Accident type, without injuries

Employee was caught in an elevator for 55 minutes. No injury occurred. MSHA was notified immediately.

Contractor Van Damme Farming not involved in normal mining operations was delivering hay at Empire tailings basin. When unloading hay the contractor employee noticed a fire starting near the driver's side door of the truck. His supervisor called 911 at 4:58 pm. Fire depart's arrived at 5:19 pm, put down the initial fire, stayed past midnight to ensure smoldering was out.

Over-exertion in lifting objects

Employee complained of pain in his shoulder after lifting a wheel motor cover off of a haul truck. He was evaluated by a physician and had surgery on 6/22/10, resulting in lost time.

Employee was walking from the loader to the pitman. As he walked up a small berm, he heard his knee pop. He was evaluated by a Physician and had surgery on March 26, 2010 thus resulting in lost time.

Fall to the walkway or working surface

Worker was working in the box of a haul truck. Walking on uneven surface across the box and sprained knee.

Employee strained his back slugging in 19 pebble mill. He was evaluated by Physician taken off from work and a referral was made to another Physician. It was made known on September 3 that it was a lost time injury.

Warehouseman reported a shore left shoulder after lifting boxes. He was evaluated by a Physician and on 10-13-09 Physical Therapy was started.

Caught in, under or between a moving and a stationary object

Employee was driving an electric cart and had his left heel outside the confines of the cart. His left foot got caught between the handrail and the cart, resulting in a fracture.

Employee was boarding a water truck, when his foot slipped off the bottom rung, catching himself with his left arm. He was evaluated by a Physician and had surgery on June 11, 2009, resulting in lost time.

Employee was pouring oil from a 5 gallon can when he felt pain in his lower back. He was evaluated by a Physician and was taken off of work and a referral was made to another Physician. It was made known on 5-14-09 that it was a lost time injury.

Employee was using a hoist to disconnect a vibrating screen from a trailer, resulting in pain to his right wrist. Was evaluated by a Physician and on 4-7-09 had surgery resulting in lost time.

Getting up from the lunch table employee complained of back pain. Was evaluated by a Physican and was taken off of work 2-10-09.

Fall onto or against objects

Employee was in house on a shovel looking for an air leak. When he went to move away, he hit his head on the fan box, fell, and hit his left shoulder on the floor of the shovel. He was evaluated by a physician and physical therapy was started on 1/18/10.

The employee, a loader operator, filed an alleged report stating he jarred his neck. He was evaluated by a Physician on December 3, 2009, and Physical Therapy was started on December 8, 2009.

Employee was descending stairs, slipped on the leading edge and twisted his left ankle. He was evaluated by a Physican and a splint/immobilizer was placed on 11-27-09.

Employee was tightening down bearing caps with an air impact, dirt blew up from the table he was working on and got into his eye. He was evaluated by an Opthalmologist, the FB was removed and he was given prescription eye medications.

Struck against stationary object

Employee struck his elbow on a handrail on 4/16/09, no treatment required. He worked his regular duties thru 9/09 without complaints. He was evaulated by a physician on 10/14/09. Physical therapy was started on 10/21/09. He alleges this current pain is related to the injury on 4/16/09.

Fall to the walkway or working surface

Employee was walking down a small incline, slipped and fell. He put his left hand out to catch himself and resulted in a fracture to his left wrist.
