in lifting objects

Employee was repositioning an 8' step ladder inby 3 North #4 belt drive to assist with removing a bad bottom roller. While lifting the ladder out of the mud he felt a pain in his lower back.

4I longwall section, employee was lifting a welder up on car and felt pain in right side lower abdomen. Diagnosed as hernia 9/6/05. Lost work days began 9/15/05.

Employee was standing on bar behind compactor drum to clean the windows of the machine's cab, when he slipped and fell off, spraining his right ankle.

EMPLOYEE WAS OPERATING #4 TEREX PAN. THEY WERE STARTING A BENCH BY DROPPING REFUSE TO BUILD A NEW ROAD. AS HE STARTED TO UNLOAD REFUSE MATERIAL THE BACK TIRE OF THE PAN SLID OFF THE REFUSE & THE PAN ROLLED OVER.

While reaching for a wrench off a rib protector a rock fell striking left hand. Rock was approximately 14"" long and 8"" wid eand 4"" thick. The roof was 9 foot in height. Diagnosed as a contustion.

4I BELT OVERCAST, SHE WAS WALKING UP STEPS, COAL CAME OFF BELT, SHE TURNED TO GO BACK DOWN THE STEPS, SHE TRIPPED & FELL DOWN 3 STEPS, TWISTED LEFT KNEE.

Employee was helping install roof bolts along 7H belt for additional support. Co-worker pulled on slack hose causing the stoper drill to fall, injured tried to hold onto drill, but caught his left hand between the drill and the belts and bed rail.

Employee was operating #11 motor in the E-12 track entry. When he went across the #35 wall track switch the motor whipped to one side dislodging the seat from the motor deck. The seat fell over, causing him pain in his lower back.

While spotting super posts at 7H headgate- ee and the foreman leaned their post on the rib beside another - the other one fell and hit employee on the left side of the face - laceration on left eye, 3 sutures.

EMPLOYEE WAS JACKING AN OVERCAST BEAM UP TO CLEAR THE CONTINUOUS MINER DURING AN EQUIPMENT MOVE. THE JACK BAR SLIPPED OUT OF THE JACK CAUSING HIM TO LOOSE HIS BALANCE AND FALL TO THE MINE FLOOR JAMMING HIS RIGHT SHOULDER

Bending a rail with rail bender and the rail broke causing a pain in his back. I was notified on 11-14-05 that the employee was off. Due to his injury.

7 H LONGWALL SET UP, UNLOADING SHIELD OFF DOLLY. AS EE WAS BACKING SCOOP UP, BACK WHEELS WERE OFF THE GROUND. AS HE TRAMMED SCOOP, WHEELS CAME TO GROUND AND SHIELD BOUNCED THE SCOOP. HE HIT HARD HAT (HEAD) ON CANOPY.

Employee was a passenger, riding out of the mine on a jeep. A low boy trip was parked on the main line (D.C. power had tripped). The jeep hit the parked trip, he put hands up to brace for impact but sustained a fracture to left thumb & wrist.

3I belt transfer, while walking along 3I storage unit, he stepped on a piece of rock and fell onto his right knee. Lost days began 5-3-05.

EE WAS ROOF BOLTING IN THE LONGWALL RECOVERY CHUTE FOR F-13 IN THE LEFT 3 NORTH SECTION. WHILE POSITIONING THE CENTER BOLTER TO INSTALL A 12' ROPE BOLT ON THE LEFT SIDE OF ENTRY, HE HAD HIS LEFT HAND RESTING ON THE BOLTER CANOPY. AS HE TRAMMED THE BOLTER, HE CAUGHT HIS LEFT ARM BETWEEN THE RIB AND CANOPY.

EE WAS DRILLING BOLT HOLE & WAS HAVING TROUBLE. HE DECIDED TO CHANGE THE BIT. AS HE BEGAN TO DROP DRILL HEAD THE STEEL BECAME STUCK. 2 UPPER STEELS STAYED IN THE MINE ROOF & THE BOTTOM WAS OUT OF THE ROOF. USING THE BOLT WRENCH TO HIT THE DRILL STEEL HE HIT THE ROATION LEVER. STEEL SEPARATED, SPUN AROUND & STRUCK EE IN THE FOREARM.

WHILE THE MINER WAS CUTTING ROCK ON THE OVERCAST, AN OBJECT FLEW BACK FROM CUTTER HEAD AND HIT HIM ON THE RIGHT SIDE OF HEAD NEAR RIGHT EAR. HE WAS STANDING NEAR CAB OF MINER. DIAGNOSED AS CONCUSSION.

Injured pushed come-along up monorail and hooked it with a chain. Chain slipped off and come-along rolled back and caught his thumb causing a crushing type injury.

7H BELT WINDER ROLLING UP BELT, THE ROLLER BROKE ON BOTH SIDES OF THE HANGER. THE ROLLER FELL & STRUCK THE TOP OF RIGHT FOOT CAUSING A FRACTURE TO 2ND & 3RD METATARSUS.

EE WAS WALKING IN THE TRACK ENTRY BY THE SCOOP THAT WAS PARKED IN FRONT OF THE LOAD CENTER WHEN EE SLIPPED ON A ROCK CAUSING EE TO FALL INTO A RUT AND HYPEREXTEND RIGHT KNEE AND CAUSING RIGHT SIDE OF BODY ABOVE THE HIP TO STRIKE THE SIDE OF THE SCOOP BESIDE THE TIRE. DIAGNOSED AS A STRAIN TO THE RIGHT KNEE

Injured was standing in loose material and lifting a concrete block overhead when he fell and block fell striking him on right shoulder causing a fracture.

Walking out of the mine during an evacuation, his right leg became tired and his hamstring muscle tightened. He continued to walk out of the mine. He did not slip, trip or fall. The repeated trauma of the walk caused inflammation to the muscle (lost day began 3/14/05).

The employee was standing on a piece of waterline on the outby side of C-2 crossover cutting off belt structure bolts when he slipped off of the waterline and fell to the ground landing on an old belt scraper. Diagnosed as a fractured rib.

EE was installing a roller at E-13 storage area when his right hand slipped off of the roller and fell to the belt below. His left hand was unable to keep the roller up and it fell catching his right middle finger between the lower belt which was supported by a metal brace and the falling roller. The injury was diagnosed as a fracture to distal phalange of the right middle finger.

While planking through the 3-2 x-cut, the miner bolters were installing a strap. As they were pushing up the bolts two small rocks fell and one knocked his hat off and the other struck him on the head causing a laceration to the scalp requiring two sutures to close.

Employee experienced an Asthma attack, physicians are diagnosing ""Occupational Asthma"", the employee has already been diagnosed with the breathing condition and maintains the condition with me ds. He ran out of meds, failed to refill it, the EXISTING condition worsened while at work, resulting in hospitalization. Hospitalization occurred during non work hours.
