The east finley elevator was down for more than 30 min due to a door interlock failure. There was no entrapment.

A Roof fall occurred in #1 Entry 3 North at 199+38. The fall was above the anchorage. There was no damage to equipment. The fall was 16' wide, 16' high and 41' in length.

An ignition occurred at approximately 98+94 in #3 Entry, E-17 Section. The ignition lasted 1 to 2 seconds and was self extinguished by the water sprays from the continuous miner. There was no damage to equipment and there were no injuries.

A mechanic was performing the daily inspection of the elevator & shaft. He was operating the controls on manual. He stopped the elevator & could not get it started. The E-stop switch was stuck in & would not allow the cage to move.

The weekly emergency hoist was being load tested for the monthly test. After the capsule had been lowered to the bottom, it could not be brought back up. The emergency hoist was inoperative.

An unintentional roof fall occurred in the 8I section at the tailgate at 35 wall. The fall measured approx 10' long x 8' high x 16' wide. No injuries & no equipment damaged.

Smoldering belt shavings on 10 H belt, occurred at 6 am on 2/27/07 was determined by MSHA investigation on march 13, 2007 of an incident that they determined that it was a reportable fire under part 50. However the operator still believes the smoldering area once found was extinguished in less than 10 min. is not a reported fire. This form is being sub. (#7024738)

An ignition occurred at 82+14 on the F-14 Longwall face at #28 shield. The ignition lasted approximately 2 seconds and was extinguished by the water sprays and a wash down hose from the sheild. There was no damage to equipment and there were no injuries.

Elevator at Crabapple continued to operate properly, until 3:05 AM, when an electrical malfunction in the solid state drive circuit board caused the elevator to stop. No one on the elevator.

Crabapple elevator was operated 35-40 times without incident & once again it stopped. No one on the elevator.

During investigation, it was determined EE was checking a coal chute for possible plugged chute between plant 1A & 1B on 6th floor of prep plant. He placed himself on side of chute & splitter gate support frame. Had pinned him against chute causing injuries to pelvis, legs, right arm &legs.

EE was cutting a top roller assembly with a torch to remove it from frame for the 3 north #5 head roller when the assembly fell catching his left hand between the assembly & frame resulting in fracture to left index finger. This injury later resulted in an amputation of the left index finger.

Struck by falling object

A lowboy mine car was being lifted by a jack. The jack pivoted; the car came down the rail causing it to go down. The miner was standing out 4 feet inby the car with left foot under rail breaking 3rd & 4th metatarsel bones.

Employee stepped down out of motor onto a track brace tie that was covered with slag and slipped off of tie twisting/rolling his right foot resulting in a fracture to the right foot.

Struck by falling object

Employee was in the process of placing the tower drill steel into the top steel he was holding with his left hand when a piece of rock 12"" x 12"" x 6"" thick fell from between the roof straps and struck him on the right forearm. Employee sustained a fracture to the right forearm.

Replacing cover plate on miner, the lid fell pinching his ring and middle finger on his right hand.

Over-exertion in lifting objects

Employee was lifting an acetylene tank in the Enlow Bottom Shop and felt a pain in his lower back.

Employee was standing on #41 Shield, F-15 Longwall when a piece of rock 14"" x 15"" x 5"" fell from above #42 shield and was thrown from the headgate drum of the shearer striking him on the nose and forehead. Employee sustained a fracture to his nose.

Struck against a moving object

Bailey elevator stopped suddenly causing employee to get bounced around. Felt pain in lower back. ** Employee was diagnosed with Sprained back on 11-14-07.

Employee was hanging a ventilation tube when he slipped and fell tearing his left Quads.

Caught in, under or between a moving and a stationary object

EE was lifting the monorail jack to place it into the bracket on the left side of the stageloader, when the valve at the end of the hydraulic hose caught on top of the stageloader & lowered the cylinder catching his right thumb between a fitting on top of the cylinder housing & the cylinder. EE was wearing gloves. HE sustained a fracture to the right thumb.

Over-exertion in lifting objects

Employee was lifting belt structure to align bedrail in E-17 and felt a pain in his lower back.

Absorption of radiations, caustics, toxic and noxious substances

Putting comet with bleach (watered down) on floor and it splashed up in his eye. Has chemical burn to eye and needs to see a specialist. EE was using an unauthorized cleaner and was not wearing safety glasses - which he had been instructed to do.

Over-exertion in pulling or pushing objects

EE was loading a TRS jack onto a jeep and while using a bar to help raise one end felt pain in his lower back. He continued to work until 1/28/08 when he began missing due to his injury.

Employee was carrying a track tie when he felt pain in his left shoulder. *Employee was diagnosed with a torn rotator cuff on 10/23/07.

Struck by rolling or sliding object

Employee was working as a rib bolter helper, #3 entry, 3 North Left Section. He bent down to pick up bolt wrench when rib measuring 4' x 7' x 1' rolled out and struck him on his right side and right leg. He sustained 3 broken ribs and a compound fracture to the lower right leg.

Over-exertion in pulling or pushing objects

Employee was removing a rock from on top of the miner with a pry bar and felt a pain in the back of his neck.

Struck against stationary object

While operating a trolley jeep the pole came off the wire. The employee attempted to put pole back on wire and struck head against the roof. Employee started missing time on 9/29/07.

Employee was carrying a post along the beltline. He slipped and fell on his right knee. He started missing time on 9/27/07.

Employee has recurrent low back pain after handling material in the mine. He was diagnosed with a lumbar sprain and started missing time on 9/826/07.

Employee slipped and fell while cleaning a belt line. He fell on his left knee. Employee began missing work time 10/15/07.

The individual was walking the 913 refuse belt looking for bad rollers. He observed a build-up of mud on a roller. He tried to remove the debris with a wrench. His arm was caught by a roller and pulled in.

Employee was helping to replace a cylinder jack on the left side bolter on the miner. Mast assembly fell striking employee's right leg.

Employee was climbing into the operator's compartment of the loader in #7 to #8 crosscut, 3 North Right Section and tripped on loader cable twisting his left ankle.

Employee was climbing down off of the continuous miner in #1 Entry, 3 North Left Section. His left foot caught on a water hose causing him to fall to the mine floor jamming his left wrist into the rib, resulting in fractures to the left wrist.

Struck by falling object

Employee was making a gas check at the face of #2 Entry, 3 North Left Section when a piece of rock fell from between roof straps and struck him on top of the left shoulder.

Employee stepped back as scoop was delivering supplies to the miner in #2 Entry, E-17 Section and stepped in a hole twisting his left ankle. Employee continued to work until 10-18-07 when he began missing work due to the injury.

Employee was helping to install cable bolts. As he was picking up a bolt, rock fell from between the roof straps striking his right hand.

Fall from machine

EE was stepping onto #29 shield that had previously been spotted over top of the shearer, F-14 Longwall Section. The shield completed its cycle and pulled forward 12"" to 18"", catching his right knee pad on the face phone and caused him to fall back, felt pain in his left hip.

Employee was carring a conveyor roller through a door when he tripped and fell to the floor.

Employee was loading Belt structure on a scoop and felt pain in middle of back. ***Lost work days began 8/13/07

Struck by falling object

Employee was center bolting in #2 entry, E-17 section when rock fell from the left rib and struck him in the lower back and legs. The rock measured 3"" to 18"" thick, 14"" to 22"" wide & 8' length.

Employee was helping to hang mono rail in the belt entry. When job was completed he jumped down from the belt and sprained his knee. (Conveyer belt was out of service.)

Employee was crossing top of #1 1 North Mother belt when his right foot slide out from under him and twisted his right knee. The employee continued to work until 8-29-07 when he began missing due to the injury.

Employee was caryying to Toyo Pump across the E16 track at #1 wall when he slipped on a track & felt pain in his left knee. Employee began missing work on 7/19/07.

Employee was standing at the E-15 longwall headgate area when a roof bolt broke/shot from the roof striking the employee in the right arm. Diagnosis as a contusion.

Employee was helping to put a supply car on track. After walking back to motor he had pain in left knee. Diagnosed with a torn meniscus on 7/16/07.

Struck against stationary object

While finishing filling rockdust tanks, the line became plugged. EE had a hold of the hose when it unplugged. The pressure on the hose caused EE to hit his hand off the dust tank. 3 sutures to close. Lost workdays began 7/9/07.

Employee was working on belt when belt tightened up kicking tailpiece sideways pinning employee between structure and tailpiece.

Underground Inby. Door blew open and pressure caused an Omega block to fly out and hit Employee in back of head.

Employee fell due to a sudden stop of the Bailey elevator and injured his left shoulder. Diagnosed on 7/19/07 with a torn rotator cuff.

Over-exertion in lifting objects

Employee bent over to pick up a loose track tie & as he picked up, he felt pian in right lower abdomen. Diagnosis: hernia.

Employee was walking on the opposite end of a Goodman Manbus that was in the process of being jacked back on track at #30 wall spur track switch E-16 Section. He tripped on the rail & fell with his left foot going under the manbus as the bus was slewing over on the other end, causing his left foot to be caught between the bus & rail.

Employee was walking in #2 entry. E-17 section when he stepped in a hole with his right foot and felt a pain in lower back. He continued to work until 1/24/08 when he began missing due to his injury.

Struck against a moving object

Employee was operating a jeep while firebossing when #144 wall switch, 3 N F-track had been left throw for the curve. Seeing switch, he applied brake & dogged the pole, but jeep kept going around, threw him forward & he struck right side/shoulder against body of jeep. EE cont'd working until 7/25/07 when he had surgery to right shoulder.

Working around underground coal bin when water and mud overflowed from bin knocking employee off his feet.

Shoveling on the wide side of 72"" belt, beneath #17 storage bin, in the bunker. The bin flushed and as he tried to get out of the affected area, he fell resulting in a fracture to his left shoulder.

Lifting a skirt board with a come-a-long when a piece of rock measuring 3' x 2' x 2"" fell from the top and struck him on the left ankle, resulting in a fracture to the ankle. (The employee had returned to work on 7-10-07 with 21 days away from work, worked 2 days and began missing work again on 7/12/07 due to the injury)

EE was attempting to put a pin in the tram motor of a locamotive on crabapple bottom, when he got his finger pinched between the pin he was installing and a track rail injury caused lost work days beginning 6/12/07

Employee was walking along the walkway side of the #3 North Belt and slipped on loose material. Grabbing hold of a chain with her left hand to keep herself from falling she felt pain in her elbow. Employee continued to work until 9/21/07 when she began missing due to the injury.

Employee was drilling on right side of the continuous miner in #3 Entry, E-17 Section when a piece of rock 22"" long, 14"" to 4"" wide, and 2"" thick fell and struck him on top of the right foot, resulting in a fracture to the right foot.

Employee experienced ringing in both ears from a loud pop during the cutting & burning of a rail at #2 wall, E-16 track entry. EE began missing work 6/4/07.

While loading cable, injured placed his foot under scoop bucket. The bucket came down and hit his foot.

While walking out from 19A bleeder inlet examination, walking along the supply cars when he walked into a broken rib strap which cut his forehead. He struck his left shoulder & jammed his neck. *Lost work days began 5/9/07.

Struck by falling object

Employee slid the slider tube back after holing the crosscut #2 to #1, 3 North Left Section when a piece of rock fell from the top of the rib and struck him on the upper right side of his back, and back of his right leg at the calf. Employee continued to work until 6/18/07.

Injured twisted left foot on hose while carrying belt structure.

Employee was walking from the F-9 bleeder inlet when his left foot caught on a rock causing him to twist his left ankle. The employee continued to work until 8-11-07 when he began missing work due to the injury.

He was using a slate bar to pry over a pillow block to align a belt roller. As he was applying pressure on the pry bar, it slipped and he fell down, twisted and fractured his left ankle.

Operator was letting scoop bucket down. Injured saw a block underneath and tried to get block out and pinched thumb under bucket.

Grader operator dismounting steps from grader. He missed the last step (cable step) & strained left arm & elbow. *Lost work days began 4/6/07.

Struck by falling object

Employee was side bolting in #1 entry, E16 Section when a rock fell knocking ventilation tubes down on top of him causing him to strain his lower back.

Injured & co-worker were carrying a bolter head from mantrip to the scoop. He tripped over a rail, fell & pulled a muscle behind his right knee.

Fall down stairs

Injured was carrying wedges down steps on overcast and lost footing and fell. Type of injury is unknown.

EE and co-worker were laying rails and using a rail drag to lift the end of rail up over the track ties. As they lifted a swung rail, it hit a tee and bounced back onto his right foot.

Employee, motorman had a car off track & was in the process of using air bags & lifting jack to put trucks back on track. He was using a bar to lift wheel over the rail, when the car shifted & fell & struck his left hip area.

Fall from machine

Two employees were filling a water tank in the back of a pick up truck, when the water was turned on the hose got away from an employee and he fell out of the back of the truck. He landed on his shoulder.

Employee picked up a bucket of bits in #7 1/2 wall crosscut, F-13 longwall section, when the corner of the rib fell causing him to turn away quickly. He lost his balance and fell jamming his right hand into the mine floor. He sustained a dislocation to the right little finger and a laceration to the back of the right hand requiring sutures. Employee was wearing gloves.

8I track, as ee was walking up to the section, he tripped over a tie & fell. He experienced lower back pain & a sore right wrist. *Lost work days began 3/12/07.

Employee was stepping onto a crib block to cut a flapper from 3 north #5 belt & felt pain in left knee. Employee con't to work until 7/12/07 when had surgery for torn acl.

Over-exertion in lifting objects

Employee was using a slate bar with a co-worker to carry a shearer rack wheel onto the F-13 longwall face. He felt a pain in his left lower back while lifting the rack wheel at #2 shield (Employee began missing work due to the injury on 4-12-07).

Slipped on coal when he was carrying buckets of sealant and hurt left knee.

10 H headroller, walked from beltline, into the track entry, he tripped over a corp board, causing his left foot to twist outward. This twist put strain on his upper leg (fibula) causing a fracture. *lost work days began 2/26/07.

Contact with electrical current

Employee was in the process of closing the breaker on #111 unit for the #111 belt drive. The breaker linkage was jammed, he reached in to pry linkage so it would allow the breaker to close, when he accidentally received an electrical shock. Employee was sent for medical evaluation & received NO medical treatment.

Block fell and struck injured's right hand.

Fall from piled material

Employee was bolting roof after a shot was fired. He was moving a jackleg when he slipped on the muck-pile falling on his left knee. This resulted in a contusion to his left knee.

The employee was removing a 4' by 8' screen deck - it became stuck - pressure/tension was applied. The deck section came loose and struck the employee on the right ankle when it dropped free.

Putting in fire tap - fell - broke right heel.

Employee was using a chain come-along to raise the belt to change a roller at #18 wall, #1 north mother belt. While holding the chain with his left hand the come-along slipped and caught his left index finger between the chain and come along resulting in a fracture to the tip of left index finger.

Employee was sliding a section of rail over top of a piece of rail at #143 wall switch, 3 north F-track. He caught the tip of his right middle finger between the two rails.

10 H laying track, jack was used to raise rail so it could be lugged to the track tie. He missed hitting lug and struck the rail. The rail came off jack and landed on top of his right foot. *Lost work days began 1/4/07.

While exiting mine on elevator it came to a sudden stop then dropped a short distance causing injured to lose balance and fall resulting in contusions and sprain to right ankle.

Help to load 4"" h-beams. He stated his back became tight. Diagnosied with sprained lumbard started missing time 8/13/07

Struck against stationary object

Mechanics were taking the cab off of the 14 BU loader, after mounting bolts were removed the cab was free and it dropped and kicked back. Employee had his right hand on canopy leg which struck his right thumb causing a fracture to his thumb.

Janitor was removing trash from can and strained his groin

Fall onto or against objects

Employee was bolting on left side of miner when a slab of rock fell inby the last strap knocking the vent tubing down on top of him causing him to strik his forehead against the miner. Employee sustained a laceration to the forehead requiring 7 sutures.

Employee was struck in the mouth by a piece of rebar while working on the 10 longwall face. *Required sutures to close*

Employee was placing a bolt on top of an extension when the 12' rope bolt he was installing fell and caught his right little finger between the bolt and jaws of the Fletcher Bolter. Employee was wearing gloves.

Employee was walking in section and was struck on the right hand by a rock that fell from the rib. (employee recived 5 sutures)

Employee was lowering a brookville bus with a jack and bar when his left hand slipped off the bar allowing the bar to come up and striking him in the lower lip.He received 7 sutures to the lower lip.

Employee was operating shearer in F-15 longwall section at #160 shield when a piece of rock was thrown by the tailgate drum and struck him in the left upper lip. He received 1 suture.

EE was installing a bolt at corner of #1 to #2 crosscut, F-16 Section when a piece of rock 12"" x 12"" x 2"" thick fell and struck his hard hat forcing his safety glasses down causing a laceration to the left eyebrow requiring 6 sutures.

Employee was handling 2"" plastic pipe at C-2 when he slipped and fell cutting hisleft forearm on a piece of slate. Employee received five sutures.

Mine bolter auger was clogged & he was hitting the auger against the drill head of bolter & his finger got pinched between auger & bolt head. Sutures to close.

Employee was drilling on left side of the continuous miner in #3 Entry, E-17 Section when a piece of rock 30"" long, 24"" wide and 2"" thick fell and struck him on top of the left forearm, causing a laceration requiring 9 sutures to close.

Struck by flying object

Employee was using a hammer & punch to hammer out a canopy pin. A piece of metal broke off the hammer & flew into his chin. A small cut under his chin. Sutures to close.

Struck by flying object

Employee was standing 8' away from co-worker using a sledge hammer to pound a bit insert into miner drum, when a sliver of metal flew & struck the back of his right hand. *Sutures to close.

Fall down stairs

Employee was walking down stairway of slope hoist house, missed step, lost balance and fell down steps striking the upper right side of his back against the steps, resulting in a fracture to two ribs.

Struck by falling object

Employee was installing cable bolts with the center bolter, after installing a bolt he lowered the atrs and a piece of rock fell from the roof and struck his right cheek causing the laceration.

Caught in, under or between a moving and a stationary object

Employee was walking along the F-13 longwall face at #105 shield when a piece of sandstone was thrown from the headgate drum and struck the spray bar and caught his left little finger between spray bar and the handrail, causing a fracture to the left little finger. Employee was wearing gloves.

Fall onto or against objects

Employee was spreading ""ice melt"" on concrete section of slope that was covered with snow and ice. He stepped on the ice before it had melted, lost his footing and fell onto a concret form that was approximately 12 inches above grade. This resulted in bruised mid back and ribs.
