Fall to the walkway or working surface

Slipped on black ice when inspecting work site. Fractured right fibula.

Struck by falling object

While sliding a liner bar off of a pallet onto a forklift, the liner fell and it hit his foot.

While positioning the grate to attach the sling to the hoist, the grate tipped and struck employee on the foot. He suffered a hairline fracture of the right foot.

Over-exertion in lifting objects

EMPLOYEE & HIS PARTNER WERE IN THE PROCESS OF LIFTING A POWER WASHER ONTO A CUSHMAN CART FOR TRANSPORT. WHILE LIFTING HE FELT A POP IN HIS SHOULDER AREA. HE RECEIVED MEDICAL TREATMENT & WAS REMOVED FROM WORK - LDW 6/1/04. SURGERY PLANNED FOR 6/17/04.

Over-exertion (Not Elsewhere Classified)

WHILE WORKING ON KILN - EE WAS CLIMBING UP & DOWN MANY STEPS. WHEN HE STEPED DOWN HE TWISTED HIS ANKLE. HE WAS TREATED AND TAKEN OFF WORK FOR 1 DAY. THERE WAS NOTHING ON THE FLOOR TO CAUSE THIS INCIDENT.

WHILE WORKING IN CRUSHER AREA, HE CLOSED THE ELEVATOR DOOR AND CAUGHT HIS LEFT INDEX FINGER BETWEEN THE TOP AND BOTTOM DOORS, HE WAS TREATED/ DIAGNOSED WITH A FRACTURED FINGER. A SPLINT WAS APPLIED.

Lifting a 14"" x 5' wire reinforced hose to stand it on end a piece of the reinforcing wire was protruding through the outside of the hose. The wire poked into the right leg of the employee and he required 3 stitches.

Fall onto or against objects

While exiting the cab of his backhoe he slipped on a step causing a bruise on his left shin. He sought treatment which later required drainage of a remaining hematoma.

Caught in, under or between a moving and a stationary object

While guiding a steel pipe using an overhead crane, the pipe shifted and employee's hand got pinched between the pipe and the steel rack on the truck bed. He received 4 stitches in his left hand.

Employee was grinding metal and felt something in his eye. He went to the Doctor three days later and had a small piece of metal or dirt removed.

WHILE USING A PRY BAR TO REMOVE TROUGHING ROLLERS IT SLIPPED & STRUCK EMPLOYEE IN THE MOUTH. IT LOOSENED HIS FRONT TOOTH AND LATER REQUIRED A ROOT CANAL TO THAT TOOTH. MEDICAL WAS RECEIVED AND DETERMINED TO BE WORK RELATED.

Inhalation of radiations, caustics, toxic and noxious substances

During MSHA screening program a pneumoconiosis was discovered which has been determined to be related to chronic asbestosis exposure.

Over-exertion (Not Elsewhere Classified)

Pain and numbness experienced in hands after using impact tools. It was diagnosed as Carpal Tunnel disease and determined to be work related.
