THE WEEKLEY ESCAPE CAPSULE WAS INOPERATIVE FOR AN EXCESS OF 30 MINS. DUE TO A PROBLEM WITH THE BRAKE MODULE RECTIFER. THIS CONDITION WAS FOUND BY AN INSPECTION OF SUCH CAPSULE.

THE EAST FINLEY ELEVATOR WAS DOWN FOR MORE THAN 30 MINUTES WITH NO ENTRAPMENT OR DAMAGE TO EQUIPMENT BECAUSE THE DOOR SWITCH BOX ON PORTAL BOTTOM BROKE AND NEEDED REPLACED.

EE was attempting to lift a large piece of rock from the toes of #32 shield, F-13 longwell face. He lost control of the rock and it fell catching the tip of his right ring finger between the relay bar and the rock. EE was wearing gloves.

Employee was walking to the bunker. He tripped on the track and fell and twisted left knee. Lost work days began 12/26/06.

Employee stepped on a piece of coal injuring his right ankle.

Struck by powered moving object

Employee was initiating a bank push of the panline at #4 shield, E-15 longwall section. The #3 shield slid back causing strain on the high pressure shield to shield hose, resulting in the manifold breaking off and he was struck in the left thigh by the 1 1/2"" hose and coupler. The ee sustained a fracture to the left femur of the leg.

Employee was dismounting a belt crossover, when he stepped down he felt a pop in his right knee. *Lost workdays began on 11/28/06.

Employee was carrying 4 rope bolts on his rt. shoulder walking in #1 entry E-tailgate section when his rt. foot caught in loose mine canvas causing him to twist his lower rt. back.

Ee struck on the left foot by a skid that was loaded w/a face conveyor motor lower supply yard. He was attempting to remove a pipe plug & pipe fitting reducer from conveyor motor, chained to the skid and had been raised on one 6""-8"" by a front end loader. He sustained a fracture to the big toe & laceration to the bottom of the foot requiring 16 sutures to close.

Injured was walking or standing along side of scoop and his hand got caught between wheel and fender of scoop. Life flighted to Ruby Memorial Hospital.

8I section, unloading belt structure, he stepped off low boy car onto uneven bottom and twisted his right ankle.

Employee inadvertantly stepped in front of #14 shield as it was advancing and caught his right foot between the shield toe and relay bar and fell forward jamming his left shoulder. Diagnosed with a hairline fracture to the 3rd metatarsal.

Employee was unloading oil at fan in E-15 section when he stepped in a rut and twisted his right knee. Employee began missing on 1/25/07 for surgery to torn ACL of the right knee.

An A-40 Volvo off-road truck rolled over on a slope while hauling clay for the construction of the soil liner in area 4.

Injured was carrying rock dust tripped over a rock and fell twisting left elbow.

Miner bolter, getting roof bolt off miner, as he stepped, onto a flat rock, he twisted and fell onto his rt knee. He continued to work normal duties until he was scheduled for surgery. 11/28/06 *Lost work days began 11/28/06.

Injured was unloading cans. He pulled out standard & can rolled on his hand. The tip of his left hand ring finger was shattered and he has two fractures.

7I at 19 wall track switch. EE got off motor and stepped on a wet rock and slipped twisting left knee. He continued to work normal duties until lost work days began on Feb 14, 2007.

Inhalation of radiations, caustics, toxic and noxious substances

Injured was spraying wall with sealant and the hose came loose and he inhaled fumes.

Employee was blocking a shield in 8h and strained rt shoulder.

7I longwall, standing in walkway at #1 shield a piece of rock fell from between straps and struck right ankle.

A longwall move, roof bolting the roof behind the shear, they were ready to set roof jack. A piece of rock fell off shield tip deflected rock from drum and cowl and it struck his left foot causing fracture.

8H longwall, injured was standing mid-shear when a piece of rock fell, hit top of shear and slid into walkway. Rock struck left foot causing fracture to left foot.

A rock fell from between the straps & stuck in the mud. Employee attempted to move the rock. When he put his hands on top of it another piece fell striking his little finger on his rt hand and caused fracture lost work day began 8/4/06.

Employee experienced electric shock/burn when he came into contact with a scoop charger.

Employee was placing a bundle of roof bolts in the tray on the right side of the miner and felt pain in his left lower back.

Injured was winching scoop through mud hole and rock fell from ceiling bruising his neck and shoulders.

Employee was dismounting the cab of the locomotive. As he stepped down to the ground, his right foot stepped on the edge of a track tie causing him to twist his right ankle.

EE was struck by a piece of broken face conveyor flight while observing the Headgate Drive sprocket, E-14 Longwall Section. The face conveyor chain broke, causing a piece of broken flight to be thrown , catching his left forearm between the piece of flight and the coal rib, resulting in a compound fracture to the left forearm.

8I section, miner bolter, a piece of rock was blocking the dust collection door open. He knelt down to remove, rock when a piece fell from between roof strapes and struck right ankle. cost days began 7/14/06 fracture

Employee was pulling an oxygen and acetylene tank on a cart along the #2 3 North beltline and strained his left ankle on uneven bottom. (Employee began missing work due to the injury on 4/19/07 when he had surgery for a torn tendon.)

Employee was installing a metal stopping. While plaicng a piece of foam on top of the stepping he placing a piece of foam on top of edge of the slipped and cut his pinky finger on the edge of the lost work days began 7/3/06 - stures were needed to close.

While hanging bedrails in 10H belt line, employee felt pain in lower abdomen. *7/31/06 diagnosis was a hernia. No lost work days yet.

While rigging up a drill rig, derrick was in horizontal position suspended approx 1' via the gantry rigging. A gussett supporting a wire rope saddle failed allowing the line to slip off. The derrick fell the 1' onto crib blocks. EE was maneuvering the crib block at the time, intending to expose the derrick area in order to paint it. His hand was caught btwn the cribbing & derrick.

Employee was unloading a supply car in the E-tailgate section when he stepped and slipped on a crib block twisting his left ankle and lower leg. Diagnosed as dislocated and fractured left ankle.

Employee removed guard for sump to put in bolt, then stepped into opening and fell into sump.

Injured was walking from track to beltline. Stepping though mandoor, he twisted his ankle, fell and hit knee on rock. His knee was swelling.

4 south track, haulage, he was bonding rails. As he went to stand up, his foot was under a track tee; he tripped and fell onto his right knee. Loss of work days began 5/23/06.

While pulling shields on the F-13 longwall face a rock slid out of the panline and struck employee in the right lower back and he fell onto a shield toe striking his face. Employee sustained a fracture to two ribs.

5I track switch, middle lowboy car jumped track. While jacking car back on track he felt pain in his groin area. 5/22/06 diagnosis was a hernia.

Contact with hot objects or substances

Employee was removing a pipefitting with a oxy/acetylene torch - piece fell as it was cut free, the falling piece struck the torch deflecting it into the employee's right wrist/lower arm

Injured was carrying an oxygen tank on his shoulder while going through a man door. It fell and he tried to stop it and it hit the bottom and shoulder at the same time. He said he heard his shoulder pop.

8I section, using loader to pull up slack cable. He walked back to move cable out of tram road. Rock fell from rib and roof line, striking his left foot, causing fracture to his big toe.

Injured was rolling rail with slate bar when it rolled and hit the other rail and rolled back onto his right foot and bruised it.

9H section, while hanging ventilation tube with slider, he was supporting the tubes with his right arm when the other end fell and he had all the weight on his right arm.

EE was operating the miner in the #1 Entry in the E-Tailgate Section, when a piece of rock fell out from between the straps, striking EE on the right hand causing a laceration requiring 11 sutures.

EMPLOYEE WAS WORKING AT THE TRACK CHUTE, COUPLING MOTOR TO RAIL TRUCKS. EE PLACED HIS RIGHT HAND ON BUMPER CAR & THE END GATE OF THE RAIL CAR DROPPED DOWN ONTO HIS RIGHT HAND.

WHILE WALKING TO THE CAT 825 COMPACTOR, EMPLOYEE STEPPED IN AN IMPRESSION IN THE GROUND MADE BY THE COMPACTORS SHEEPS FOOT DRUM & EE TWISTED HIS LEFT KNEE.

Injured was helping another operator to raise bosco beams to roof. As the TRS was raising, the TRS pad with beam on it tipped causing the beam to slide off and hit his left forearm.

After putting supply cars in #5 run-around, he was walking past cars and tripped on a piece of coal or rock and twisted his right knee.

EE was pushing bolt up into the mine roof while bolting on the left side of the 3 north left miner. Roof bolt caught on roof strapor in bolt hole, causing the bolt to bend, catching his left middle finger between the bolt and atrs. EE sustained a fracture to the tip of his left middle finger. EE was wearing gloves.

EE was using a hammer and chisel to remove a staple from plug on a new leg jack for shield in F-13 longwall set-up face. The plug came out under pressure causing the chisel to strike him in the right hand, resulting in a laceration to his right hand requiring 7 sutures. EE was wearing gloves.

WHILE DRAGGING A 16"" X 10' VENT. TUBE UP TO THE FACE BY HAND, EE WAS TURNING THE TUBE AROUND BY LIFTING ONE END. AS HE TURNED THE TUBE, HE TWISTED HIS LOWER BACK WHILE LIFTING AND PULLED A MUSCLE IN LOWER BACK.

Was scrubbing walls. A basket fell onto her neck and shoulder.

Employee was lowering a discharge line to repair a bad coupler. While lowering line the coupler blew apart, causing water and air under pressure to strike him in the face and both eyes, and threw him back against the coal rib. Employee sustained a fracture to the bottom orbit of the left eye.

Employee was on 7I section track attempting to align couplers on battery pack & locomotive. As he was aligning on the locomotive, the battery pack car moved & his right leg was caught between.

8H section, using a scoop to spread calcium on roadways. The scoop drove over a rail, causing the rail to roll onto co-worker's left ankle.

Injured did not release pressure before removing staple while trying to add hydraulic line. It struck him in nose and left eye.

EMPLOYEE WAS SHOVELING ALONG THE FEED BELT DRIVE, WHILE LIFTING THE MATERIAL UP TO THROW IN ON A BELT HE FELT A PULL IN HIS LEFT SHOULDER. LOST WORK DAYS BEGAN 1/11/06.

EMPLOYEE WAS INSTALLING A COVER FOR THE HEADGATE DRIVE SPROCKET IN THE E-13 LONGWALL SET-UP SECTION. THE EE WAS ASSISTED BY ANOTHER EE THAT WAS USING A PRY BAR TO HELP INSTALL THE COVER IN TO THE CORRECT POSITION. THE PRY BAR SLIPPED AND THE COVER CAUGHT THE EMPLOYEE'S LEFT THUMB TIP BETWEEN THE COVER AND THE DRIVE ASSEMBLY. DIAGNOSED AS A FRACTURE & SUTURES WERE REQUIRED.

Employee was loading supplies on the 4 south right miner. He was passing bolt to a co-worker who wasn't ready to receive them. Employee while in motion had to catch himself and pull bolt back - felt strain in left shoulder. Lost work days began 2-2-06.

EE was hooking up chains to pump second hook hit him on side of head. He seemed ok. He finished the day. EE took off following day then reported his mishap to office personnel. On 4/12/2006 EE felt he needed to see Dr.

Over-exertion in lifting objects

EE aggravated an injury while unloading a control panel at the Bailey Mine. He had been experiencing pain in the abdominal area prior to this incident. The pain worsened over the weekend and ee visited the physicians office on Monday and learned he had a hernia.

Employee was using a 2 lb. sledge hammer to dislodge a stageloader chain link wedged behind the tail sprocket F-14 longwall set-up. As he was strking the link the stageloader chain sprung forward and caught his left hand between the chain and tail sprocket causing a laceration between the left thumb and index finger resulting in 7 sutures to close. He was wearing gloves.

Employee was using a utility knife to cut a piece of 2"" plastic pipe, the knife slipped & cut his right thigh.

Standing on side of scoop fork walking cans around spotting them when pinky finger on right hand got caught between two cans. Scoop was shut off when accident occurred.

Employee was using a ratchet and spud to tighten hex bolt for drum extension on the head of miner. The spud slipped off bolt and his right hand ring finger struck the drum extension causing fracture to right ring finger.

While sliding a roof strap across the miner, the strap got caught on the ear of the plank jack. He turned his head into the edge of the strap causing a laceration - sutures to close.

EE was pulling a shield over top of shearer F-13 Longwall face, when a piece of rock fell from between shields, striking the shearer and then struck him on the left hand, resulting in a fracture to the left ring finger. EE was wearing gloves.

8 I track, preparing to set posts because of bad roof. He used a pick hammer to pull post out of jeep the pick hammer slipped out of post and hammer struck him below his lowey lip causing small cut. *suture to close out

4 south right side, installing an 8' cable bolt, expander caught on strap. He attempted to free up the expander and his hand was pushed into bolter mast.

4 South RT. Section EE tied bolter cable up with a nylon rope.A rib strap was used to place rope behind rib strap & to tie bolter cable up. They activated the cable reel pickup the tension, cut the rope & the cable struck him in the mouth & cut his upper lip. Sutures to close wound.

WHILE CUTTING IN THE #5 ENTRY IN 3 NORTH LEFT, THE UTILITY KNIFE SLIPPED OUT OF THE BELT AND LACERATED EMPLOYEE'S LEFT LEG JUST ABOVE THE KNEE. EMPLOYEE RECEIVED 6 SUTURES TO CLOSE THE WOUND.

Employee was pulled through a Regulator @ the bottom of Weakly Air Shaft. The force of the air caused him to lose his footing causing him to fall to the ground. Resulting in a laceration to his head.

EE WAS CHANGING AN AUGER BIT ON A DRILL STEEL WHEN A PIECE OF ROCK FELL FROM BETWEEN THE STRAPS, HITTING HIS RIGHT MIDDLE FINGER WHICH WAS ON TOP OF THE AUGER BIT. 10 SUTURES.
